Is Having a High Arch Worse than a Flat Foot? - Lewis Nurney, Singapore Podiatrist
Flat Feet (Pes Planus)
Flat foot is a very common condition in Singapore. This is due to the genetic demographics of the region where this type of foot may be hereditary, acquired or due to ligamentous laxity. The artificial surfaces we walk upon every day expose our feet to greater wear and tear or degeneration.
There are 2 kinds of flat feet:
1. Congenital flat foot is hereditary and present in varying degrees of severity in children as young as 2 years old. In most paediatric flat feet cases, the child may appear to have arches while they are seated or standing on tiptoes. The medial arch only collapses when the child stands up. Flat-footed individuals may also be diagnosed with hypermobility, a condition related to the laxity of ligaments. This laxity can also cause problems elsewhere in the lower limbs, such as the knees and hips.
2. Adult-acquired flat foot can be caused by conditions such as posterior tibial tendon dysfunction (PTTD), arthritis, injury or diabetes. If untreated, the affected foot may deform and collapse more and more over time. This can result in loss of mobility, postural changes, and pain all the way up to the spine.
Foot Orthotics
Customising insoles for flat feet is a common treatment to address the issue. These customised foot orthoses are prescription-only medical devices tailored for individual patient needs to address specific foot pathology. The podiatrist will decide on the materials, densities and designs of the orthotics depending on the medical requirements and lifestyle choices of the patient. Due to advancements in technology and manufacturing, specific custom foot orthoses can also be fitted into pumps, court shoes, and even specialised footwear like soccer boots or hockey shoes.
Painful High Arches (Cavus Foot)
The foot is the foundation of the whole body. Fallen arches put the feet out of alignment. Consequently, greater stress is placed on other joints in the body to help compensate. This is often the cause of soft tissue pathology; the two most common ones are Achilles tendonitis (tendon overuse injury) or Plantar fasciitis (inflammation of the fascia).
The foot is generally divided into three main arch types, low, normal and high arch. The high arch foot type is also known as the pes cavus or cavoid foot. In a high arch foot, excessive weight is placed on the ball and heel of the foot when walking or standing, leading to problems such as foot pain, gait instability, metatarsalgia or plantar fasciitis. Problems associated with a high arch can develop at any age in either one foot or both feet.
Whilst high arch foot is a structural presentation of the foot, it may also be suggestive of other medical conditions, especially those associated with neurological (nerves) conditions, to clinicians. Some of these conditions include cerebral palsy, polio, muscular dystrophy and/or stroke. In some cases, a high arch is inherited through either parent. If you or your child has been identified as having a high arch, you should seek out a health professional such as a podiatrist to ensure that there are no underlying medical issues that are detrimental to your foot health.

Thank you for the information , it is helpful . I have a high arch and I underpronate . 15 days back I sprained my ankle and in the X-ray it should no signs crack but it did show a heel spur . Now the ankle is not yet healed , pain is there around the whole circumference of ankle and pain comes from heel to knees . What treatment would you suggest ?
@EastCoastPodiatry
@EastCoastPodiatry Жыл бұрын
Hello Monal, thanks for your comment. Heel spurs are a very common finding and even though they may look scary on the x-rays are quite rarely symptomatic. What we can recommend would be to allow for the ankle to heal after the sprain before intervention with customized insoles that would correct your gait so that it doesn’t cause you any pain. In the meantime, you could try some gentle calf stretches and consider Focused Shockwave Therapy for better healing.

Hi Kevin, based on your description, it would seem likely that you have a high arched foot which is causing increased stress on your plantar fascia. For more information on high arched feet, check out our video at kzbin.info/www/bejne/nWfRnpKuip2Cjqc. Exercises that utilize the toes such as towel toe curls or using your toes to pick up golf balls or marbles can also be useful. If your arch is hurting, you can try rolling a frozen bottle of water under the arch to help soothe the discomfort. Make sure to wrap it with a towel to avoid causing excessive discomfort from the cold.

