Isoniazid is an antimycobacterial drug mainly used in the treatment of tuberculosis. It penetrates macrophages and is active against both extracellular and intracellular organisms.
Daily dose of isoniazid is 300mg. It is given orally & is readily absorbed from the gastrointestinal tract, optimally on an empty stomach. It is a prodrug that is activated by KatG, the mycobacterial catalase-peroxidase.

Activated form of isoniazid forms a covalent complex with an acyl carrier protein (AcpM) and KasA, a beta-ketoacyl carrier protein synthetase, which blocks mycolic acid synthesis. Main adverse effect of isoniazid is hepatotoxicity.
