I have backed out of getting the vaccine three times now. Every time I’ve went to go, I get massive anxiety and just can’t bring myself to go in and do it. I am a strong believer in vaccines, but I am just not there yet with the options we have available right now. This brought me so much comfort and hope that we are moving towards so many great things and options that will be more comfortable for more people to move forward with. I am a person who has a history of bad reactions so the side effects of the current vaccines really make me anxious (even though I know that they are mild for most people and the likelihood of me having a serious adverse event is very low) but that’s just my reality and that’s where I’m currently at. I am incredibly grateful for science and for people like you who take the time to address real concerns, fears, and facts with the public without shaming people. Thank you for shedding light on the truth and for helping those of us who need the little extra TLC make educated, informed choices. 💪🏽

I got my first Pfizer shot over a week ago and got only side effect that I had was the slightest of soreness in my arm at the injection area. Only about a third as sore as my flu shot this year. I've had vaccine reactions too but this is one of the easiest vaccines that I've gotten.

CNN had a doctor on and said the big story missing on the J&J is that the efficacy continues to go up over time. You mentioned the 49 days with no hospitalizations. He says they don't know when it reaches full strength. The trial data has been cut short to get early approval. It isn't like they will stop tracking how much better it gets, but it is good enough now to get it early approval. This is really, really good news.

Hey Dr. Z, can you comment on Canada’s decision on forcing everyone to wait 42 days between the two doses of Pfizer or Moderna vaccine instead of the recommended period?

I'd be interested in his and Paul Offits take on this as well. There has been a lot of discussion on this within the US as well when releasing all available doses instead of holding on to the 2nd dose for people. From what I've seen from immunologists on this is that we obviously don't have the data on the effects this could have. However, they are taking an educated guess that having the 2nd dose 6 weeks after the first (instead of 3 weeks later for the Pfizer or 4 weeks later for the Moderns) shouldn't drop efficacy by a significant amount of you are unable to find the 2nd dose in time. They are also taking an educated guess that its better to just switch to the other brand in "extraordinary circumstances" of not being able to obtain the 2nd dose of the original brand. I'm assuming this is what Canada is basing this policy on. However, I'm curious what data will come out about this later.

@@lissaw4 Most of the talks in the US is based on not having the availability for a second dose, when you need it. Canada is different. They are forcing everyone to take it late even if doses are available.

@Alan d'Eon Yes, I understand their point. We just don’t know how lasting that first dose and are you going to end up with the 95% efficacy as well. I can understand the educated guess but we should be driven with data every time we go against the trial or at least leave the option to people. It is all a trade offs! More population with 60+% protection or less population with 95% protection, especially in the high risk groups.

I am participating in the J&J clinical trial for this vaccine and the day I got jabbed I woke up with a 102.7 fever later that night - which cleared 18 hours later. No other side effects reported other than minor fatigue for the two days following. I figured that was a sign I was given the real vaccine as opposed to a saline placebo, but it is interesting that a large percentage of the placebo group had a serious adverse reaction. I have not gotten sick though!

The phase 1/2a trial showed additional improvement in the titer counts after a second shot of the J&J vaccine. I suspect they'll offer a booster or "upgrade" of a 2nd shot when they get FDA approval further down the road. Btw, I'm a volunteer in the phase 3 trials for the Ensemble 2 (two-shot version) vaccine.

The moderna and Pfizer concerns are due to severe allergic reaction. mRNA is quickly disposed of naturally in the body, (also why it can cause a severe reaction). People are concerned with the long term risks, but mechanistically the odds of their being long term effects are so infinitesimally small it’s not worth worrying about.

Once this pandemic is under control then News channels will actually have to do journalism and find storys. This pandemic has everyone glued to the tv looking for info because their lives are on hold. The media knows that and they are fighting for clicks.

Big fan, leaving your FBook show only because people have figured out I am some kind of Pharma "whore," and I am working way too hard in clinical and epidemiology to deal with that. But will rejoin your Tribe on this venue. I worked in HIV & Hep C for 22 years, and HIV-TB before, and then contraceptive clinical trials before grad school (Hopkins). Will share this clip widely, even my vaccine denier friends light up now when I mention that J & J / Janssen product is a one-dose vaccine. You said natural immunity might not last more than a few months. I had it in December and therefore not in any rush to get the vaccine. I would assume that if it were just a few months wouldn’t we have a lot more cases of reinfection than we do? Do asymptotic cases not give the same immunity?

A Denmark research team found that, at about 6 months after initial infection, protection against repeat infection was approximately 80%, with no significant difference in reinfection rates between men and women. @@johns8620 no you don’t get a choice of what vaccine you’re getting. For example In Europe it would depend on what’s been approved and at what costs. In Greece we’ve been told we have got the Moderna. Astra Zeneca Pfizer and Moderna are the main distributors currently, and it will depends what countries can store what vaccine correctly. I hope this one dose is approved.

It depends on how long the immunization immunity lasts and how much the virus mutates. The flu viruses mutate morders of magnitude faster than the COV-2 virus. It is too early to guess, but immunity in the Phase 1 trials still seems to be holding up, and those shots were given about a year ago.

I have a lame question....in the covid vaccine trials, are the subjects vaccinated and then inoculated with covid 19 virus and their response to be exposed recorded to measure efficacy?

It's not a lame question! Lots of people don't know how vaccines are studied. Generally, researchers will only give the vaccine, wait until a threshold number of people get sick just living their normal life and then compare the entire group of people who got the vaccine to the control group who got a placebo. Although, I did see some researches wanted to deliberately inject vaccinated people with COVID to see how their bodies would respond. But that did not happen for the initial studies.

Does the efficacy of the Moderna and Pfizer vaccines also increase beyond the time window they looked at in their respective studies? 95% is already high enough there's not a whole of room for improvement, but I'm wondering if we might see an improvement in efficacy against the African B1[fill-in-the-blank] variant?

This will test if two doses improves how effective it is. It sounds like 1 dose will protect the individual. Will 2 doses protect the individuals ability to spread it? So the 1 dose can stop the healthcare being overwhelmed. Then they can potentially have a booster to stop transmission, a longer term goal.
