Yes it does. It gives you the option before you start the game and you can change it at any time in the settings. Just finished the game yesterday on the hard mode. Enemies do double damage and seem to take longer to kill. Plus no hearts ever appear from defeating enemies or breaking items or grass.

My vote goes to Xbox 360. It had an online multiplayer service that in many ways puts its more modern console equivalents to shame. But alongside that, it still very much displayed a full commitment towards local multiplayer functionality. LAN, split screen, couch co-op, etc. Endangered species nowadays.
