Hi Guys!
If you're considering using intravenous immunoglobulins (IVIGs) for your treatment, it's important to know the side effects. In this video, we'll discuss the most common IVIG side effects and how to avoid them.
IVIG is a life-saving treatment, but it can also have some side effects. In this video, we'll discuss the most common IVIG side effects and how to minimize them. We'll also discuss how to deal with IVIG side effects if they do occur, so that you can have the best possible experience. In this video we get into some of the most common side effects that you can expect when infusing IVIG and also go over some of the more serious potential side effects. When you start this therapy it's important to be armed with as much knowledge as possible to ensure you're armed with the most up to date and comprehensive information.

Spot on with the the information! I am a retired RN who was an infusion RN for 44 years. I just had my first course of IVIG last week for GBS. I love you “Pop Ups 😂

Hi Arlene! Thank you :) it’s always awesome to get positive feedback from a colleague. Hopeful that you’ll have a good response to the Ig as well. We have so many with gbs on Ivig and typically it makes all the difference. And lol those pop ups are my way of correcting after I’m seeing all the faux pas I’ve made during filming 🤭

I have had 2 favorite infusion nurses over my over 25 years of taking it so miss them both they both retired nurses just are not the same anymore there nice they just don't seem the same anymore I use to be a CNA so I know life gets busy but some people don't have a family or if they do they just don't understand what you go through in life

I would love to get more info on how to better control the dull, bone crushing pain that happens during infusions and the days afterwards. Beyond that, I would like to get more info on the IVIG side effect of sudden delirium. I have had both of those as someone with hypogamnaglobulinemia in all 4 IgG subclasses.

This was helpful. Don't know yet if my dysautonomia is auto-immune. Mayo is cooking the sample now. Neuro thinks probable and IVIG would be next step. Just trying to be prepared - just in case. Thanks

IS HIGH BP DURING INFUSION A SIDE EFFECT. I had four treatments and reactions twice. The recent reaction I was shaking uncontrollable and tightness feeling and feeling cold my BP shot up to 191/ 84. T he infusion was stopped and I DID NOT GET THE WHOLE DOES. I have two treatments left but should I have them. I would be expecting another reaction.

I’m getting IVIG for Cerebellar Ataxia. I’ve been getting worsening of my Ataxia symptoms after the infusions that last 3 or 4 days. Why would this happen, if it ultimately helps?

My pre meds are Zofran, Advil and Benadryl. The Benadryl gives me RLS, so I take a Requip

I am having Ivg treatment. I went through chemo and rituxibam sub q for two years as maintence all for NHL . I have the wadenstrom rare am a woman. She is giving it to prevent infections as my immunize system is low. I HAD SURGERY FOR BREAST CANCER RADIOTHERAPY and a catheter in my chest for a month for fluid in the lungs. I am a nurse retired . THIS I AM VERY ANXIOUS ABOUT. THE DOCTOR JUST SAID THERE WERE RISKS BUT DID NOT EXPLAIN. Thanks for the hydration tip. had covid treated at home with paxlovid two rounds. I was severely dehydrated had to go to hospital to get iv fluids. TOLD TO DRINK 2 LITRES A DAY . I HAVE HAD A HIGHER CREATIN LEVEL .IT ID COME DOWN BUT NOT NORMAL. SHE THE DOCTOR IS TESTING FOR PROTIEN LEVEL AND CREATINE IN THE NEXT VISIT IN JUNE is getting I G beneficial as compared to the risks. WITH CHEMO I had to go twice for high temp to ER . These side effects sound worse. AS A NURSE NOTHING IS RARE IF YOU ARE THE ONE THAT GETS THE DIEASE OR SIDE EFFECTS.

Hi I’m lupus patient I have lupus nephritis my kidney are functional I do have kidney issues not CKD have u seen people with lupus nephritis get Ivig my drs looking at this for me from asthma issues n pneumonia keep happening ?

I’ve been getting IVIG for Cerebellar Ataxia for almost 3 years. The side effects that I experience are worsening of my Ataxia symptoms for 2 or 3 days after infusion. My balance is worse, and have more disequilibrium. 😂After about 3 days after infusion, it gets better. Any idea why this happens?

