Claire - I took your message to my extended family. As we care for aging parents, the team approach is bringing peace of mind for the entire family. Most can't believe that medical error is a common cause of death. Your story illustrates how it is possible, and HOW we can work together to reduce mistakes. Thank YOU!

Thanks Renee for sharing the message of putting your health in your own hands and how a T.E.A.M Approach can help do this! I am glad it resonated with you and hoping it can help others, as you said from younger families to aging parents. We often take our health for granted. It's the one thing we need to try and be proactive about :)

Thanks for watching Marlon. The reality is that health care systems all over the world are complicated and complex and mistakes can happen - we are only human after all. So, we need to find an approach that works for us as patients & care partners to help us put our health in our own hands and partner with our health care teams to help prevent these mistakes. This is where I hope the T.E.A.M. Approach can help out!
