Dear Dr. Bolin, I find your USMLE videos extremely helpful. A brief note on this one: in your presentation of inflammatory bowel disease, the pseudopolyps in colonoscopy photographs actually represent regenerating mucosal islands between ulcerated foci. This has been demonstrated by histopathological analysis. The areas in-between are the ulcerated zones.

The two most common symptoms of IBD (which almost no review videos ever point out and which cause the most deterioration in quality of life) are the increased frequency of bowel movements and the increased urgency to have a bowel movement. These are almost always present with IBD and should be the first thing you look for in my opinion. Also, though he keeps emphasizing that there is never disease in the small intestine with UC, that is actually not true. If one has pancolitis, then backwash ileitis is fairly common, and we will see inflammation in the terminal ileum.

He mentioned both diarrhea and Tenesmus (urgency to have a bowel movement despite an empty colon) as symptoms in the first third of the video :) I looked up "backwash ileitis" and you look to be right to some extent, but it is a rarity. Occurs in 10-20% of pancolitis, which is only seen in extensive U. Colitis. However the ileum is involved in as much 80% of Crohns cases.