Aseptic meningitis is a terrible side effect. I have myasthenia gravis. After 4 IVIG sessions with bad headache after each one, with a wonderful infusion nurse and a supportive neurologist, who prescribed meds to help, the last one was the last straw. It is 2 weeks since, worst migraine ever, blood pressure sky high, wearing double sunglasses, hiding in my house with chills and sweating, I have decided to stop IVIG. I'll keep my mg symptoms.

I found that drinking plenty of water took care of the headaches for me. However, I suffered with them for quite a while until a nurse told me it would help and it did!

Hi Gordon! It depends, IVIG is used for many conditions. If it's neuropathy then typically an EMG and skin biopsy which looks at the health and numer of your small nerve fibers. I've had six IVIGs so far and am scheduled for six more over the next six months. I was sitting in the recliner chair hooked up to the IVIG during the first one, and within the first hour, I began to feel very cold and began to shake uncontrollably. I asked the nurse for a blanket, and she quickly came over from her desk and went & got a warm blanket for me. She then slowed the infusion, and I began to feel better after a while. However, after each subsequent infusion, I feel like I have the flu the next day, with headaches and muscle aches. I've had staphylococcal pneumonia, Pseudomonas aeruginosa, and cryptococcus gattii during this time. I still have all of them. Due to high doses of Prednisone used to treat my asthma, my immune system's T cells have been reduced by half. But I must admit that I am still alive today thanks to medical intervention in the form of antibiotics, fluconazole, and IVIGs. But it’s a battle.

Hi Stoney! These are all expected reactions and the nurse did the correct thing by slowing your rate. Typically with each cycle these side effects improve however hydration is also key. I'm assuming they're running a bag of normal saline pre and post infusion and administering tylenol and benadryl as well. D

Thank you for this. I did IVIG late 2021- March of 2022. Now I’m back to needing 6+ rounds, due to hemolytic anemia. This is helpful to refresh my memory.

@firetech700 if you are comfortable, can you tell me if it helped with the anemia? I’ve struggled with a red blood cell anemia they can’t diagnosed after I had Covid for over a year requiring frequent transfusions. I had two days of IVIG this week and praying it will help. Thank you for any information. By the way, it’s the day after two days of IVIG infusions (my first infusions) and I have a wicked headache. I’m hydrating and hoping this won’t last too many days

@@kristiny4727 ask them to do PREEMPTIVE Measures, on next infusion. And yes it definitely helped me. I will say, I had headaches with IVIG. But I had KILLER migraines with Truxima. After failing 15x of Truxima, they swapped me over to IVIG and I loved it.

Hi, Great video. I have had CIDP for 25 years. Unfortunately, I wasn't diagnosed for the first 15 years. At that point, I was quite severe and was finally referred to a neurologist. Unfortunately, again, she was a terrible neurologist. She put me on a huge dose of prednisone. I had the rarest of the rare side effects to the prednisone, which the doctor had never seen and knew nothing about. She refused to acknowledge these symptoms as having anything to do with the prednisone. It was horrible and a very long story. I am now suffering with severe osteoporosis and other awful problems due to the prednisone. I finally was able to find a wonderful neurologist who put me on IVIG infusions. I have been on for 8 years now and it has helped me quite a bit with my peripheral neuropathies. The problem is that in the last year, I have developed CKD and extremely high BP. What can I do? The IVIG is the only thing keeping me functioning at all at this point and I don't know what to do. I have an upcoming appointment with the neurologist in a couple of weeks so I will see what they have to say. I only see the neurologist every 6 months. I am on the infusions every 2 weeks and am on 40 grams each infusion. I tolerate it very well. I just have some fatigue for 2 to 3 days afterward. Any advice you have for me would be so very appreciated. Thank you so much!!!.🙏

