I'm excited to sit down with Dave Feldman. We talk about cholesterol, Lean Mass Hyper-responders and much more. Make sure to listen to the full interview to learn more.
Dave Feldman is a senior software engineer and entrepreneur. He began a low-carb, high-fat diet in April 2015 and has since learned everything he could about it. He has given special emphasis on cholesterol given his lipid numbers spiked substantially after adopting this way of eating. As an engineer, he spotted a pattern in the lipid system that’s very similar to distributed objects in networks.
He has since learned quite a bit on the subject both through research and experimentation, which has revealed some interesting data. With this new general theory, he has shifted around his cholesterol substantially without any drugs or special supplements of any kind.

Hurry with the study! Lol My son (12 and type one) is LCHP, HDL/TG/A1c great but LDL super high (>350). Endo prescribed statin and I said no. I SURE hope I'm doing the right thing. 😢😢😢

Why do none of the carnivore talks never mention low HDL? There's a million about high LDL on carnivore but I want to know if having lower than avg HDL is bad for me. I just can not get it to go up and Im nearly 100% meat organs and fat.

HDL should go up on a carnivore diet if you are eating fatty meats and lifting heavy things as Dr. Ken Berry suggests. Mine was on the low end until I started carnivore and then it raised by 20 points.

I have 2 of the three but HDL is below 80. Great information. Thanks for the video. My CAC score was 32. I'm 70 years old. I workout generally 5 days a week for 2 hours a day and have eaten carnivore for over 4 years. My obese physician wanted me to take a statin but I refuse to take one.

I'm 70 years old. Last blood tests found my ldl at 167, hdl 86 and triglycerides at 43. My doctor breathlessly called me first thing after getting the report to urge me to go on a statin. I'm embarrassed to say, I laughed out loud at his apparent horror and told him not to worry and that I didn't need a statin. My BP is chronically below 115/70, I walk 4 miles three times a week, and lift weights 3 days/week. I still bench 275lbs. I also bike and play golf weather permitting.

The doctors call about LDL and never mention how good your HDL and triglycerides are. Standard of Care says they must push the statin or get dinged.

We need to remember that most people come to carnivore damaged and their liver function regarding gluconeogenesis etc is very individual and will change over time with healing and mitochondrial increases.

My AST/ALT figures were sterling after going carnivore. They indicated NAFLD before.

High cholesterol may exist in the presence of arterial disease, but that doesn’t mean causation. Especially since cholesterol has been noted to assist healing of inflammation.

carnivore 2 yrs now. First year had high LDL but 2nd year LDL signficantly dropped. The only difference I had more exercise and ate more seafood. I either maintained or slightly increased intake of saturated fat

I just found out that I am a lean mass hyper-responder, with triglycerides of 55, HDL of 110, and LDL of 277. VLDL of 5, A1C of 5.3, and AST/ALT both 20. My BP was high her office, but at home when relaxed, it 120/80 (and usually lower). I am very lean and fit. My doctor freaked out about the LDL..even in this context, and wants me to eat low-fat/low-cholesterol diet (I currently do high-fat carnivore for psoriasis and arthritis). Really looking forward to the results of Dave’s research!! And so grateful that I could order labs from his testing site so I don’t have to deal with pushback from my doctor! Thank you, Judy, for having Dave on your channel!

Own Your Labs is a great place to get testing done. The mainstream doctors will only tow the line with statins if your LDL is 100 or above. The truly don't care about your HDL, A1C or triglycerides.

I'm carnivora for 2 month to fight inflammation, I have Polymyalgia Rheumatica, very painful autoimmune disorder. You said you do high fat to fight inflammation? Is the 80/20 ideal to fight inflammation? ... I'm new at this and i want to make sure I'm doing this right to get rid of this autoimmune disorder! Thank you!

I am 23 yrs old female with cholesterol total of 451 HDL 98 Trigs 64 LDL 348. I have been on the carnivore diet for 2 months now and I can’t wait until my body starts regulating the process however I feel better and train more than ever!!!❤🎉

I started carnivore in April 2022. Got my blood work done in January 2023. My total cholesterol was 250. My LDL was 170. Push that aside, my HDL is 62.5, my triglycerides are 90, my VLDL is 18. My doctor was not concerned at all about the higher lipids although he does want me to get my 170 down. I exercise every single day and I never eat carbs, so I’m going to get my blood work done in July and see if it’s any better.

