It isn't. Both the book and the movie are set around WW1. The sort of medical technology needed to save and keep alive someone who has been this gravely injured simply did not exist in that decade. They might've been able to stop any bleeding, but the infections and other health complications would have been impossible to address. The story is a hypothetical depiction of just how horrible war can be. It can turn a perfectly healthy, able-bodied, and optimistic young man into something that is *less* than a shell of his former self. Medical technology of this kind would have been science fiction for the time.

(and the nurse comes in takes off his mask puts a pillow over his face cutting off his air, Joe finally feels happiness again as he knows his life is about to end and he is thankful to this kind Loving nurse, he hears the voice of his father and his mother calling him home he runs to them as the life drains out of him, as he runs to their voice with a huge smile on his face, his last thoughts are seeing his parents near his father's garden waving as he runs to them, then he breathes his last breath and dies. The nurse quickly puts his mask back on as if nothing happened. She notifies the doctor that Joe has passed. The doctor inquires how that is impossible when he was healthy and no infection. The nurse looks at him with a puzzled look ) "Dr he was suffering you should be happy that he's gone, why worry about his health?" A pause then the Dr says, "Nurse Joe was our research here to see how long someone could live and we could use him for testing Medical procedures on him and medications he was a valuable tool for us and now we are unable to collect that data, are you admitting to me that you helped assist in his death btw?" The nurse looks nervously at him and states " he was suffering terribly and I just had to put him out of his misery, I could not allow him to go on in such a state everyday seeing him like this broke my heart, so yes I ended his life". The doctor looks appalled and says "how dare you how dare you end the life of this young war hero, how dare you make this medical decision, now we will be deprived of using Joe as a medical guinea pig and won't be able to collect all that data... You know that I'm going to have to call the police" the nurses face reddens and then she begins yelling "you're evil absolutely evil you have no heart, what happened to your hypocritic oath?" The nurse shaking with fury.... The doctor then says "keeping him alive was for the greater good, all this research will be lost now thanks to your sentimental heart, now you can rot in jail" the doctor quickly walks out to call the police and the nurse sits down and sobs sitting next to Joe and rubs his stomach as she says to herself "I did the right thing Joe don't you worry about me, you're suffering is over," tears stream down her face as she nervously awaits the police" police walk in with the doctor the doctor instructs them to arrest her for admitting to the killing of Joe.....The nurse does not resist.... As she is walking out she looks the doctor in the eye and says "Joe is free from all you monsters now, the medical monsters and the military monsters" (fast forward 6 months, the doctor is reading the newspaper, the headlines read) "Nurse charged with the death of War Hero Joe Bonham gets a not guilty verdict" the doctor says to himself "good God what kind of message does this send to the public"?
