I really like your videos which are fair and unbiased. I am a subscriber from Australia and have learned a lot about lipid profiles and bio markers. I went on low carb due to HbA1c at 5.8 and IF for a year. All bio markers improved except ldl at 4.16 (Australian unit) My GP was freaked out and referred me to see a cardiologist. I paid out of my own pocket to do a lipoprotein subfraction blood test and the result was most of my cholesterol particles was of large buoyant size. My hdl was 2.6 and my TG was 0.6. I showed this to my cardiologist and he asked me to do a CAC scan and my calcium score is zero. He said see me in two years and no statin. I am 75 non smoker, non FH, and moderate exercise. BMI 21. I take a punch of supplements like CoQ10, cod liver oil, B complex, B1, turmeric, garlic, D3, K2MK7, magnesium and liposomal Vitamin C. Eat two meals a day. No seed oil. Use avocado oil and eat whole food as much as possible. 95% dark chocolate and pecan for snack.

My cardiologist is soooooo old school in this issue AND strongly advised against my Keto/IF journey. I believe I’ve outgrown him.

That’s a dangerous thought to have. Your doctor is basing their opinion on guidelines which were established as a result of factoring in the totality of the evidence. I wouldn’t be so quick to dismiss everything your cardiologist says.

@@Seanonyoutube Too many of them live in the past. I'm 74 & haven't been to a doctor for 5 years I study my own body & have no intention of going to a doctor for quite some time. And for me in my country all medical procedures are free & prescription drugs at my age cost next to nothing!

I've had high LDL for 35 years, 62 now. Got a CAC done, score of 0. Doing LCHF for 2 years for T2D, LDL much higher, HDL much higher, Trig very low. I'm not concerned about LDL at this point.

​@@sheddkkhan6758 At some point each of us is going to die from cronic heart failure... eventually, everyones heart stops beating. I'm 62, hope to make it to 90 like some others have in my family. Heart disease is not in my family tree, I'm not going to stay up at night worrying about my LDL level, especially considering the demonstrated unreliability of using LDL as a predictor of stroke/heartattack. Statins are harmful, worrying is harmful.

@@wesleygordon1645 Yes, heart disease is no fun but in the end, each of us will die when our heart stops. I stay in good shape, healthy weight, and great diet, as prevetative care. The problem I have with LDL is it's unreliable in predicting heart attack and stroke. I won't take statins, or any drug, unless it becomes a matter of life-or-death.

Thank you for recognizing Ezetimibe. My cardiologist took a pretty wild guess IMO after I ceased a statin due to the side effects. She prescribed Ezetimibe. I was ready to look for another cardiologist - she (and my PCP) poopoo'd my low-carb diet event hough my HDL/Triglyceride ratio went from over 3 (on no special diet) to low-carb low of about 1.1 and my LDL lowered to hover around 130. They said that my LDL should be less than 70. I will see the result of Ezetimibe in a month . Other youtube doctors have been skeptical about Ezetimibe but without considering low-carb diet in the remarks. Even though it was anecdotal about Ezetimibe on low-carb, thanks for the encouraging words.

After spending nearly five years worried about my LDL (235 mg/dl) and wondering if I should go on statins, I came to my own decision not to be concerned about it. HDL of 80 and Trigs 40. Blood pressure 110/70. I’m not going to take what is literally a metabolic poison in order to treat a secondary, potential marker of possible disease and not a disease in itself. My genes think I need high LDL, I don’t have FH, I otherwise feel better than ever on an extremely low-carb diet. Why do my doctors think they know how to optimize a human body better than 4.5 million years of evolution?

Yes, evolution is a fact and Darwinian Natural Selection is the driving principle. Of course all life could evolve from a single microbe. You did it yourself and it only took you nine months.

The only power it has is to kill an organism before it reproduces. That's a clumsy club compared to the random muta_ions that introduce about 100 new nucleotide changes (mostly damaging) per human birth, and similar numbers in other organisms. Do you seriously think such unbalanced forces could design the fabulously intricate molecular machinery in today's life? Can you defend the evolutionary origin of any new protein along the way from microbe to man? Either a specific protein or proteins in general? Do you know what a protein is?

It *_develops_* according to that genome in nine months. That's _extremely_ different than they hypothetical *_evolution_* of the genome from one class, order or family to another.

Dr. Sher is an excellent, articulate presenter of information that, to this layman, really helps make sense of my particular health journey. This one was a little out of my league ("lean mass hyperresponder" what? Sounds like something from a scifi movie). So I'm going to educate myself with the articles on their site, which are always useful. New cardiologist mentioned I might need statins, pending CAC and other tests. But I'm skeptical, and frankly, not too worried about my latest numbers (which have been trending down): Total chol: 198 HDL: 44 LDL: 129 Trg: 123

But have to say, Am in a tailspin after watching interview regarding Women’s Health secondary ‘look thru’ conclusion about LDL. I've been Keto for almost 2 years. I recently had a lipid panel done and, not surprisingly, I have High HDL, Low Triglycerides but extremely high LDL, so I likey have FH. I was referred to a lipidologist, who must have been over 80 years old. All the info he gave me encouraged "Low-Fat" options, including replacing saturated fat with Polyunsaturated Oils as well as including Oatmeal and Whole Grains in my diet. Evidently this octogenarian doctor is not exactly "up-to-date" with anything low-carb related.