You talked about eating sticks of butter like "Bella, the steak and butter gal," she did this for a month I think she said and she did gain weight, but the point of the butter(using Kerry Gold and good butters, making brown butter bites ) Is to help with feeling hunger, most can't tll if they are hungry so when should I eat..Butter helps satisfy the hunger and the urge to binge eat and helps cut down snacking so you can fast longer and since it does not raise the Blood sugar or break the fast it is helpful. I personally use this method to get me through, because when you have been eating every 2-4 hours every day you can't tell if you are hungry or if it is mind hunger. I don't think after awhile you would con't and probaly cut back on the butter consumption which I have. I feel more satisfied and don't need it anymore. I just finished the Dr. Boz 3 day Sardine fast and it was amazing, finally able to get my bllod sugars down in the 100's. very helpful. We have to be reasonable with all things and be aware of blood levels and make adjustments through out our journey. ThankYou, this was very educational.

I am also a hyper responder. My husband pressured me to get a lipid panel done and now he wants me to stop carnivore because of the results. I am bracing for my call with my primary care physician in a couple days. I don't think she will be as anxious as my husband but I am a hyper responder too. I am hanging on to the fact that my triglycerides are super low, I'm thin & muscular, I work out despite still having chronic pain and environmental sensitivities even on carnivore. My digestion is great. I think I will tell her that I need to focus on getting the inflammation down.

Ask your pcp for a high sensitivity crp test. It measures cardiac inflammation and should be below 1. I am on keto carnivore, have high ldl but my other markers are excellent. Have an ldl particle count done as well. There are two patterns, A and B. Pattern A is indicative of a healthful ratio of large to small ldl. And yes, in spite of my being pattern A , crp .86, and other numbers being excellent, triglycerides 44, HDL 63, fasting insulin 2.3, A1c 5.2, etc., he wants me on a statin. I told him no thanks, no way.

I’m also a hyper responder on carnivore. My husband is insisting I get my blood tested every 4 months because he’s concerned. I feel great and my digestive system & my mental clarity have improved massively. So I have no intention of going back to my old healthy diet.

The recent Korean study on almost 13 million people showed increased cholesterol was correlated to a lower all cause mortality and lower cholesterol was correlated to an increase all cause mortality. It basically showed the levels our doctors say is healthy is too low and the level our doctors say is too high is perfect. So I don't understand why this is still an issue!

I heard on another yt channel that it is because with certain disease (like cancer) ldl level drops, so the correlation is that the certain disease makes ldl level lower, not that lower ldl 'invited' the disease. What do you think about that?

As MrMoonshine says, various diseases and injuries from heart attacks to viral infections cause cholesterol to decline. Studies of employed populations show low cholesterol is associated with lower mortality risk but in general populations, including unemployed and retired people, the opposite us often seen.

@@tomgoff7887 But that's not what the study proved. They had the medical records for those 13 million people and drew the data based on that. It wasn't their cholesterol levels at the time of death, and even if it was, the argument still doesn't explain the massive signal.

My last cholesterol test was Feb 2023 Total Cholesterol 639 HDL 45 Triglycerides 190 LDL >350 off the charts basically Glucose 69 I don't smoke or drink... This is while giving myself a shot of Repatha once every 2 weeks. They tried statins after fighting with my doc about it for years, but they make me like crap so I just refuse them now. My Dr wants me to try the shot...so I am. He doesn't mind semi high cholestoral but he's convince mine is to high. I'm 6'1"...I lost 90 lbs keto from 280 to 190ish 8 years ago. I still weigh 190 today. I am now 99% carno with. I drink coffee, and I'll have the occasional apple or mango or fruit in general but really...not much. My cholesterol has been high since I was in my early 20s...I'm 47 now. I'm a brown belt in BJJ and trained 3 times a week for the last 8 years....I'm semi active I suppose but I'm not killing myself doing BJJ. Am I at risk...I have no clue what the right thing to do it. I also am suffering from some sort of histamine intolerance. This is why I am carno...it the only way I can eat otherwise I feel like I'm dyeing.

Not LDL but inflammation (from sugar and seed oils) cause CVD. I eat keto since one year and sometimes have cheat days with bread and chocolate. My LDL has gone up to 280, HDL 85, TG 80, HbA1c 5,2%. I am lean and do sports, 53, in peri menopause, no symptoms. I do not worry and will not change my diet.

If you were being followed by a doctor he/she would tell you to get on a statin immediately. I stopped with my world-famous Johns Hopkins cardiologist because all he cared about was the LDL number. He did not care to look at LDL particle size, HDL going up, triglycerides going down, A1C and VLDL-C being very low.

