It was very much not portrayed as ‘fine’. Don’t get me wrong, I’m not really a fan of the movie, but it was a serious scene and was definitely not played for laughs. The actual act itself also wasn’t shown, just heavily implied. And gender has nothing to do with it. There have been plenty of such scenes before, the vast majority of which involved women.

I always thought that Arthur would never be the Joker in Batman's time because he was old. All of this took place in the early 80s and Arthur was in his early 40s. By the time Bruce Wayne becomes Batman which is 20 years, he would be in his 60s. So the way I see it either Lee's child will become the Joker 20 years from now or the dude who stabbed him.
