Should be none. The reason inside a car is the safest place in a thunderstorm is not because of the rubber tires, it is because the car’s metal exterior reroutes the lightning’s electricity around the outside of the car, which then jumps through the air to the ground from the bottom of the car, leaving the inside of the car completely unharmed. The same would happen in Mario’s metal suit. Electricity doesn’t penetrate the inside of metal, it flows on the outside of it. Even if it’s not a metal suit and it’s somehow converting Mario’s body into metal, electricity still flows only on the outside, leaving Mario’s body unharmed.

It's worth noting at in the ds version with wario (who has the metal cap power) when using the power in lethal lava land, the lava doesn't hurt wario but while active the player slowly sinks in the lava similar to quicksand. Despite that, it's very helpful there!

What if you used damage overflow in LLL, then immediately equipped the metal cap? Would the damage counter reset to zero after putting on the cap, or would it start to take away HP?

Note the *clang* sound when mario falls over while in his death animation as he has the metal cap. This means it was intentional for him to die one way or another.