I have a high arch, and I always hurt my Achilles’ tendon. Simply pushing off my toes to step up something or do a short run while playing with kids or dogs will have me waking up the next morning wondering what the hell happened as I am in so much pain I can barely walk. This has progressed now from when I was around 25, to now 29. I thought it was due to periods of inactivity and then suddenly being active again. But I now know even when I am in great shape I am still susceptible to an Achilles’ tendon injury.

I wore regular shoes my whole life and developed over time plantar faciitis, a dorsal bunion, weak abductor hallux muscles, and flat feet. I have worn Vibram five finger shoes with no arch support, no stiff soles, in fact, I am wearing full-time Vibram EL-X shoes the most minimal shoes you can buy with toe pockets. I have no longer any foot problems and I am so glad I never went to a podiatrist. I am fully cured by going back to a barefoot life.

Hi Aurpi Chowdhury, apologies for the late reply. Yes, it is possible to suffer from fractures caused by a high arch foot as this type of foot type can lead to a greater risk of tripping or falling or other conditions and injuries. However, flat foot can also have a number of similar risks so it's always best to have your feet regularly checked by a podiatrist. Even if your friend has recovered from her fractured foot, do encourage her to have her feet checked by a podiatrist as well.

I’m 24, I’ve had problems with my feet since before I could walk. I like to be active and is stopping me from doing my activities. @EastCoastPodiatry
@EastCoastPodiatry 4 жыл бұрын
Hi Katie, there's a bit to unpack so bear with us here. First, overpronation is one of the symptoms of flat foot. Those with flat feet tend to have low arches, and as a result, their weight is more heavily distributed over the inside of their foot as that is lower than it should be. When you overpronate, this means your ankles are more likely to roll inwards, causing you to trip, stumble or fall easily, thus increasing the incidence of ankle sprains. When your ankle has been sprained multiple times, which sounds like the case for you, the ligaments in the ankle can become permanently weakened, making them more susceptible to future injuries. It is a vicious cycle. Insoles are meant to help by propping the arch up, reducing pronation and incidence of your ankles rolling inward. If you are overweight, walking, rather than running, is often a good way to start, however, with your risk of injury, you may be better off doing lower impact exercises such as rowing, swimming, aquaerobics or aqua-jogging. We understand that the current circumstances may make such exercises difficult, but it would probably be best to lose more weight before resuming long walks as this will reduce the stress on your feet and lower the likelihood of injury.

Hey Mousa, it's less common for someone to have a flat foot in one foot and high arch in the other. I would recommend that you consult a podiatrist as your different arches will require different types of support. It's likely that your right foot is experiencing pain as the high arch is putting a lot of stress on the midpoint of the arch, causing you to feel pain in that area. Please look for a good podiatrist in your area and book a consultation as soon as you can.
@EastCoastPodiatry
@EastCoastPodiatry 3 жыл бұрын
@Angel Reyes Hey Angel Reyes, usually we recommend insoles for those who are very active as the insoles help support the plantar fascia and cushions it. However, do take note that we do not recommend insoles bought from stores. Insoles are medical devices that should be customised and prescribed by a podiatrist. Since you are experiencing discomfort, you should look for a good podiatrist in your area and schedule a consultation before your foot condition worsens.

@EastCoastPodiatry

Hi Chak Lam, based on your description, it does sound like you have a severe case of high-arched foot that has affected your well-being. Due to the inherent instability, your body may adjust unconsciously to give you the best support and most stable gait it is able to derive naturally. However, this gait may also put increased stress on other parts of your body, including your knees, which can adjust to accommodate. However, we cannot be certain without assessing you in person. Conservative treatment would typically focus on pain relief first, followed by a set of orthotics to help you stabilise and correct your gait over time.

