isnt this realy risky if the screw gets pulled out I dont think you can do it again. Extractiong the tooth afterwards will be tricky becaus you cant leverage it anymore due to fractioning. I would try it only if there is no way to leverage it

This method works well if the single rooted tooth is fractured off or decayed at the gumline. Nothing is perfect, but I find the EasyXtract system invaluable in these situations. What is the status of #8?

Not good, long term. That tooth has obviously had endo, a cast post and crown and an apiccoectomy. It is asymptomatic with no fistula and the radiolucency has not gotten any larger radiographically in 5+ years. My feeling is the radiolucency at the apex is an apical scar from the previous apico. You can see how many teeth have been extracted over time and implants placed one or two at a time. The patient began in my practice 30 years ago with less than ideal teeth. Many old fillings, etc., but unable to afford to restore everything in one stage, and many of the teeth were stable and functioning then. He now lives about 3 hours from Waco, but has always been a dedicated patient in my practice. We have worked our way to this point, one or 2 teeth restored at a time as something breaks down. His home care was not the greatest and is not very careful with what he eats, including biting hard foods, sweets, etc. Bottom line, many of my patients are just like those probably in your practice. seems overly complicated. i've used a wards carver to atraumatically extract a root that had a huge post hole. the tooth was super thin.
