On The Matter of Infectious Disease Localized to the Female Reproductive Tract, A Gynecology Matter, with Discharge (Various Morphology), and Differential Diagnosis Herein (By MD Paul W. Bolin, Creator/Architect of CRASH! Medical Review Series): 1) Communicative Infection (Usually Sexually Transmitted) within GU Tract are herein Due to 1) Protozoan Trichomonas vaginalis or 2) Candida albicans (Yeast-Fungus of Microscopic Dimensions); whereas 3) Bacterial Vaginosis (Gardnerella vaginalis or Lactobacilli SPP Aetiology) is overgrowth of a Previous Normal Homeostatic Microbiota within these Organs and generally not considered a Communicable Disease or Sexonotic (Arising from Context of Coitus, Cunnilingus And/or Sodomy). Antibiotics Hygiene Theory or Immunosuppression (Inherent or Environmental Causes) has a Probable Contribution. SSx of all have Discharge as a Common Factor but Differ in Morphology therein: ( 1)Trichomoniasis is Yellow-Green and Malodorous; 2) Candidiasis has a Thick and Cottage-Cheese Texture; 3) while Gardnerella vaginalis Infection is Characteristically Gray-White and Fishy Smelling) but the Laboratory Investigation(s) and Presentation/Manifestation Therein is Diagnostic and Differing 2) Therefore Dx for Trichomoniasis has Erythema and Irritation (Mucosal Injection); Wet Mount Microscopy has Motile, Flagellated Protozoa Visible and an Elevated (pH) Acidic Environment (> Than 4.5); Tx is via Metronidazole (Same for Partners) because Complications Possible are Related to Pregnancy (Preterm Birth, Low Birth Weight [LBW], Premature Rupture of Membranes [PROM]); 2) Candidiasis Localized to the Vagina (Candida albicans Vaginitis) has the Erythema/Irritation on Px Similarity, KOH Preparation Yields a Diagnositic Morphology of Pseudohyphae and Spores of Candida albicans, Normal pH (4.0-4.5), Tx Topical Antifungals (Azoles Drug Class) or PO (Pregnancy Topical Only); 3) Bacterial Vaginosis (BV) is Distinguished by Wet Mount Where Clue Cells (Clustered Bacteria to Individual Epithelial Cells) and a Characteristic Potassium Hydroxide (KOH) Preparation yielding a distinctive Odor upon KOH Application (So-Called Whiff Test) and Elevated pH Laboratory Test (pH > 4.5); Tx is via Metronidazole for both Gestating and Simple Female Condition; Cx of Gardnerella vaginalis can yield Prematurity, Low Development Weight at Birth (LBW) and Premature Rupture of Membranes (PROM).