do some exercises for the feet + wear the orthotics maybe your foot could return to normal after years if not only surgery can correct the deformities of the foot but it must be the last chance and only if you have pain. I have a level 3 high arch. It depends on the level to which we are because level one of the flat feet and high arch is not a problem or at least just for sports. Level 3 can hurt in everyday life. the high arch does not absorb shocks and it causes cartilage problems. in my case I must always wear the orthoses because if not the pain will return in my knee.
@EastCoastPodiatry
@EastCoastPodiatry 4 жыл бұрын
Hi Bashkim, you have brought up some points that we think are very important. If a person has been prescribed orthotics, they should definitely wear them as much as possible as their clinician has decided that there is a medical need for them to use the orthotics. And as you mentioned, surgery is an option for those with high-arched or low-arched feet, however, it should be the last resort and not something we recommend lightly as this requires a long recovery period.

Hi Michelle, plantar fasciitis is very common amongst those with flat feet, but those with high arches are also prone to plantar fasciitis. Custom insoles can be extremely effective at managing pain and improving the underlying conditions responsible for the pain but they need frequent reviews to ensure that they are having the desired therapeutic effects on patients. At East Coast Podiatry, we ensure this by arranging for our patients to come in reviews over a 12-to-18-month period to allow for periodic adjustments of our insoles, so please make sure your boyfriend attends any reviews he has with his podiatrist.

Seems like nobody have the problem I have. When I stand for say 15 20 mins, my heels start having discomfort, not sharp pain like p.fasciitis though. I have to continuously keep hopping on each of my foot. For years, I have to look for soft cushion, and soft sole sneakers. if I am wearing black leather formal hard sole shoes for parties or wedding, then I start having that discomfort very quickly, and I could hardly stand for 10 mins without discomfort. I never have that heel discomfort while walking with soft sneakers. But when I am barefoot standing in the kitchen cooking on tiles or wooden floor, I cannot stand more than 10-15 mins before having serious urge to sit down because of the pressure on the heels. It feels like my heels does not have enough padding, is it possible? I went to the orthopedist a couple of times, but they don't see anything structurally wrong with it, so I gave up and bearing this disability for at least 20 years. And, that may be the reason that my both knees started giving me issues for few years now. Back home, my brother-in law was orthopedic surgeon, and years back he used to order custom made (for me) leather cushion arc insoles to put in my shoes which helped somewhat. I have high arches and my ankles come in a little when I walk. I’ve ran since I was in 6th grade making it to states every year up until freshman year. This is because me knees have been hurting very bad when I run and I don’t know if it was because of my high arches or if it was something else. I’ve seen doctors and they say that it was because of weak hips or overuse but my hips have turned out or be strong and I haven’t ran for over a year now. Please help me out.

Hi FastFish, high arches can lead to knee pain, so it is possible that they are responsible for your knee pain. Since you haven't been running for over a year, recent overuse is unlikely to be responsible for your pain. However, as a frequent runner in the past, it is possible that you have developed a chronic injury during that time that is causing your knee pain today. We would suggest consulting a podiatrist who can assess your gait as this may give you a better idea of what could be causing your pain.

@EastCoastPodiatry
@EastCoastPodiatry 4 жыл бұрын
Hi Mark, flat foot doesn't always cause pain, and pain in the knee can be caused by other areas of the body. The knee pain in your leg could even be due in part to the flat foot in the opposite foot. Such situations occur when the body tries to adjust movement and gait to accommodate issues and the adjusted gait winds up putting more stress on the other joints, including the knee in the other leg. You may also be suffering from a leg length difference that is causing you pain in the knee. You should see a podiatrist as soon as you can to see if that's the case.

Hi Ruchi, we understand that flat foot can be extremely uncomfortable. We would recommend that you seek treatment immediately. If that is unavailable, there are some options you can try at home such as soaking your foot in hot or cold water, freezing a plastic bottle of water and rolling it under your arch can help to relieve some pain. Others have also used a tennis ball to suitable effect.