As a lean mass hyperresponder, I would like to add a comment about CAC to this excellent conversation. I recently had my first CAC test at the age of 72 and my results were 287 in the 60th percentile for caucasian men of my age. Because calcification of coronary arteries takes decades to occur, and because I may have only a decade left, most of its value in my case is retrospective, a snapshot of one's cardiac history, rather than predictive. I have only been on ketodiet for two years, and until Dave tells me otherwise, it is unlikely that this particular value should change significantly year by year. The predictive 10 year risk of a myocardial infarct changed only from 13 to16% given my other LMHR values, and that may be enough. But I do plan to repeat it in 5 years, hopefully as part of Dave's next research project. MBS

Great information. Any chance interviewing Dr. Malcolm Kendrick? Dr Kendrick does a great job explaining about cholesterol in his book "The Clot Thickens: The enduring mystery of heart disease" on exactly what causes hearth disease. He also explains a lot on the misinformation regarding to cholesterol specifically LDL.

@Nutrition with Judy Excess Vitamin A can raise triglyceride levels. 1 stick of butter (114 g) has 50% DV of Vitamin A according to Cronometer. If people are eating more than 1 stick per day and have other significant sources of Vitamin A (liver, kidney, eggs, other dairy), they might be overdoing the Vitamin A.

Great interview! I have trouble, though, with the confident assertion that high BG in the context of low insulin and blood glucose stability doesn't matter. Is there science to support this? High blood sugar is high blood sugar - not a good thing. Talk to Dr. Cywes about this. For me personally, when I upped my fat and my fat/protein ratio, my glucose went from close to 100 to 85. And no, my triglycerides did not go up from the increased fat. While it may be true that for carnivores like me, higher BG numbers don't matter as much, I don't think there's enough research to know this with certainty and to laugh at those who worry about higher numbers being a problem.

Great conversation. Absolutely agree that too many people eating/ drinking too much butter !! Agree if they want to follow our ancestors, then they wouldn't have had access to modern pasteurized/ homogenized/ processed butter. Plus at the end of the day, too much fat is still too much. All good things have limits.

I also think that the signaling is affected when you eat vegetables because your body has to figure out how to get them digested, and how to get the nutrients out of them if possible

Fellow Keto with high LDL, low TG and high HDl. Not yet convinced we fooling ourselves as LMHR is safe. Going to cut saturated fats whilst try preserve ketosis

I have total 354, hdl 101, ldl 246 and triglycerides 54 after 6 weeks on carnivore. Bmi 20.8 My appointment with my pc is next week. I know she's going to recommend a statin.

It would be interesting to see the numbers of people who go pure carnivore with animal-based oils, vs carnivore with plant-based oils, and like a mixed Keto diet just to see if perhaps Sterols from plants are making an impact, I think Dr. Paul Mason brought up something interesting with plant sterols impacting cardiovascular health that isn't quite present if avoided as plant sterols can accumulate in the body differently than animal sterols. Perhaps that could explain somethings with LMHRs

lol When Dave says eating animal fats is "mechanistic laziness" around 56:00, I think what he meant to say was "ultimate efficiency". Given the choice, why would any animal consume nutrients out of proportion to what their body needs? That would be evolutionarily inconsistent. The fact that humans can capture and consume animals that contain essentially the same nutrients in the same proportions as what occurs in the human body tells us that we evolved as carnivores, made flexible by tool innovation that enables some level of plant consumption.

64 y/o. High protein low carb for 8 years, lost 130 lbs, low triglycerides, high HDL and LDL. Doctor tells me I need a statin every time I get my blood work done. All other numbers fantastic, no other health issues, complaints or diagnoses. Even if I got a CAC and the score was high I wouldn’t take a statin. What would be other treatment options? I won’t change my diet, period, and even the AHA has said there’s no connection/correlation between dietary cholesterol and heart disease.

I just had to listen to this three times. So much interesting given in the usual comprehensive Feldman style. Your last point, Judy, is spot on. I had decades of text book perfect chol levels, yet was riddled with auto immune diseases, burgeoning NAFLD markers, was pre diabetic and had loads of allergies. After going carnivore, I had my bloods done two years in. Not fasted, I might add. My GP lectured me on my high LDL, but REFUSED to look at every single other marker being optimal. I told him I'd take the risk of higher LDL and no auto immune diseases, and the rest, over having all I lived with for decades. He could not comprehend the logic on my point. All he saw was LDL and the need for a statin.