I've heard a couple of speakers say that higher LDL correlates with reduced mortality in folks over 70 in some big studies but it hasn't been mentioned here

4:06 "The size of the LDL went down." Nope. Misspoke? 6:15 "The majority of people with high LDL likely have FH." Nope. FH can only be diagnosed by genetic testing, not by lazy Doctors simply checking a box because of a high LDL reading. This was a difficult video to follow, just like the three attachments are very detailed and referenced. A person new to this issue may have problems.

Yeah, my cardiologist took one look at my elevated LDL and concluded that I am FH. Turned a deaf ear when I told him my cholesterol had always been normal until I started LCHF diet when I turn 59! Very infuriating.

LDL is just a marker of different metabolic processes. One of them is the atherosclerotic dyslipidaemia (high TG, low HDL, high LDL/TC) caused by high insulin. The other is a state of fat oxidation. Both are completely different.

I suggested Ezetimibe to my GP who is eager to put me on statin and he agreed to try for 6 weeks. My Ldl has been going up since I stopped talking Statin and starting a low carb diet + IF to reverse diabetes and while I've done that very successfully my LDL has gone from a normal level to 6.8 mmol/L (UK units). I've noticed that increasing red meat has also increased my triglyceride and while its value is still in range the ratio triglycerides over HDL is not as good as it used to be. HDL is better than ever on my last blood test results at 2.1 mmol/L or 86 mg/dL. I stopped taking statin because I was in a lot of pain and this pain was all over my body and it didn't come all at once so it was difficult to put my finger on one thing that caused it, therefore eliminating statin was the option that I took. 90% of the pain went away gradually and the rest I attribute it to old age...however, I not longer feel like I've done 10 rounds of boxing every morning and I'd like to keep it this way. Thank you Dr. for the heads up on Ezetimibe.

I have been trying to research this for myself (Statin or no Statin?) for the past year and this topic seems complicated and there are many and often conflicting opinions on the internet. As I understand things, you need to go beyond the LDL count to fully understand ones risk - Genes, family history, Inflammation, Insulin resistance, ApoB, Apo(a), Oxidation, Glycation etc etc. I also follow Ford Brewer on KZbin. He does not recommend statins for primary unless you have soft plaque. He also recommends low dose statins if you have inflammation to reduce risk.

I wanted to lower my LDL, even took statins (felt really crappy). Then I figured it's difficult to have low LDL on 70% keto(especially if you're athletic), because 70-75% of your energy are fats. So, if you remove LDL by statins what would be transporting those fats to organs and tissues? You just cut the supply of energy to your body. And if you have higher energy requirements - higher is your need for LDL to spread the energy stored in fat around your body.

I heard the same thing from Dr. Ewes. How else would the body move fat for energy? If your LDL isn’t higher you aren’t doing keto right. People who halfway do the keto lifestyle are the ones with acceptable levels.

I ignore all LDL focused studies. The angle on almost all of them is to land on the conclusion, see you need a statin. I don’t see any studies that show success driving down that one marker and looking at side effects and long term damage to the body and brain. There’s no money in it.

Dr. Paul Mason recently discussed two studies whose results sat unpublished in researchers' basements for decades because the results were _"disappointing",_ meaning they showed higher LDLs meant _lower_ risk of death, not higher.

Thank you doctor, excellent analysis and review! I thoroughly enjoy your channel. Just a thought here when you say "clearly higher LDL indicates more risk". I think this language is too vague for the situation at hand, because you aren't saying "higher" than what level. I believe it has also been shown that LDL lower than 80 or so is associated with increased all cause mortality and CVD. Also Dr. Lustgarten has a video where he shows all cause mortality increases only approximately above LDL 140. So there is a range of LDL, maybe 80-140, where all cause mortality is about equal and lowest. That would indicate a more qualified statement (as an example) would be "clearly LDL when it becomes higher than about 140 is associated with higher all cause mortality". This sort of statement would help those of us on Keto who saw their LDL go from say 110 to 140, while their doctor is saying OMG what happened?

I have normal LDL at the same time I have heart falilure was rushed to the hospital because of mild heart attack, and brought to ICU for three days. I am now taking 7 pills but still i am not ok and always feel dizzy. I used to eat vegetabes with low salt and eat a lots of sweet fruits and low salt in my food no changes at all. But when I started to have Keto diet just for two weeks my body is alert and my mind is on focused, And already walk fast. Good sleep and always happy.

