Yes, persistent fetal origin of the right PCA, you can also see it on the MR image (but easier on the axial than sliced which I haven’t shown)

very good lecture, thanks a lot! We do routinely T1 black blood after contrast agent., however, I see very frequently vascular wall enhancement (especially aa. vertebralis, sometimes a. basilaris) and have difficulty to interpretate it. How is your experience with this sequence?

completely agree and doing a nuanced presentation on vessel wall imaging was on my to-do-list. The extradural segments of the vertebralis arteries (V4-segments) are allowed to have vessel wall enhancement due to the presence of vaso vasorum. In elderly people, this vessel wall enhancement can extend a bit intracranially. Furthermose, areas of ahterosclerosis can also enhance mildly, all possible causes of confusion (but mainly in older patients).

I have a horrible autoimmune disease, i feel my brain is inflamed, and a beating from the heart, artery, and a kind of movement. And like warm/hot inside, heaviness , pressure. The pain is horrible. I wonder if it could be vasculitis? It makes ne feel really sad because it doesn't feel like a normal headache nor migraine type. I pray for finding a great neuro-radiologist, doctors that can catch what I have. 🙏🏼