Hi Grace! Thanks for your questions. I'm so sorry to hear all you've gone through with the Prednisone and poor medical care. I'm really glad to hear you've found a better neurologist. The 1st question that came to mind was what brand of IVIG were you on? Privigen, Gammagard, Gamunex-C, and Panzyga do not contain sucrose as a stabilizer and are better to use with CKD because of this. Panzyga also has FDA approval for CIDP and has a low osmolality which is very helpful with renal insufficiency as well. IVIG is not inherently contraindicated with CKD...it depends on your GFR and benefits outweighing the risks. I've had many patients on IVIG with stage 3 CKD and do fine with close monitoring. Hydrating of course is important as well to flush your system, slow infusion rate as well. I'd discuss a potential brand switch (depending on your current brand) if you happen to be on one with sucrose and high osmolality. 40g is an average maintenance dose so it's not high where I'd be concerned. Dear Friend as a parent of a CIDP Child who is also using IVIG at least every 3 months kindly remember to check your Vitamin D and Blood Ferratin levels it would greatly help in helping your recovery process if your able to keep these levels safe tolerant upper levels

I'm newly diagnosed with ocular myasthenia Gravis. I received IVig x5 dose April 17 , had a follow up and my neurologist recommended Rituximab as next treatment. Please explain the side effect and function of this infusion.

I have been on IVIG for over 13 years. .5gram per kilogram every 3 weeks,4 days in a row. I run a flat rate 110 mils per hr. I did titrate early on but home health nurses wanted to speed up infusions so they could go home. Without regard for my side effects. I now run a flat rate and side effects still come but are much less severe. I found that has been the best way to deal with that issue. I have had nurses miss my port during access several times over the years. Talk about pain, that will do it. Missed port access is my biggest fear now. I am concerned every time my port is accessed. I have brought a mirror to the clinic and shown the nurse the bumps/ target area of the port just to have her at a later date miss my port. They confuse the catheter exit point on the reservoir as a bump. I keep telling them two bumps on the top and one at the bottom. I have alot of anxiety over this issue. Thanks for your video. I have been lucky to have not had bad side effects over the years.

Hi Rich! Thank you for commenting and sharing. I hear this all too often unfortunately. I'm glad you were able to enforce the 110ml/hr top rate. And how in the world do you miss a port access?? That's the whole point-- ease of access. Sometimes it can be (I guess) challenging if it's a dual chamber but even then...Part of me secretly wonders if the nurses who are fairly new or recent grads just didn't have enough exposure. I've had several patients tell me the nurses lately seem to really be struggling with access, line insertion, and infusion concepts. You may have given me an idea for a future video. Danielle

The side effects on my infusion you didn’t said anything I didn’t have any headache or allergies but like 4 hours later my blood sugar dropped and I am not a diabetic I knew about the need to be hydrated but one of my current problem is that I am suffering from chronic dehydration for a long time it doesn’t matter how much water with electrolytes i drink my body doesn’t absorb my dr was aware of that it was a nightmare but when I tried to contact her she never called back or answer all my emails. The second infusion was worse because now I hav high blood pressure which I never had it before, tightness in my chest, nervousness, shaking my whole body all the time I have to pay myself for hydration but I think it wasn’t enough and many more symptoms and the worse is that fr th symptoms it was prescribed has worsening and making myself unable to sleep and the dr continue to ignoring me some thing I wouldn’t expecting I know her for like 10 years so trusted her now I don’t she have let me down with symptoms I have never experienced to deal by myself and is hard when you have to treat yourself not having a medical degree but is my life and I don’t want to ended that way

I just had my first injection 4 days ago.. Octagam 300 ml for severe MCAS driven mainly in my belief by infections.. bad genes MBL and more.. very complex illness today as all systems have suffered for decades.. i had no issues with my injection and smooth sail.. am i wrong to think that i might need 2-3 more injections before i will feel some change ?(if i respond that is) once every 4 weeks is the schedule for first 6 months. I'm not feeling much different yet.. my sleep perhaps affected like on LDN? but way less.. psoriasis little bit better though.. very surprised by my non reation as im isolated with 2 foods and no human contact and no drugs due to extreme intolerance.. 2-3 months before any change is realistic? I know no one knows for sure, just thought i might ask. Ty kindly G