Began this journey after blood work showed I was pre-diabetic. Many sources tell me that most “sweet fruits” are a real problem, due to high fructose levels. Your focus on mechanisms is important. That same focus should be applied to the basic question: does LDL increase the risk of cardiovascular disease? You can cite study-based associations until the cows come home, but unless the association is very strong (multiple times increased risk, not something like 50% increased relative risk), then you need to be explaining the mechanism to have any hope of effective treatment. And the only people I have ever seen explain the mechanism of how LDL causes cardiovascular disease are the low carb people. The proponents of the lipid hypothesis never do, they just fall back on "high LDL bad, take a statin". You can be this lazy when you are part of the dominant paradigm, because the only people who will call you out can be dismissed as fringe lunatics. As it happens, the mechanism is known. Cardiovascular disease is caused by damaged LDL, and the damaged particles are smaller and more dense, and lose their affinity to the liver LDL receptors, and thus accumulate in the bloodstream, leading to a high LDL number in a standard blood test. There are several mechanisms of LDL damage, the two most prominent are glycation and oxidative stress. However, there are other mechanisms that lead to a high LDL number by increasing the number of healthy, undamaged LDL particles, which have nothing to do with cardiovascular disease. This is why total LDL is irrelevant. You need the particle count or particle size (the two obviously correlate, smaller particles, higher count, and vice versa). Or you need to use other indicators, two which are on a standard blood test are HDL and triglycerides. Low HDL and/or high triglycerides will almost always occur in the presence of damaged LDL particles. The most powerful predictor is the ratio of triglycerides to HDL. And the simple fact is statins can do nothing about either of these things. And indeed, they also do nothing about damaged LDL, as their mechanism is to up-regulate LDL receptor expression in the liver, meaning more of the healthy LDL particles will be removed, and nothing whatsoever will be done about the damaged LDL particles that are the actual cause of the problem. Statins are thus utterly useless, unless you're a pharmaceutical company making a huge profit from the sale of them. Unless some medication can specifically target the damaged LDL particles, the only solution to the problem is to avoid creating them, which means altering your diet and lifestyle to remove the stressors (glucose, i.e. sugar and carbs, and oxidised polyunsaturated fatty acids, chiefly from seed oils; also exercise, which strengthens your cardiovascular system and makes your blood vessels less susceptible to being damaged and accumulating the damaged LDL particles as plaque). Note that cholesterol itself is not a stressor of any kind, and also that cholesterol is not the same thing as a lipoprotein (the L on the end of HDL, LDL, VLDL, IDL), rather lipoproteins are carriers of cholesterol (and other lipophilic and hydrophobic molecules). Why scientists and doctors persist in this mislabeling, which is present on every blood test, completely escapes me.

Perfect. I'm lean mass hyperresponder. I suspect this phenomenon occurs due to a very low blood insulin levels, that's why we are lean. But, without enough blood insulin levels, the liver insulin receptors are not fully activated, and, consequently, the liver stops to filter more actively LDL in the blood, letting LDL levels fluctuate higher. Is it dangerous? I believe that, in general, it is not. But it's necessary to follow-up the low grade inflammation biomarkers and the carotid intima thickness. Low carb diets are in general good to lower inflammation, but it is still kind of tricky, since low grade inflammation is not fully understood by science so far.

I heard about that theory. Low insulin level causes down-regulate insulin receptor, at the same time caused down-regulate of LDL receptor. That’s why LDL is high. But why is it only for those with BMI under 25? Because those above 25 is more likely insulin-resistant?

I have a slightly different question -- high HDL and a high HDL/triglyceride ratio are claimed to be more powerful markers for CV health than the various LDL markers. Is this true? If so, why are these markers always ignored when LDL markers are high?

Not true. The totality of the evidence (not cherry picked studies) indicate that while triglycerides and HDL levels correlate with general metabolic health, LDL, or more specifically, Apo-b, is an independent risk factor for heart disease, and risk does *not* come down with favorable HDL/TGs.

Been doing keto for about 8 months after blood tests showed I was pre-diabetic. Have learned my real problem is insulin resistance - all info I have found completely matches my circumstances and history. My research has led me to several sources discussing your question. I think it has something to do with LDL shuttling around fat for fuel. My vldl is 8 but LDL 206, particle count 1969, but less than 90 are small particles. What keeps me scratching my head is up to the point of heart attack 50 % have low to normal LDL. Does this mean it doesn't matter what your LDL is? The key to this is study is the LDL level at the time of heart attack or stroke. That's where we will get the answer. Statins lower LDL, but in some people, increase the blood sugar to the point of type 2 diabetes. Which is worse LDL or type 2 diabetes. Not mention cognitive impairment. I know someone who experienced this. Can't remember why she left the house, put her car keys in the freezer etc. Drs chalked this up to old age age and febleness. But, why did this all improve when they took her off the statins?

If you think about it, it makes perfect sense. Your LDL drops suddenly probably because your immune system is attacking that oxdizied LDL (wich was previously the small and perhaps medium LDL) and then your body just clogs your arteries in your heart to stop that LDL and after that the thing wich covers the oxidized LDL and therefore it dosen't detect it... it only detects the rest of the LDL.

It is know lipidphiliac statins cause brain fog and dementia risks. But hydrophilic statins do not. Searchyou will find different statins in the two categories

I wish all these sites would settle on one definition for the term "particle". Are you referring to remanent particles or particles of sdLDL or oxidized or what. Also, how can vldl and idl be armaments ( leftovers) if they are upstream of smaller ldl ?

