Another experience I want to share. While doing post graduation. We were instructed to prescribe some specific brand by our teachers. These medicines are branded but still price effective, but somewhat expensive than generic. In medical college we see patient from all classes, some are very poor and couldn't afford these drugs. So we prescribe them with generic name. Patient goes to pharmacy and given some very expensive choice by that pharmacists, which is no better than our regular brand in quality and affordability. Poor patient suffers. Doctor connects to the patient, they understand their need and give MOST APPROPRIATE treatment. This relationship is not established in case of a pharmacy shop owner.

See I used to see whether the women is wearing thread or gold chain mangal suthra to judge their affordability.... plus smart phone or not...... generic MRP tags are same or costlier than branded drugs.... But the profit margin is huge..... It is upto the pharmacist if he passes on the benefit to patients or sells at mrp... packing wise cannot differentiate between the two... I being dispensing doctor have both costly and cheap medicines.... both work similarly.. plus subsidizing by giving samples

the way forward would be to improve the 1) quality control tests on Generic Drugs 2) Improve the stock pile of Medical stores 3)improve the doctors salary as these big brands provide huge commission to doctors 4) Enhanced Audit and Assurance reporting about the Drugs 5) Ensure that the doctors do follow the Regulations and the Ethics of providing Cheap but Good Quality Drugs rather than selling Over priced Drugs. if I was the CEO/CFO for these institutions this would be my answer.

Firstly I am a doctor. While studying, living in hostel everyone wants to save some money. I did same. I bought cheap generic montelukast for my asthma. Took it for about a month. Meanwhile I started developing itching all over body. It gradually got worsen day by day, but I wasn't able to find the cause. One day after excercise I developed skin rashes all over body, needed to take emergency injectables. Cause was identified and stopped taking generic. In few day skin itching was gone. Montelukast is supposed to treat allergies and it was creating one. Quality of generic is questionable.

I used to write generic but the experiences were not great, 1) I wrote moxifloxacin eye drop, you can get it branded for around 140 rupees, that's what i told the patient but the pharmacy guy sold him some drops from some unknown company for 210 rupees. 2) Gave one patient betahistine but the shopkeeper gave him betadine. 3) If i write atorvastatin 20mg the shopkeeper can give you a 25 rupee medicine or a 194 rupee medicine, you can guess which one you will be getting. My personal experience, they don't stock the 25 rupee one. So if you think this move will protect your pocket, it won't. The comission money that was previously going to some doctors will now go to someone else.

Majority of doctors (individual) do not get commission Some small PD companies do give commission as some doctors stock themselves... These are not freely available and usually are stocked by dispensing doctors. Generic can only be successful only if standardized and Government will have to ensure their quality.. All branded things are costly.. Be it a soap, shampoo, garment or a technology item... Even with same specification.. There is DPCO list.. A list of medicine specified by government with maximum price restrictions.. Pharma companies arm twist the formulation even slightly to stay away from the list... Say a vitamin of certain composition to cost below 2.00 rs.. A company adds say zinc.. In a very small sub pharmacological dose.. Price is rs 16.00..escape the rules... This product will be focused Asking for generic is like ordering say chicken biryani... Best selling ones around 350.00 per plate... Not what is available in 5 star hotel You'll get some at Rs 90.00 or lower may be per plate perhaps.. If someone ensures both are same quality then generic makes sense Another thing to note is Original research product does not have generic in the initial exclusive marketing period around 6-8 years for tablet, and 12-15 years for injection.. Generic are available after patent expiry Major pricing anomaly is in injection segment.. Sometimes abnormally landing price of a injection and mrp may be 150-1200 4-12 times.. Used in nursing home hospitals.. Doctors do not have a say unless doctors are owners.. Very few

As a doctor who works in Govt setup who prescribes govt supply generic medicines to pts, i can confidently say that this is a stupid decision. Before making it compulsory to prescribe generic medicines , govt should work on improving the quality of generic drugs in the market. I have seen patients who come to me asking for generic substitute for branded medicines, once we change them over to generics, their diabetes worsens at the same dosage. If they were taking 2 tablets of branded drug to control their sugars, now they need 3 tablets of generic drugs to control their sugars.