Same here! From moving to a carnivore way of eating, I dropped over 20kg, reversed my type2Db - now have a non diabetic HBa1c. All aches and pains disappeared. My HDL was really good, Triglycerides very low and LDL had gone up quite a bit. My Dr focussed only on the LDL! I had a CAC test which was zero. I’d had ok cholesterol in every test for the past 20 years yet I was obese and developed type 2 diabetes and was unhealthy and unhappy. I was very stern with my Dr and told her I’d done a massive amount of research - way more than she likely had and that I was fine with my results and never to offer me a statin.

My A1c went up as I went from keto to more keto carnivore to 5.7 and 6.0 but maintaining blood sugar close to 100.

More people should insist on getting a CAC score prior to taking statins. My doctor was pushing statins . . . my CAC score is 11. Very clean for 72 years old. In another few days, I will have blood drawn for an Advanced Lipid Panel (particle size of LDL, etc.). Then I will do an exercise stress test. I expect all three tests to indicate a low cardio risk even though I have high LDL after going on low-carb (my HDL is 98 and Trig 58). Worried about LDL? Look into K2.

I just did an interview with Dr. Baker this morning and have been carnivore for last 7 years and looking over all my labs over the last 10 years I realized I am a lean mass hyper responder and I have all the labs to show. My HDL went up to a healthy level so did my Triglycerides go down my VLDL are great but my cholesterol went up to 310.

You are in the same boat with me and others. Doctors do not care what any other number is except LDL. They do not worry about particle size either. They must toe the line of Standard of Care or get dinged.

I am pretty active. I strength train 4 days,2 days of cardio. I’m on my feet all day at my job for 10 hrs. I wake up at 5am and workout right away. My morning glucose is always high over 100 everyday. Decided to wear a CGM to see what’s going on and it is high every morning. I don’t know if my body knows that I will workout first thing in the AM and gives me more glucose to use in my workouts. It will rise some more during my workout and come back down after I eat my first meal. It’s elevated over 100 all day while at work then come down when I’m ready to go to bed and stay in the 70-80 while asleep then 5am comes and it shoots up again. Is there something I need to worry about?

My S-cholesterol is 7.15 mmol/L, S-Trigl 0.42 mmol/L, S-HDL cholesterol 3.42 R-LDL, Martin Hopkins 3.42 mmol/L, and R-non HDL cholesterol 3.72 mmol/L. I wonder how much is that in US units. For the context: no heart disease in my family. I was always on the higher end with HDL around 5 when I ate low carb (max 50g carbs daily). For the last 3 years, I went under 20g daily and increased fattier meat cuts and saturated fat and my numbers increased. But I felt better before this. I'm lean, less than 50g, 35 years old and I eat low carb no processed food for almost 2 decades now. I believe context matters very much. I see people in the US are better with saturated fat whereas here, in the Mediterranean I was better on Olive oil, goat yogurt, leaner meats, small fish, and walnuts.

I would love it if Dave did a glycine experiment. I always think when carnivores' blood sugar goes up that their methionine/glycine ratio is off. Google that ratio and Chris Masterjohn for more info on how much glycine to take. I'd be curious about blood sugar and cholesterol numbers if he took 3g glycine before each meal.

The biggest limitation of any discussion of a carnivorous diet is the lack of research. All of the studies on an animal-based diet were either very short (a few days), case studies of Antarctic explorers or victims of scurvy, or reviews of cultural practices which always involve at least some plants. Studies that last a few days can only tell about short-term effects. In the longer term, poor nutrition may cause thyroid and hormonal problems, nutrient deficiencies, and other health issues. Proponents of the carnivore diet claim that the human body’s nutritional requirements are different after several months of eating only meat, but there is no research to back them up.

I'm Vivek from India, I'm 23 years old. I was diagnosed with osteopenia at 18. My bones are still weak. There's been a very tiny improvement. I, for about 22 days, have been eating one carnivore meal (buffalo meat with about 40% fat, eggs, ghee) And one normal meal of whole wheat, lentils, cooked vegetables. I'm feeling tightness in my chest from last 24+ hours. It's in the left side. It's pretty pretty noticeable. And I have felt shortness of breath since I was a kid. I went to a cardiologist Here's the reports 👇 So my total Colestrol is 291 (it should be 0-200) My Ldl is 205 (it should be less than 100) Triglycerides is 113 (it should be 0-150) I'll see the cardiologist tommorow again.