Okay here is what I was told. The arteries get damaged, then cholesterol sticks to it. Vegetable oils do nothing to prevent heart disease, but they lower ldl.saturated fat may raise both ldl and hdl. Saturated fat does not cause heart disease. I had no risk factors except high ldl. I had a heart attack. The only thing I had was high particle size. My ldl went down. I do not eat refined carbohydrates or sugar. One of my relatives had terrible cholesterol and never had any heart disease. She died at 87. I do not have hypercholesterolemia. Imo people need to focus on what is damaging the arteries. The statin drugs lower inflammation. The drugs do work in several ways. Imo simply manipulating cholesterol with food, does not do much. I was 55 and had a heart attack. Stop focusing on ldl and work on what damages the arteries.

'Eventualities are eventual and random things happen to random people'. What I mean by that silly nonsense is that there is a 100% chance our heart will eventually attack and kill us. Some people are incredibly healthy and their brain essentially short-circuits at 30 years old. My point is, as I am echoing from Dr. Peter Attia, don't make your death as slow/long as your life. If you feel phenomenal and die suddenly for seemingly no discernable reason, well that's the way I want to go, how about you?

You literally said you had high LDL and a heart attack. That confirms what the guidelines say: LDL is an _independent_ risk factor for heart disease.

I took Zetia when my LDL was high about 25 years ago. But soon switched to statin. I took myself off statins in February 2022. My doctor was not happy, and tried to convince me to start taking them again by using a study from 2000. I gave her newer studies and she said she would read them. But at my next visit several weeks later she said she had not read them, and she did not read at home. BUT she sees patients all day at work so she obviously does not read at all. I have an appointment with a new doctor in about 4 weeks. I will be asking him about Zetia. I was KETO from June 2021 to April 3, 2022. Carnivore since. LDL was 272 in May. All other markers were excellent. Zero Calcium score in March, same 20 years ago.

Here's a problem. As a lean mass hyperresponder, I have very high LDL cholesterol, so I have very high LDL particle numbers and very high apo-B. Because 1 apo-B exists for each LDL particle, I will automatically have very high apo-B's despite having a triglyceride/HDL ratio of about 0.4. This makes apo-B a poor indicator of cardiovascular risk, especially if all the other inflammatory risk markers such as c-peptide, c reactive protein, Lipoprotein (a) are low. Add a low fasting insulin (4.2) and appropriate lipoprotein size for insulin sensitivity, and I think that apo-B and total LDL don't mean very much in my case. Let's hope so.

you need VLDL, but it's estimated by the lab. Doesn't make sense that they gave you HDL and LDL, but not total cholesterol.

I can access my blood test reports thru the patient portal of my primary care physician. From lab work done this past Monday, my “Lipid Panel” results include: Chol = total cholesterol Trig = triglycerides HDL = HDL cholesterol LDL-C = LDL cholesterol NON-HDL-C - this equals total chol. Minus HDL. Consists of LDL and VLDL, or “remnant” cholesterol, which is not separately listed I found helpful in understanding these two videos by Dr Rob Cywes The Carb Addiction Doc, Episodes 5 and 181. Can point you to some additional videos if desired - post a reply and KZbin will let me know. Good luck!

Thanks for sharing. I started out with high LDL(200), low HDL(37), high Tg(180) before Keto. After Keto for more than 6 months, my LDL(220), HDL(48), Tg(65). My LDL seems to stay high, but other markers going good. I suppose I am lower risk than before, but still want to lower LDL just to have a peace of mind. I am trying to do Mediterranean Keto eating seafood as main protein/fat source, and occasion red meat to cut down saturated fat. I will see how it goes. If it still can’t reduce LDL, I will do another experiment with higher carb like an small sweet potato per meal, although I don’t think low carb is the reason for my high LDL since I had high LDL with my old high fat high carb diet.

You should be very pleased - clearly lower risk. The risk from high Tg's is very under appreciated. But to further define risk, you should measure Lp(a) especially, and LDL particle size and number, measure inflammatory markers like hsCRP etc and of course have a CT angiogram. If your CACTA is completely normal, then no further major changes are required. But if abnormal, then one cant take comfort from even "normal" results, and more drastic action may be required. Also, you didn't state your age and wether you have abdominal (visceral) obesity. The importance of avoiding "over fatness" and visceral obesity cannot be over stated .... these conditions are at the core of the metabolic syndrome and cause inflammation in all your organs aka fatty (inflamed) liver means also fatty pancreas, fatty heart, fatty muscles. Coincidentally, this inflammation is a risk factor for COVID-19. So, whats your waist to height ratio?

@@garybranch4383 Thanks for the feedback. I am clearly super high risk prior keto. I am now 39 yrs old, Waist 30”(was 35) and 5’8” in height. My weight was 210lb prior keto, and I had fatty liver, A1C 5.8. I am now 145lb. No fatty liver, with body fat around 13%. A1C 5.2. I clearly had metabolic syndrome. I didn’t check fasting insulin prior Keto, but two months ago, I tested at 9.5(still not good). I also did a NMR and LP(a) two months ago and also turned out bad. Only good thing of NMR is my particle size is pattern A. LP-IR score also suggest a little insulin resistant. LP(a) also high around 190. Both NMR and LP(a) was done to test myself with much higher saturated fat. It turns out HDL was highest(58), but LdL(290) was highest too. Doctor say I have FH and need medication, but I am giving myself two more experiments(higher carb or less saturated fat) before taking any cholesterol lowering drug. I will incorporate more lab tests next time to access more risk factors. Sadly those tests would have to be out of pocket since my Kaiser doctor just wanna give me statin.

