I think it is also true that dysfunctional epithelial barrier will release inflammatory cytokines, potentially leading to MODS and issues with tight junctions lead to increased gut permeability and risk of infection????

Agreed - pathology affecting the epithelial barrier tight junctions would cause problems with unregulated permeability and translocation of bacteria (particularly if the immune system is challenged).