Hi Somer-set! Thanks for your question. First I just want to clarify when you say injection you mean infusion? To my knowledge Octagam is not available as an injection but back in the day many Ig brands were given as intramuscular injections. I'm glad to hear you haven't had any side effects! For MCAS many will experience results a bit quicker than my neurological people...sometimes the GI symptoms/bloating/ hives etc begin improving in as little as a week after 1 cycle. I truly hope that you experience the same. When I made this particular video I had so many immuno/neuro/hematological conditions I could mention I chose the average and said 6 months-a year. It's very difficult to estimate for everyone as there are so many variables (meds they're on, diet, comorbidities). The Ig can absolutely effect sleep, many will be quite taxed and fatigued immediately after and then gradually feel their energy return maybe within a few days to a week. @@lucidmed3343 Hi and tks so much for replying.. my dr has no time for chat or anything else for that matter,) ofc i meant infusions sry,) but the case with me is i have it all.. i'm one of the most complex cases.. the one all dr's naturopats etc walk away from to be honest.. every system has broken down, esp endicrine, neurological (extreme brain issues, sleep etc) GI is big here, pain is not my problem but extreme intolerances with isolation for a decade and as i said 2 foods yet very affected just by air pollution levels.. not to mention my nemesis, mold. My energy is ok but the mcas and fricton with increased inflammation keeps me home on the couch..no walks anymore but i have ok stamina. My sleep is worse from the infusion.. same as happened with my month on LDN.. i have candida, lyme ofc & co's.. multiple active infections and viruses.. i think candida is the biggest trigger and my 0 levels of Mbl might play part there, (despite fungal drug for yrs) im closer to death than life and i dont know what comes out of this, i fought hard to get this cause my gut tells me this is my only shot... i see people feel difference from under 4 weeks to a year even... i know mine will be slow as the damage.. was just thinking.. i should feel some change after 3 months?? Had Aseptic Meningitis 5 times. Had IV morning, steroids. 10mg morphine didn't take pain away completely Been on IVIg 8 years. I get tired post with body aches too

Question my doctor said this is safe to take while pregnant is this true? He said I would have to stop the treatments in the third trimester of the pregnancy due to risks of getting blood clots and then start the treatment back after having my baby. I’m having to make a lot of decisions. I have a Plexin d1 antibody and I have small fiber neuropathy and autonomic neuropathy. I’m wanting to try one more time for a little girl before my body says I can’t have kiddos anymore.

Hi April, Congrats on the pregnancy! Yes IVIG is safe to use in most cases depending on what the diagnosis is and the benefits outweigh the risks. As with any medication you take during pregnancy you'll need to be monitored of course and the 3rd trimester stop sounds accurate. We've had a lot of patients who are pregnant receive IVIG without issue. I'm so sorry you're going through this but hopefully you'll have a beautiful baby girl eventually 💡

My child is epileptic case .. when he recived ivig he had horrible sizures every month he was reviving ivig the other day of recivig he developed horrible sizures then we refused to give it to my son again

Hi Fatima! Hope you and your family had a nice holiday. IVIG is certainly not for everyone and I'm so sorry to hear about your son having epilepsy.. To clarify, he had seizures after starting the infusions or was he given IVIG as a treatment for preexisting epilepsy? I'm assuming his seizures haven't responded to other types of treatments? Is it normal for me to feel normal the day after but then experience a headache after 48 hours? Would that be related to my ivig infusion?

Hi fvtown! Thanks for your question and yes absolutely. IVIG can cause a delayed headache from hours to days post infusion. Not sure if you're new to IVIG but if the headache continues, and you can anticipate this, then make sure to premedicate with Tylenol or whatever you normally take. You may even want to continue regular dosing (every 8 hrs) for a day or so to try and stay ahead of it. If it gets really uncomfortable you can speak with your doc/NP about adding hydration (saline) or sometimes an extra dose of Benadryl can help. I've also seen IV steroids given as a premed for this reason however many MD's don't want to get into this if it can be avoided. Danielle

So I had my 1st session Monday.. For the most part tolerated well. But since, I have been getting up once or twice a night to urinate.. Wondering how long this will go on. I have been and still keeping water and LMNT, so I don't know if that's the issue or just clearing the product.