@@tktkwei i’m more or less in the same boat as you: 30 yo, LDL 200, T2 diabetic in remission and running some experiments to lower cholesterol before accepting a statin. Instead of adding sweet potato, I’m adding lentils and beans which will both give me more protein (allowing me to lower animal products which contain saturated fat & cholesterol) while also giving me lots of soluble fiber which tends to lower LDL. I can report back if you’re curious and would be curious to hear your results as well.

@@Seanonyoutube Yeah, I would definitely report back when I get my next blood work done probably in a few weeks. I am currently cutting down most saturated fat and replacing it with seafood(5 days of fish/shrimp dinner). If I eat chicken, I only eat breast, no skin. I tried to keep red meat off mostly. I still use a little butter for flavor when cooking. Increasing carb would be my next test if my current diet doesn’t work.

You’re about the only one I could find who recommends Zetia. All the research I’ve found agrees that it lowers cholesterol but is doesn’t have any arterial benefits. What is your source?

I remember that when Zetia-- or some other drug that stopped BOTH cholesterol production AND cholesterol absorption-- came out, they had to stop administering it to people because they were dying of hemorrhagic strokes and heart attacks (caused by bleeding, not vascular blockage). It turned out that, in the absence of adequate amounts of cholesterol to do the arterial patchwork that nature designed it to do, people were hemorrhaging to death. So a few doctors found out what cholesterol was for. I saved some articles about this when it happened, but when I go on the web now, it's hard to find these old articles. This happened in the last 10 or 15 years.

I do wonder if it all comes down to the LDL context. LDL in a body full of inflammation caused by a poor diet (and in activity) is likely to be worse that LDL in a body fueled with a healthy wholefood low carb diet.

The correlation between apoB and non-HDL cholesterol is like 0.9. Just subtract HDL-C from total cholesterol. It basically captures all the ApoB related lipoproteins.

I cut my carb intake from mid 300s to about 120-160 per day and had a modest increase in fat intake, for the first time my hdl is in range at 48 but my triglycerides about doubled along with my ldl. So strange

Dr. Bret Scher, I would like to share my story. My doctor for the first time, before reading my test results opened his eyes very big, watched me and said, "your A1c is good, your cholesterol levels are very good, HDL, LDL, and Triglycerides everything is good so keep doing exactly what you're doing and see me in 6 months". I was expecting bad results so I was shocked because I had once again started consuming coffee with cream, and using butter, cheese and eggs on a daily basis; I hadn't done that in many years since I heard that they were high in cholesterol. I sometimes ate mixed salads (colours of the rainbow) made with lemon juice, acv, olive oil and blackpepper; I had it with eggs/meat/seafood. I also ate mixed steamed vegetables (colours of the rainbow) tossed in olive oil/butter and blackpepper with eggs/meat/seafood. I don't eat beef and neither pork, but I do eat eggs, chicken, duck, turkey, goat, lamb, tuna, sardines, crab, oyster, mussels, lobster, and salmon, that's it! I'm continuing to do exactly what am doing until my next blood test then I can make changes if, and accordingly. Dr. Bret, I may not practice many things that you'll said but am learning a lot from you and other doctors on KZbin, NOT from my doctor! Thank you all and God bless🙏 ❤

You may have your belives in what to eat and not to eat, but i has exactly zero to do with your extremely important colesterol levels! I got one thing to say to you. Do you realy think the body makes and ensyme who is so higly important to the function of the body, that it damage the body itself? It's so stupid, and it's all about holding on to this idiotic hypothesis, that make billions of dollars every year! Please use your commen sense. I've been on Keto for about 20 months. I started when I was diagnosed with prostate cancer. I'm 58. I lowered my weight from 207 to about 190. My blood work from last month has my doctor very upset. My A1C was 54. My triglycerides were 105, HDL 62 and my LDL was 200. I'm bothered that my LDL small particle was 1034. I guess that I'm a hyper responder. Also in Jan of 19 I had a zero calcium score. My blood pressure is also pretty good as it averages about 125-30 over 80-85. However, I am concerned about my small particle LDL being so high and its actually causing me a great deal of stress. Can anyone offer some advice. I don't want to go on a statin but of course I don't want to die either.

I am a medical intern soon to be M.D. and a scientific researcher. There are still on-going trials on the relationship between the type of fats and particle sizes. No conclusion yet to be declared. To be able to give you a medical advice, I need to study your case personally but as a non-medical advice that I have experienced firsthand from my own patients. First, replace butter with olive oil. We still don’t know how butter affects LDL size for sure, but we know that olive oil polyphenols decrease the size of the particles. Second, Alternate day fasting. These two general pieces of advice that I gave to you, have been proven to decrease the size of LDL. Let me know if you need further help, Sir.

@@franksemmes1633 I follow all of those good people as well. My problem is that my small particle LDL are thru the roof (1030) and the consensus seems to be that this is dangerous. Also after 19 months of strict Keto I am still insulin resistant. I can't believe it. I feel that at this point the videos by these guys can only take me so far. I need some one on one time with a keto minded doctor to go over my numbers and try to make some sense out of them. I'm in the Dallas area and there doesn't seem to be anyone who practices this sort of medicine.

