Multinucleated melanocytes with fibrillary or vesicular cytoplasm and eosinophilic cytoplasmic inclusions are usually found benign long-standing nevi. Thus, this finding is a clue for benign melanocytic lesions (I don't recall ever seeing one of these inclusions in a melanoma). I really love this little eosinophilic inclusion because if you zoom in close you can see that it has little radial projections around the periphery and a bright dot in the middle. It reminds me of a tiny sun. ☀️ And I also love this particular multinucleated melanocyte because of it's lovely Touton-esque ring of multiple nuclei. If you look carefully for these multinucleated vacuolated melanocytes, you will easily find them (in big polypoid congenital pattern nevi especially). And some of them will contain these little eosinophilic inclusions. They are often overlooked, but they aren’t rare. Thanks to my friend, Silvija Gottesman, MD, who taught me via Twitter that these inclusions are called Gottlieb Bodies.