Hi Ithaus! Thanks for taking the time to comment. Few questions...1st-did they give you a bolus of IV fluid before and after your Ig? 2nd- do you have HTN (high BP)? If you received fluids then I'm assuming it was either normal saline or D5W. The thing is I don't really recommend LMNT...it's very high in sodium...really high. Yes, it's vitally important to hydrate before, during, and after your cycle however the reason for your frequent urination is your body trying to rid the excess fluid it's holding on to. With IG, hyponatremia can result due to an initial elevation in protein levels in the blood due to the Ig itself (which is a hypertonic solution) and will therefore naturally cause your body to initially pull Na from inside your cells to lessen the concentration. Fluid shifting due to proteins and sodium can get rather complicated but once you understand it it makes sense. I tend to say plain water or low sugar gatorade is best. In contrast gatorade has only about 160mg of Na per 12 oz. LMNT is good for people with dysautonomia or low BP where this is more of an issue. Normally with subsequent IVIG cycles you'll learn how you react and can make adjustments. For the time being I'd say try stopping or reducing the LMNT and see if diuresis improves. Danielle

@@lucidmed3343 I did Not get a pre IV, but Your videos and others caused me to arrive very hydrated. I am on a pretty low carb diet and that is my reason for lmnt, but... I will switch to only water the Sunday before my next infusion and see how that helps. My sodium and Potassium counts were normal at the pre-lab for the infusion. None the less, if it helps me flush out faster, I am Game!! thank you for great info.

oh and BP, and sugar are normal. MY BP directly after the 1L was high, but ~15 min later it came down to near normal.

@@lthaus OK that's great. Ig can drop or elevate your BP...normally lower but I've seen both.

I drink loads of water to prevent headaches. Thankfully I've never had side effects.

I'm having to call lawyers to get the insurance to pay for my gamma globulin infusions. Am I neuropathy is spread all over my body because of it.

Hi! It's been a while, nice to hear from you. Unfortunately this is not a unique reality. I've had patients go in front of a state insurance board to argue their case and within 10 minutes they approved her. Why it has to get to this point is beyond me.

@@lucidmed3343 Yep, I'm glad u mentioned that about the insurance board, may have to try if they keep denying it. I got the neuropathy from Levaquin, which I'm gonna go fly to Hollywood Ca for a doc that treats this type of neuropathy, Dr Ghalili at Regenerative medicine LA for the stem cells and peptides, or whatever protocol will help me. But it's all over my body almost. I'm thinking they wont grant the IGG for me before then, etc. I've been waiting for 3 months now.

Hi Ed! I've been trying for a full day to respond to your comment. This is not unusual but several questions come up when I hear this..was this your 1st cycle? Were you premedicated with Benadryl or an antihistamine? Do you have a rash developing on your hands, feet, trunk? This can be a type of eczema that pops up with IVIG and normally it's treated topically with hydrocortisone and an increase in antihistamine but I'll be honest--this rarely makes a significant difference. They can possibly add a steroid to your premeds (solumedrol) but not sure if you have any contraindications for that. A brand change is often necessary unfortunately. I know this can be a pain since prior auth can take so long to begin with. This rash normally develops around 7 days after a cycle and will tend to inprove over a couple of weeks but then reappear after your next cycle just as it was clearing. Hi Tommy, Which part is frightening can I ask? The more serious side effects can certainly be concerning but as far as the run of the mill ones go if you read the list of potential side effects on the bottle of Tylenol that's probably in your medicine cabinet you'd never take that either ;) D

Hi Pam! that's unfortunately fairly common. Depending on the severity you can possibly try something like Zofran as a premedication. Pre infusion hydration can also help but not a magic bullet by any means. I know the questions I'd normally ask you if I could such as what brand you're on and how long your infusions normally take. I'm assuming you're on IVIG not SCIG as well.

How long do these side effects usually last i just did 5 doses of ivig for suspected gbs..Day 2 of being home and have headaches,dizziness ,bodyaches is this normal?