I'm still with Dr David Diamond on this. BTW, LPa is great, Trigs/HDL is great, and LDL is only a little high, so I'm still not going to worry about it, and my GP (and you know my GP-co-wrote "Eat Rich, Live Long," co-hosts Low Carb Denver) says the same.

I recently learned that statins can increase coronary calcium scores. Now I’m trying to understand what that means for me given my 10+ years on statins and my now non-zero CAC score. Could the statins have caused it?

Hmm, usually I agree with videos here, but at 8:37 all I have followed so far from other surces, high LDL does not correlate to CVD at all. Many CVD patients that got stroke had completely normal LDL. Actually, with elders it is even desirable to have higher LDL for longer lifespan.

I dont know which sources you are relying, but that's wrong. High LDL does correlate highly with CVD. With high LDL, Risk of heart attack or CVD is only lower when your HDL is good, but that's not the case usually.

I really appreciate this video. Have generic Zetia I haven’t used yet. Just got a repeat Lipoprotein profile done and waiting for results. The one from 2 years ago looked good. Hoping this one is good too.will use gen Zetia if , since it doesn’t work in the liver, if this test not good. Still Low Carb.

Beware. Zetia increases your small dense ldl. My spouse took zetia and went from pattern a to b. Plenty of studies say this is the effect. It reduces your overall ldl by nixing your large fluffy ldl.

My opinion on LDL it can be used as a screening marker but not for clinical decision. Once a patient is diagnosed with high LDL, a thorough assessment of his/her metabolic health should be done, before prescribing any drug. Primum non nocere.

What about those who have a major heart event before like a bypass surgery? Is taking statin critical to this group of people and does a high LDL matter? I realise a lot of these conversations have overlooked this category of people with prior major heart events. I hope someone can weigh in on this concern.

If u have coronary calcium, stents, bypass or other procedures, you are in the only class of people who can potentially benefit from a statin and aspirin. But you still have to try to figure out why you have ongoing disease and stamp out whatever is continuing to drive that. That's the only solution. The aspirin and statin only help mitigate ongoing damage.

Every test I have seen shows lowering LDL doesn’t improve all cause mortality. Some appear to show it actually worse. Look it up. Can anyone find actual data showing life was extended by taking a statin? If so I have not seen it.

Just got my blood results and my cholesterol is way off the « normal » chart. Not sure how to react since it’s way up from a year ago. Total Cholesterol is at 560.6, HDL is at 74.1 and LDL is at a high of 464.5. Have been on paleo and keto for the last 7 years….should I be concern and worried…

-High- LDL correlates with cardiac risk, because everyone has LDL. Plenty of studies show that as many with supposed low LDL suffer cardiac events as high LDL and that lowering cholesterol has a neglible outcome and that high LDL is associated with longevity. It's very easy to scare people into taking a drug they don't need.

The question I have as someone with high LDL is should that high LDL be treated. Statins have such a minimal effect on people with no pre-existing heart problems and such a long list of side effects that I have been very reluctant to take them despite pressure from my doctor. For context I am fit and healthy with high HDL, low trigs, low blood pressure on a low carb diet. I also have a zero CAC score.

Looks like you've got it covered. I've got the same good numbers as you (lean-mass hyper-responder) but had a heart attack two years ago and since on 20mg Lipitor. I doubt the usefulness of that statin. I hope to heal by a low carb diet that is above all NON-INFLAMMATORY.

Nailed it Dr.! Exactly what I’m dealing with. I’m 61, metabolically healthy and super high CAC score. When going low carb my ldl shot up and my doc says crestor and aspirin. If I didn’t have the high CAC I would not even consider starting a statin and would just eat a more Mediterranean diet. But with my score so high I guess I’m going to have to start the statin regimen. I sure wish I didn’t have to! Statins scare me, but I’m kinda scared not to. Very confused and much worry.

I am glad you addressed FH as I have had a history of high ldl over 200 and I have a high CAC score. I had been confused about this and had was concerned that my cardiologist was not treating me appropriately. This clarifies that he is likely treating me appropriately given that I am on a statin and ezetimibe. I think the low carb community has to be much more circumspect in describing FH as an exception, especially with people with a high CAC score.

6 months ago my LDL was 153 , I changed up my diet from 30 net carbs to 20 . I focused on avacado , butter , olive oil , heavy cream. Today I got the results from my recent LDL . To my surprise it was 78!

When you were eating the 30 net carbs, what were your carb sources? And did your fats change in ratio before/after initial LDL results? Also, any other changes? Supplaments? Exercise?

@@Seanonyoutube I've been on keto for 7 years.. more of a dirty keto . I've always taken supplements ( multivitamins, vitamin D3 , Turmeric, fish oil , apple cider vinegar and probiotics. In the beginning my LDL was always in the 110 range. It was getting increasingly higher the last few years. Not dangerously of course. Initially everything was whacked out. I weighed 186 pounds more , had an A1C of 12.1. Currently I have that at 5.2 and got off everything except metformin which is next. I exercised 4 days a week just focusing on cardio which shredded the weight very effectively, Over the passed few years I now do cardio followed by light weight / reps / super sets / 2 connecting muscles except leg day

@@charliehowell6015 I’m specifically interested in the changes you made to go from 153 to 78. You mentioned decreasing carbs. Is that the only change you made in your life? Did you increase fiber? Change up some foods? Anything of that sort? I’m in the same boat, on lowish carbs (not true keto) and my LDL is 200 and i’m looking for natural ways to bring it down.

