Short handed job. You should have weld-lock that insert on the backside, where you inserted the 3 bolt pins, then clean it up. Also the insert was not even tightened in place, that's why you used the 3 pins, therefore you should have welded the insert. EXTREMELY POOR JOB, THAT PRESSURE PLATE WILL FAILE AFTER ANSTALLMENT
