There were lots of times that Ichigo had instinctive fear or temporary fear of an opponent thrashing him until he realized he could win. But, in reality, as Suzume175 already stated, Ichigo's biggest fear seemed to be not being able to protect people especially his family or friends. In reality Ichigo's biggest was losing his family/loved ones. This fear stemmed from losing his mother early on. This led to the fear of not being able to protect others because Ichigo felt he had failed to protect his mother and in a way was the cause of her death. It takes him a long time to comes to terms with that (his father as well) and I don't think Ichigo or his family ever 100% get over it even if they try to and do make progress.

Quick question related to Ichigo's new formed blade in TYBW arch will we ever get to see the swords spirit form like how we see Renji's Baboon king Zabimaru, we know it's a newly forged sword and we have 2 blades that can merge as one , since " fake Zangetsu/and his hollow side" were existed as 2 entities is the new one gonna have 2 spirits or will they appear as a merged entity the way Tensa Zangetsu ripped the hollow and showed us a "Vasto lord" form when Ichigo had to learn the final getsuga??