It amazes me that we really have no clue what goes on in our bodies. Everything is always should, could, might, may, we think, it's believed to be. I can't wait till their is an actual blue print to the human body. You can have to groups interpret the same study and come up with 2 opposite opinions. If you feel good about what your doing then keep doing it. It's almost like you have to bet your life on it. Always be open to change. Don't turn your lifestyle into a religion and put your common sense blinders on.

And another cause for high LDL that could leade to increased risk of heart disease is combining saturated fat with something like trans fats, or glucose/fructose, or certain chemicals that the food industry adds to their food and this could be the real cause of bad LDL causing heart disease.

Are hyper responders only hyper during the weight loss phase? It’s really weird there are hundreds (thousands?) of youtubes and articles about losing weight. You lose the weight and then what? Does everyone just start eating carbs again and then lose weight again. What do you do when you don’t want to lose weight, just add a piece of fruit-for one daily spike? What is healthy weight maintenance? If you have a positive CAC score, the doctor will automatically prescribe statins. No discussion of life style changes…. Statins do not reverse CVD.

I’m doing LCHF because of pre-diabetic. I don’t need to lose weight. I stay away from food that can spike my blood sugar. Eat mostly fat, protein and veggies. I use time restricted eating and the amount of food I eat to control my weight so it stays roughly the same level.

And you still had a heart attack at 55? Did you have any weight issues on that high carb diet? Or did you have metabolic syndrome?

Maybe the previous diet damaged your arteries and they are slowly healing? A calcium score would help you know how much damage you have and repeating the test after a couple of years might show if you have improved vascular health.

@@isabellelee2658 the cardiologist says that test is not recommended once a person is on statins because statins pack down the calcium in the arteries. Possibly that low fat diet creates oxidized cholesterol. It has to be oxidized cholesterol that is the problem. Sugar, and omega 6 oils oxidize cholesterol.

@@TheShumoby I am normal weight, a1c 5.4, low crp, fasting sugar 85. I exercised for 30 plus years. Everyone knows me at my gym. My blood pressure was perfect. My cholesterol was 229. I did an advanced lipid profile. Hdl was good, low triglycerides. I did not have apo a or b. My ldl particle number was high. My parents had heart disease in their 70s. My sister is much older than I and she does not have heart disease. I had my right artery completely closed, my left 90 percent closed. I now have 4 stents. I exercised the day before the heart attack.

Interesting, since this topic is on my radar screen right now. I'm on statins, 40 mg. My values were so good the doctor suggested to go to 20 mg. But first she took my BP and it was 140/90. So she wanted me to stay on 40 mg. My reaction was to go to LCHF 6 months ago and now BP is a consistent 115/75 with a weight loss of 9 kg (4 lbs). Just this week had a new lipid test on my LCHF and the cholesterol increased from 150 to 200. TG and HDL unchanged but the LDL have increased from 85 to 120 (hyperresponding?). However APO B is a measly 0,79(good) and my APO B/A is 0,41 (good). Still I want to reduce my LDL so I'm thinking of substituting butter and cream for more olive oil and such. Think it will do the trick to get the LDL down to impress my doc?!

Yer but, yer but .... whats your CT angiogram show ?? This will give you a CAC score and tell you if you got any soft plaque. Why presume making minor changes will affect outcome if one has gross silent CAD ?? If you don't look you don't see. Your most significant change is your weight loss, which probably significantly reduced your visceral obesity (fatty liver). When all you numbers are good, the rise in LDL is probably insignificant, and you'd be better served by increasing muscle mass and further reducing fat mass ... i.e. becoming leaner and stronger.

Hi Mats. I am a 67 year old female who was prediabetic. Went keto about 3 years ago. Seeing an excellent endocrinologist for thorough metabolic workups every 4 months. I also have hypothyroidism. (not Hashimoto's Disease which is an autoimmune disease) and take a prescription dessicated thyroid hormone derived from a porcine source. Hypothyroidism can raise cholesterol levels. Bottom line: this past June 2021 total cholesterol soared to 330 with LDL at 203. Decided to cut out all extra fat except what I consume from 100% grass fed ruminant animals, fatty fish, eggs. Only cook now with olive oil or avocado oil, occasionally grass fed butter. Stopped using coconut oil, beef tallow, duck fat for cooking fats. Cut down on healthy fat snacks and now go for the protein snack, also sunflower seeds, pumpkin seeds, macadamia nuts, pecans, pistachios a few handfuls at a time. These nuts and seeds are fine to consume but avoid ALL commercial seed oils such as sunflower, safflower, canola, peanut, corn, etc.. Jan 18, 2022 blood work: Total cholesterol now dropped to 255; LDL dropped to 158; triglycerides were always good and range from 50-70. Fasting glucose, A1c, fasting insulin and C peptide all in good value range. I consume 20-40 grams at most of carbs in a day. I do not include low carb veggies in my daily carb load. I live in the U.S. Got a CAC test last April. Showed mild coronary artery disease in 1 major coronary artery. This would be from past damage due to high carb diet and possibly other factors such as heavy metal load (mercury, arsenic, lead in body tissues) which practically everyone has. Check to see if you are possibly over consuming fats, even healthy fats. It may be a bit much for your liver to handle and may be affecting cholesterol levels/liver function. Get fats from grass fed/organic animals as much as possible. You may wish to get a full metabolic workup: all hormones, all blood sugar factors mentioned above,. vitamin D, RBC (red blood cell) magnesium, vitamin B12, iron, TIBC, serum ferritin. You can get an NMR (nuclear magnetic resonance) test that will analyze your LDL further to determine the size and particle numbers. A total cholesterol reading is not a great indicator of cardiovascular health as 50% of those suffering a heart attack have normal or below normal cholesterol levels. Suggest you also check. CRP, homocysteine, fibrinogen levels if you have not done so. Hope this is helpful. Wishing you excellent health.

