If I remember correctly it went something along these lines. Konoha suffers from amnesia and doesn't remember ever being Haruka. Kuroha is the version of Konoha who remembers, I guess since Konoha is a cyber being not a human, kinda like a data file he got corrupted and turned into Kuroha. Oh and EVERYONE in the mekakushi dan have all died once, that's how they get their powers. Go to the wikia and read the translation to the latest disintegration book, it explains how they all died

This scene was before the time the girl lost her body turning into an Artificial Intelligence inside computer. The boy got an illness that killed him with it and does the scientist made a clone to create him tho I think he forgotten all the memories with that girl. The Scientist is their Teacher.

And you dont need someone to die with you and stay in azamis world to get the eye ability,You need to die yourself to get it and be revived again. (Again thats what im thinking)