Hi Pascal, having a flat foot could be responsible for these issues particularly if your foot arch has completely collapsed. As your body adapts to the collapsed arch, it may cause you to walk in a manner that is unnatural to your usual gait, causing stress at certain joints and pain in certain parts of your body. Without a proper assessment by a podiatrist, it is impossible to say, but it is certainly possible.

I have a weird thing where I have high arches, but I balance my weight on the inner sides of my feet like with flat feet? I think it was from years of ballet and turning out my feet position so more of your balance is on the inner side of the foot and that just kind of subconsciously stuck with me. I have a lot of pressure on my inner tendon in my arch and it feels like it's pulling so hard I'm going to injure it just from walking.
@EastCoastPodiatry

Hello Nathan, our foot structure develops intensely before our teen years. If you have started ballet at an early age it could be that repetitive movement has altered the shape of your feet and your balance. It could also be a problem coming from your lower leg such as knee position. If it is causing you much trouble a proper musculoskeletal assessment should be performed by your local podiatrist who then could discuss a treatment option best for you.

Hi Sae, I'm afraid we are not familiar with "treadlabs" but if you are experiencing a lot of pain, we would strongly advise against off-the-shelf insoles. Instead, please consult a podiatrist who will be able to prescribe and customise a pair of foot orthotics that will help address the root cause of your pain. While over-the-counter products may seem to address mild cases, those who suffer from severe pain should not rely on them as they are of limited utility. Prescription orthotics take into account each feature of your feet in order to do what's best for you.

Hi, I was wondering which is better when you are walking: putting pressure on the front, middle or back of the foot? Should you land with the middle part first on the ground and then roll your foot while you take a step? I had flat feet for years, started getting a bunion and pain associated with plantar fasciitis, but now it's much better because I walk barefoot more and I put pressure more on the front of the foot, while trying to maintain an arch. My feet have gained an arch but still a bit painful, perhaps I need to stretch more with the strech ball to get rid of the bunion - or stop wearing climbing shoes 😅 - but in my case the problem seems to get lifted once I stop wearing shoes and walk with weight distributed on the front toes. Any thoughts on how to walk properly while wearing shoes?

Hi Sharon, sorry for the delay in replying. Based on your description, when a bunion has gotten to the point where it under- or over-laps a toe, the only option available is likely to be surgery. Bunions do get worse over time, that's why it's important to seek treatment early. If done sufficiently, you can halt or possibly even reverse the development of the bunion. However, in your case, it is likely to continue to worsen if the underlying issues are not resolved. These reasons could be hereditary in nature or the type of shoes that you are wearing.

Hi Larry Pavone, a clinician will typically tell you to not wait at all and seek professional help as soon as possible and we would give you the exact same advice. The demands at work definitely contribute to your injuries and it seems likely that this has been caused by the higher arch in your left foot. In your case, you have tried some off-the-shelf insoles and they are not improving the situation. Shoe insoles for high-arch foot conditions are prescription-only medical devices and you should not be buying them over the counter. They should be prescribed by Podiatrists only after they have obtained a clinical diagnosis through a comprehensive assessment. I was a runner for quite a number of years, but went to university, stopped running and gained a good bit of weight. I have a rather pronounced arch, but what I find interesting is because my weight distribution while running is concentrated on my heel and first metatarsal I tend to over pronate when I impact the surface I'm running on (this used to not be the case, but rather has come up since returning to running). Unfortunately, as a result of over pronation I have developed some rather irritable medial shin splints. I seem to have some relief from inserting an arch support into my shoe to mitigate this over pronation, but as I compress the support throughout my runs the benefits of this support subside. My question is, should I consider increasing my arch support (such as through a custom cut compression foam on top of the already inserted arch support)? Many thanks in advance.

I have this condition that I can't explain, tried googling so many times but can't get anything related. So my feet can't stretch to all they way, they are not flat feet, it makes it hard for me to walk on high heel and even feel like my feet are sexy. Normal people do stretch their feet up to like 180° or more, I can't get anything past 140° approximately. What could be the problem and what's the remedy. I'd really appreciate if you get back, a girl needs help.

