It's all about setting the hook at an upward angle instead of back. When you set the hook in an upward motion you're making the point go up towards the roof of the mouth versus setting the hook back which makes the hook want to pop straight out.

Happened to me while saltwater fishing. Cut bait was picked up, I was sure the circle hook was set and reeled him all the way to the pier and right there he spit the bait and swam off. I was dumbfounded he hung on for so long 😆😆
