The interesting part is that Cyanide induces a version of darkness that fades in and out, and also, how it removes all your hearts you have - containers and everything. So if you had any additional lives, 9 Lives would be alright to have, but having anything that doesn't give you additional hearts would be a nightmare as you would die on hit *and* you need heart containers in the first place to even survive being sneezed on.

Since it's just a health zeroing effect exactly like taking damage while temporary-lost'd it also seems like Berserk! effects could be used to persist after the timer ends if you at least have a nearby health source at the ready too.

basically each Hallowed Ground act as a damaged check so if you fail the first 50/50 you get a second 50/50 using nerd math it's basically a 75% chance to negate damage when you stand in between both

I like the 💀pill(don't) but I think it could have been a bit better (or should I say bitter) what if, the 💀pill (don't) item gave you a chance to get the pill after you clear a room(kinda like a room clear reward) and taking it gave you a great stat boost, but each floor there would be a leathal dose, it would be a number from 2-5 and if you took a 💀pill (don't) that number of times Isaac would die. I think that this could add a risk and reward like feel to the item and spice it up a bit (sorry if I misspelled anything I am not a native English speaker)

A challenge where you have Little Baggy and Deadly Dose, you're always on the Cyanide timer, and non-cyanide pills reset the timer instead of curing it would be interesting... _(Little Baggy would be included in the starting loadout just to make sure you can't kill your run with Starter Deck.)_ I wouldn't be surprised if such a challenge already existed in Fiend Folio.

I thought cyanide pills would give a permanent stat up at first that stay after the timer is gone. But after some testing I discovered that the stats vanished which basically ruined the item for me. It would be pretty interesting for an item to make pills deadly like this but more easy to get various stat ups (though not as drastic as they are now)