Hi Fade Store, your ankle restriction could be due to a soft tissue tightness or a cortical impingement. However, unless we are able to physically examine you, we will not be able to come up with an accurate diagnosis. Please look for a podiatrist in your area for help. Take care!

@EastCoastPodiatry

Hi Lamb Sauce, Does that mean you're mint since that always goes with lamb? Jokes aside, having a high arch isn't necessarily bad or dangerous. It does mean you are at greater risk of developing certain injuries and conditions, but it will not necessarily hurt. We would recommend you choose shoes that are suitable for your foot type as well as be more aware of any discomfort as that could be early warnings from your body.

Patients come to us with issues caused by flat feet on a daily basis. This is a very common problem here in Singapore. A podiatrist would be able to diagnose the underlying cause of your problems and prescribe a treatment plan to get you on the road to recovery. Massaging your foot will not solve your problem. Look for a podiatrist in your area and seek treatment as soon as you can.

@EastCoastPodiatry
@EastCoastPodiatry 3 жыл бұрын
Hi Puneet, ankle joint-related issues can be caused by a high arch, however, without assessing you, there is very little we can tell you. If the pain is on the outer ankle, it could be peroneal tendonitis which can be caused by high arched feet but is often caused by a sudden increase in activity as you have described. Please look for a podiatrist in your area and schedule a consultation as soon as possible.

Does flat feet make feet bigger and wider? And once you develop a normal arch does ur feet get smaller and narrow
@EastCoastPodiatry
@EastCoastPodiatry 4 жыл бұрын
Hey Brittany, you're right about the possibility of the flat foot causing the foot to spread out and becoming wider. However, when the arch is pushed up, it also depends on how the other ligaments in the foot have been previously affected. If these have relaxed greatly, it is possible that the foot is widened permanently.

I have struggled with high arches since I was in 4th grade (now in 7th) and Me playing 2 high running sports (lacrosse and basketball) I find it difficult to keep from injuring my Achilles and frequently strain it, making it hard to run for 4-5 days. Is there anything you can suggest that can help me with this?
@EastCoastPodiatry
@EastCoastPodiatry Жыл бұрын
Hello seesprite, Achilles tendon is a structure very prone to overloading, irritation, and injury. The more trauma the easier it will be to injure it again. The best option for you would be to see a Sports Podiatrist who could design insoles accommodating your foot type. Moreover, you could consider shock wave therapy to improve the quality of your Achilles tendon tissue. Insoles combined with proper stretching of the calf muscles should prevent the injury from recurring whilst shockwave therapy could reverse the damage already done.

Flat foot is best. You are more grounded onto the ground being flat footed. Imagine pushing on a wall with your hands arched vs your hands flat on the wall. Which you think is better for pushing power on a wall if your hand is flat on the wall or arched

@@idekanymore1624 Hi Ray, while there are some exercises that can help to stretch and strengthen the arch, but it would be best for you to consult a podiatrist so that you will receive a proper diagnosis of your flat foot condition. The podiatrist will be able to offer you the latest array of non-surgical solutions to address your foot condition. Surgery should only be the last resort and it would be best for you to seek a second opinion if anyone offers that to you. For our practise, we work on strengthening and healing the injury to the plantar fascia as well as incorporating customised orthotics to provide increased support to the patient's foot to prevent future injury.

Hey Little Ninja-TV, if you notice yourself constantly not walking straight, do speak to your parents about your condition and see if they are able to bring you to see a podiatrist. It is important to get this sorted out before you get older. Take a look at the other videos we have on this KZbin channel and see if there is a video that reminds you of your walking problem. It will be better for you to watch these videos with your parents as well. They need to be better educated about your foot health and bring you to seek appropriate treatment ideally from a podiatrist. Take care!