@@garybranch4383 Thnx Gary. Yeah, the background is I had a TIA 2018 and been on statins since. For me its about taking babysteps to wean them off, as I dont want to discard any risk with it. So my increase in dietary fats the last 6 months and the corresponding rise in good ol' LDL makes me a bit worried, and might so also for my prescribing doc. Even if rest of the key values are good. Something dr Scher also mentions in the clip. Anyway thnx. By the way, my CT scans have been quite OK so its probably fine with LCHF for me.

Does a low carb high fat diet (KETO) cause the liver to quit making cholesterol because it's in the diet? Does the liver then quit up taking LDL?

Not really. Low carb lowers insulin needs. Low insulin doesn’t nudge the liver to recycle ldl as much. This is why he suggests carb cycling as one solution if the numbers bother you.

​@@andienantz I don't know if my high LDL numbers should bother me. My calcium score was 7. I don't want to solve a non-problem. My primary concern is to reduce my A1C, which is now 5.5 (my target under 5). My pre-diabetes, insulin resistance, fatty liver, obesity, heart attack risk, etc. should follow. I've lost over 35 lbs in the last 6 months. Here's my Lipoprofile NMR test: LDL-P (LDL Particle Number) 1,921 nmol/L 39 mg/dL Triglycerides 75 mg/dL 0 - 149 mg/dL Cholesterol 231 mg/dL 100 - 199 mg/dL H HDL-P (TOTAL) 35.0 umol/L >=30.5 umol/L Small LDL-P 643 nmol/L 20.5 nm LIPOPROTEIN INSULIN RESISTANCE SCORE.SER/PLAS.QN 29

I am going off Keto. It has been an interesting ride to say the least. My LDL-P and small LDL-P are off the chart high and so is my apoB also off the chart high. Did you get that? I hope you did. You really want to know their values? Over 3500, over 1150 and 235 respectively. Any arguments? Check yours. Go and get an NMR lipoprofile. One diet does not work for everybody. Do not be a fool. Experiment. The best diet in my perspective is this: I won't tell you what diet to follow, I can tell you what not to eat. DO NOT EAT JUNK FOOD AND YOU KNOW WHAT I AM TALKING ABOUT. YES REFINED CARBS ARE JUNK TOO. the list is long.

Putting more ground flax in my food & taking 5g chlorella in 1oz of purified water everyday (first thing am) has helped me. I refused the Coumadin & did my own research. When I went back & my levels were down so much my doctor asked with folded arms, “What did you do?” 😂

You kind of danced around the satins thing. I supposed if you were really unhealthy with multiple Comorbidity’s then yeah I suppose it’s one way I suppose to help bring your health back into alignment quickly? But I’ve seen relatives who have taken statins and other supposedly “life saving pharmaceuticals” for over twenty years and watched their health diminish by the side effects of those statins and other crap’! The doctors who prescribe those drugs can not keep all those pharmaceuticals side effects straight of each other especially when they throw this pharmaceutical and that pharmaceutical at people who’s memory are being effected by all those drugs! None of those doctors ever talk about diet! But if that is what caused those Comorbidity’s to happen in the first and your diet is not changed then I believe you would be fighting an uphill battle? People trust their docs and will not think for themselves of those side effects that come with them! They just line up and hold their hands out or roll up their sleeves and get the jab! No questions asked. So I forgot who supports most of medical schools? Oh yeah, it those pharmaceutical same companies! The critical thinking by patients and doctors have gone out the window in this country. Not all but many. Sadly It all boils down to the buck anyway;( Im I wrong about all this? Or do I owe you an apology? Peace out:)

I have a relative high LDL (around 185), already my whole live. HDL is around 60 and Trig is around 50. My whole live I have been very lean and metabolic healthy, Primarily eating whole foods and train 5 times a week in the gym (heavy weight training as well as endurance (Zone 2) and HIIT) . However my Cac-score is 150, not good! I noticed when I am eating low carb high fat (LCHF), my LDL is further increasing. For me it is obvious LCHF diet is not the way to go for me, it increases my LDL and most probably cardiac risk as well!!