From the looks of your labs, you are eating way too much carbohydrates in the form wheat, lentils and cooked vegetables. Avoid those and see what happens.

Actually, I understand Dr. Tro's discomfort with high ldl. He has patients in front of him. Agreed, the info he has is not optimal and don't really apply to lmh but still that's all he has. He had to go with what he has. Dr. Cucuzzella was my doctor before he went to the VA and he was the same way. Dave has the luxury of being a researcher.

I fit the profile of a hyper responder but my lipoprotein (a) is under 10 . My insulin high 20.5 keto 4 years. Question high insulin driving high lipids ? Triglycerides 117, 320 cholesterol 40 hdl, 255 ldl non hdl 280.

Bill Harris is the Omega 3 Index guy. I think the US test lab is Omegaquant. The one here in Germany is Omegametrix. I did it a couple of time now and it's 80 bucks. It's from a dry blood sample and they are looking at the red blood cells (or some short cut), I think and say it's very similar to most other cells in the body.

My understanding is that by the time calcium shows up in your cac test you already have advanced plaque buildup. It's way too late in the disease process. Apparently there are tests that can catch it earlier.

At approximately the 1:05 hr time stamp the comments on ‘trusting our bodies and eating what makes as feel good’ is a very odd comment. Most people eat for pleasure and that obviously makes them feel good but they are not healthy. Can’t fault people that are searching for scientific guidance and trying adapt their diets that seems to have scientific basis as opposed to giving into dopamine responses.

Something def seems off are you truly no carb? Most strict carnivores have much lower trigs. I wouldn't worry about your LDL period, plenty of studies showing the higher your ldl the longer you live. And btw its only a snapshot in time, your levels can vary widely through out the day. Time of day, stress, genetics, time of year, recently sick, etc etc

Mines like that when I eat carnivor or very low carb with a lot of fat, numbers get better with less fat and having nuts, oats as my carb intake. Personally not everyone suits one type of diet also I would worry about inflammation in your body more than cholesterol numbers

Did you fast 12-14 hours before blood test? What are your other markers? A1C, fasting insulin, hsCRP. What was your cholesterol before you start carnivore/LCHF? Did you rule out FH? Maybe do a NMR lipid profile to see if you have mostly large LDL particles or the small dense ones.

Why do none of the carnivore talks never mention low HDL? There's a million about LDL but I want to know if having lower than avg HDL is bad for me. I just can not get it to go up and Im nearly 100% meat organs and fat.

Maybe I missed it, but an hour in and I've yet to hear context for butter. I heard "eating sticks of butter", still with no context. Multiple sticks of butter per day? A stick a day? Granted I was working and may not have focused in on certain parts, but for being over an hour long, that dude said A LOT without saying much at all. A lot of technical jargon and analogies without giving a straight forward, to the point answer on anything at all. Is it just me or does the tag, "Choosing healthy fats" not actually mention a single healthy fat? Judy asked a very specific question and he just said he doesn't really avoid seed oils to the extend of others and he eats a lot of saturated fats. Again, he said 10 minutes worth of content and stretched that out with a lot of useless words to make himself sound credible. I'm tapping out at the 1 hour mark.

I am sure there is a story there but you have to admit yesterday's was a bit too technical for my 3rd one. I get we are all trying to figure it out. What is it she is trying to teach us. She sounds like the 2 talk show hosts that used to be on SNL. The quiet polite and soft taking ones. That skit was hilarious. So no I am not trying to stir things up. I am the guy that came in at the middle of the lecture and I am trying to figure out what we are talking about. Sounds like you are the guy I am sitting next to.

I listen sometimes but she is so inconsistent with what she promotes IMHO and her guests sometimes like this guy are talking medicine as a engineer so I take it with a huge grain of salt :) Look up on YT Dr Chaffee Dr ken Berry Dr Shawn Baker And watch them for carnivore info and another viewpoint from doctors that have been doing it a long time Myself carnivore meat salt water guy 59 yr old 5’11” I am back to my prime weight of 175-180 lbs and feel so good no more joint pains no more brain fog super good sleep etc... but IMHO worth looking into after trying so so so many dif ways to get into super shape and this is working so insane good and I just feel so insane good and my lab work was good a while ago might have to do it again in the near future but for me truly life changing

Saying high LDL isn't a risk factor for disease is preposterous. Literally every healthy lifestyle change lowers LDL. Quitting smoking, exercise, losing weight, reducing stress, literally any healthy activity you can imagine lowers LDL because having a high LDL plays a causative role in diseases.
