Incident Investigation: Forklift Crushes Worker | WorkSafeBC

This video explains the events that led to a forklift operator being crushed and killed when a forklift moved while unattended. This incident investigation slide show uses animation and photos taken from the incident site. No graphic imagery is included.
It describes the dangers of exiting a cab when a forklift’s load is raised and the hazards of overloading the rated capacity of the forklift. It details the risks forklift operators should be aware of when operating mobile machinery as well as the importance of lowering the forks of a forklift before exiting and keeping out of the crush zone at all times.
This educational video is a useful tool in health and safety training and toolbox talks for forklift operators in British Columbia.

I don't think i have ever worked at a place where at some point they didn't want the employees to work beyond the safety limits of the forklifts. Luckily for me, the first was a manufacturing company that worked with steel products. So I got to see real quick how very heavy some seemingly very small loads could be. A lack of maintenance to equipment can kill you just a fast as ignoring safety. Love these videos. These should be part of safety training courses

Got in trouble for for, first, shouting at a supervisor who walked underneath the load on a manned and moving forklift. I was fresh off 4 years on an aircraft carrier so it was instinct. Second, shouting at the same supervisor not to spray water at an energized electrical fire. They fired me for that one. Thank God it was before that idiot killed me. There was a lot more going on there so I tipped off the fire marshal and OSHA. Not sure which one is responsible but the place got shut down for at least a couple of days.

I hate to state the obvious here but anyone working between a forklift and stacked goods with a load raised is paying the ultimate price for foolish thinking. Never exit a forklift with a raised load. Hand brakes fail like relationships.

A kid at a local construction company died this past winter when his forklift slid off an embankment with him in it. It was also grossly overloaded.

I worked with a guy that died in a garbage truck wreck because the air gauges didn't work and the brakes locked up while he was turning. Wasn't wearing a seat belt and fell out the window and the truck rolled over him. I work for the government. These are the type of trucks they let drive on the roads next to you. On a related note, the parking brake on our forklift at the shop doesn't work at all.

Staying out of the crush zone is super important, anything mechanical can fail. When it does, you don't want to be in a stop that will get your injured or killed.

The root of the problem was "converting" a front end loader into a jury rigged "forklift", a front end loader is ot a forklife because some fool modified it

Besides the diver vids I have done most of these jobs. Construction, trees, equipment operator, tire changer, ect. Try and have some respect for equipment and older workers with experience. I have been nearly killed a few times. It's by luck that I am here. Be safe people!

Long time ago a local well known haulier employed a new driver who used the diesel forklift to load something on his trailer. The forklift was an automatic with forward neutral and reverse on one stick and he had a habit of leaving the handbrake off on level ground with the forklift in neutral. He was lined up to make a lift when something needed adjusting on the load on the truck directly in front of the forklift. He put the truck in neutral and climbed down and walked in front of the forklift while it was idling in neutral. Unfortunately the gear linkage was worn and the vibration of the engine must've caused the lever to go into forward and in idle mode the forklift moved forward on its own. They found the guy dead crushed by the forklift against the side of his truck. The boss decided to close the business down soon after as the shock upset him so much.

The problem with adding a governor for excessive weight is that when the lift drives over bumpy ground or if the brake is applied, it will also shut off because the lift will momentarily experience an excessive weight when it experiences a bumpy ride as each descent from a bump will cause the lift to "fall" down the slope of the bump a couple of inches.

my friends femur was crushed by a small forklift. the brakes were terrible and it had an electrical problem that caused it to jolt forward when you finagle 2 levers at the same time. the forklift wasn't fixed until a year later. the boss who was operating it was told about the issue but forgotten for a moment. even if you tell someone to get equipment repaired like their handbook says too doesn't mean they will when its a high priority machine that's used all day.

That is unfortunate and I feel the sadness of ones who die by something that can just happen at the end of a long day, when you want to get home and are a little tired from hard work to make our nations civilization a better tommorrow. This guy was killed by the negative energy that knew it was going to get at least an injury, for they can taste it and see the outcomes of many disasters thwarted by the good sense of mindfullness that is alerted to see dangers both hidden and sudden and also to stay exact to the safety rules and procedures, and especially to help a brother. The dependable worker was thinking, close to going home and felt like he was not doing anything over the life or death line of you crossed it and must die, and went forward to secure the dunnage and set the load and square yourself in the mirror and head back to the home to wife and young kids... didn't even see it, just off the balanced square of soldity (tires off the ground). And so when he stepped forward the very silent overloaded machines ebbed forward and then slipped and slid to pinch the guy up crushed and rolled forward to unexpectedly kill him.

@jnewbon00 The brakes were designed properly to function when there is weight on all 4 wheels. I have spent quite a bit of time in articulated forklifts such as this one, and I have spent more than my share of time with the rear wheels off the ground! That is why there is a design load limit that matches the amount of counter-weight that is mounted on the rear of the machine. If you exceed the rating, you are asking for trouble. Operator Error can't be justified by claiming faulty design.

@ateras360 It's safety managers that don't understand what they are managing that often cause trouble! :) If you don't have your operator trained properly, you can have all the automatic safety features in the world, and your operator will still find a way to do something stupid. With a forklift, how do you measure the weight? How much pressure in the lift cylinder? Oh, but what if the load is on the tip of the forks, giving it 4 feet more leverage? And then you raise it and tilt it forward...

*Most workplace accidents happen because workers are incentivized to work quickly* . The pay structure for forklift operators is something like this: $15/hr base $16/hr if you're 10% faster at completing your task $17/hr if you're 20% faster at completing your task $18/hr if you're 30% faster ...... etc. $19/hr ...... 40% faster..... etc. Guess where most workers want to be? Everyone wants to make max money, so they rush. They operate at unsafe speeds and the inevitable happens.
