I've just had my labrum repaired. He did the FAI plus found a torn ligamentum teres, and floating piece of cartilage (neither of which showed on the MRI). I did eight months of PT with no real change. Some glute strengthening helped but I could barely make it around the supermarket.Give rehab a solid try - cycling high resistance, laps swimming with fins, and leg press stuff. If no good then get to a surgeon.

What if you have painful labral tears with hypermobility, but no FAI or dysplasia (verified by 3D CT). Are outcomes similar?

you can still have great outcomes, just need to ensure those characteristics are addressed with rehab post op

Can't surgery for FAI still help symptoms caused from dysplasia if the surgeon also tightens the capsule? I'd assume it would have to be more of a borderline dysplasia?

Not for the long term if the main issues is instability. It is the indentification of what is driving the pain that is the difficult thing to answer.