I think quality of drugs should not be doctors concern, our regulatory bodies should be concerned about it, this rule empowers the patients to choose what they want, if they can afford they will go with branded if they cant they go with generics, I worked in manufacturing unit i know how the quality is increased & how much money is spent for what , majority of amount is spent for marketing ,Indian Pharma companies export FDA standard generic medicines to the world. But same companies cannot maintain the quality in domestic market. this issue should be addressed. without any base telling this decision is stupid, is not wise for educated like you, i also worked in govt setup & aware of services provided there.

The quality of excipients (non active ingredients used for "bulking" the drug should be good as well. In principle, it is fine to ask generic drugs to be prescribed. Who will do the quality control? I live in the USA for decades. Our insurance covers only generics when generics are available. Many of the generics come from India. One can still ask for the manufacurer name for the generics. e.g I go to Walmart or Walgreens, each of them have generics for the same brand name drug. I can still choose Walmart or Walgreen ( and perhaps both may or may not be made by the same manufacturer). I do not remember using any brand name drug for decades. So only brand names are effective may be a myth that cannot be scientifically proven in a double blind study. There may be some exceptions, but exceptions should not drive the rule, like tail wagging the dog. Even Costco carries generics from India. Generic does not mean poor quality. What we need is transparency with respect to pricing, manufacturer and quality. Anecdotal evidence is just that - just an opinion but not scientific. AFAIK all medical insurance in USA , especially Medicare Advantage and HMO type insist on generics whenever available.

Different branded drugs of the same medicine have different bases or combinations which help is various ways like controlled absorption , increasing Bioavailability, reducing side effects or reducing dosage of the drug given . I've experienced myself that The generic medicines like paracetamol 1000 given in govt outlets work like paracetamol 500 dose .There r many other drugs which have the same problem. So these are also important factors which a doctor looks into while prescribing medications . More important than checking and ordering doctors not to prescribe branded medicines is to tell pharma companies not to make anything branded and keep their prices within a certain limit and also to improve the quality of generic drugs . Doctors have studied for years to reach the position where they can decide correctly which drugs can be given in which situation. Non doctors shouldn't impose unrealistic rules in a field which is not their area of expertise

In your example when a doctor writes Paracetamol and fever does not subside then make sure the doctor is neither roughed up nor taken to task for no relief 2) It has become a norm now to pin the blame for each facet of Healthcare on doctors. High time everyone has a defined job and is accountable for their part. How can a doctor be blamed for non functioning DG set or a short circuit in the government hospital. 3) Stop this one way traffic And if all people feel doctors in private practice are crooks then make them stop their small nursing homes and hospitals.....let everyone go to govt or corporate hospitals....1 week is sufficient to know that the Healthcare of the entire country depends on these small nursing homes ans specialty hospitals. Pls don't forget the Rajasthan strike

A few questions arise -If every allopathy doctor prescribes compulsorily generic medicine, who will prescribe the bonded medicine? -Why are direct drug companies not banned from making branded drugs? - Who will take the responsibility of the quality of the medicine if the drug shop owner will give the medicine of his own company on the prescription of the generic medicine of the doctor? - Will the drug companies making millions and crores stop making branded drugs? -Many Ayurvedic, Homeopathic doctors, people without degree prescribe allopathic medicines, what to do about it? Who will take responsibility for the quality of generic drugs? -What to do if many educated doctors ask to prescribe medicine of a bonded multi-national pharmaceutical company? - Can you implement these rules in big corporate hospitals?? Due to new rules being implemented day by day..the tussle between doctor and patient is increasing. It is not right to suspect every doctor because of the nexus of a few doctors with drug companies. Day by day the medical packages are getting tougher and the new rules are becoming very confusing. Rather than this fix the price of each drug.. Compare the price of the same drug of any company.. there will be no problem. With this rule it is not possible to take action on every doctor but it will be easy to take action against the disliked and selected doctors.

