My interpretation/ headcanon is that this is the same Joker we're all familiar with. He's at Arkham, and he's creating this story in his mind where he becomes a hero to the Gotham people and ultimately causes the death of Batman's parents. And that is the "joke" that the therapist in the final scene wouldn't understand, that Joker causes Batman's origin. The final scenes in Arkham are the only "real" Joker we see, rest all is fantasy in his head.

This is the best Origin story to be ever made and it gives origin to two of the greatest heroes Batman and Joker.... Like i don't think any movie has ever given such an amazing origin to these characters I wish they used this in The Batman as it has similar dark tone and continue with Joaquin Phoenix as Joker he would have been perfect stepping stone to DCEU it's a missed opportunity that they didn't.......

Laughing Disorder: Not a coping mechanism. A neurological disorder caused by brain damage. He was hit in the head alot by his dad, and possibly even his mom too, when he was very young, and doesn't remember it. He literally feels uncomfortable with his laughter, which you can tell by the gestures he makes and his body movements. It hurts him, but he can't help it. Additionally. Watch the Murray scene again, after he shoots Murray and walks off stage. His suit is red. When he steps into the area slightly off stage to the blue lighting closer to the camera, the blue lights hit the red of his suit and make it look like the classic purple suit the joker wears from the comics, shows and games.