The solution is called “regulating the price”. We can most certainly know the baseline of what it takes to develop a medicine. Now brand’s might get fancier with taste or processing but there should still exist an acceptable baseline minimum cost of production of a pill that meets quality control standards. Giving leverage of about 5%-10% of the cost price from the baseline price will work fine for everyone - patient, doctor and the business. But the pharma industry is a sophisticated colony in itself; runs by rules of maximizing extraction not by regulating profit margins/prices. Whosever suggested trying to improve generic drugs while leaving “branded ones” as is in their profit model is BSing.

This is a wonderful initiative by the Govt. Not only are branded medicines expensive, but frequently simple generics like paracetamol or ibuprofen are mixed with vitamins and marketed with higher prices. This racket needs to stop. What the medical council can suggest to these doctors opposing is to write your prescription for generic and let those who can afford buy the higher priced brand because they "trust" it more. For majority of poor people in India medical costs are bankrupting them. They should also look into runaway charges for MRI, CAT scans and other services that are frequently unnecessary. India needs protocols and standards like in the US that all hospitals and doctors must follow and there should be a consequence for breaking them. Just don't use insurance and legal recourse like the US

I also found that generic drugs are not working like branded ones. My pharma friend said that generic drugs may have same medicine but while making them the catalysts used in them may not work as intended, the impurities are not cleaned properly, active and inactive ingredients may not be in proper ratio. All of these shortcomings will make the generic drug dangerous rather than working for the disease. Testing labs not available and even they are available , large scale testing may increase the drug price again and so generic drug makers won't go through such rigorous testing. Government should come in to start testing labs in that case. Simply forcing doctors to suggest generic drugs will be a stupid thing to do.

In case of epilepsy, even if the patient tries to use a different brand medication from the one that he is using before can prove fatal without the doctor’s intervention. A lot of times, due to lack of availability of a drug , the patient chooses to use another brand which could lead to death and guess who is held responsible? The doctor! Not everything is about profit and money.

Bro my father has bipolar disorder and the doctor was sending it's own local manufactured medicine and was very expensive more than the branded in the market .so I decided not to go with doctors medicine . I purchased the same branded medicine of same composition from market and my father is doing well. Now I have moved from branded to generic medicine few years back and it's quite effective not having harsh result . So , every affordable things are not bad and harshfull but can be good also just like tata Nana which was backthrusted by people earlier so called cheap and people are buying the same things at a triple price (mg cars ) so we have to appreciate the efforts of our government because there are various committees who have worked upon this final commitment to sell generic drugs and they are more certified and heighly educated than us . Son we should not deliver false claims regarding generic medicine as it can work as a boon for middle class family

And the chemist is free to choose the brands?? How does this work as long as pharmaceutical companies make branded drugs?

Generic drugs prescription is done in govt setups coz tey cnt afford good brands. However thse drugs hav such poor efficiency double d dose r given often. In pvt places if docs dnt prescribe brands thn medical reps wud instead go to pharmacists and giv tem money to sell their brands. Nd how is tat a better option? Coz tey hav no idea abt thngs + in india pharmacists r not running pharmacies tey r jst business ppl wid no degree therefore its evn more dangerous to patients.

Good decision by the Govt. Companies also must be mandated not to put any brand name for their drugs.

Even if today any doctor prescribe a generic medicine you won't get it easily...in my case the nearest generic medicine store is at a distance of 25km ...

It is unfortunate that in India people are ok with poor quality just beacuse it is cheap. A large part of it is due to freebies distributed by various parties which have changed the habbit of people and made them so dependent. Ultimately its the yax payers money getting affected

Tax is meant for freebies only. In west tax is The reason education is free in some countries and affordable healthcare. You should reconsider what you describe as freebies.
