An Kalitta Air Boeing 747 registration N707CK performing flight CKS690 from Cincinnati/Northern Kentucky Intl Airport (KCVG) to Ted Stevens Anchorage International Airport (PANC). Was instructed to line up and wait at runway 27 but the crew lined up and started takeoff roll without permission.

I think the visualization on THIS channel is perfect.I do not like the other(new)channel because it uses flight simulator of some kind. At that moment fixtion and reality are mixed too close so it becomes less real.

The video and audio are not in sync. Perhaps you have deleted dead air in the audio? This did not happen over a 3min-ish time span. When you change the timeline like that, it alters the perception of what really happened.

@@joedaltow8878 , if you can’t take some heat, get out of the kitchen, flying or discussions about it are not for the faint of heart, sometimes things have to be stated clearly and unequivocally! No reason to sugarcoat it. No participation trophies allowed in aviation😉

@@user-kb8gh5jv9t The tower handled it as it should be handled. That's it. The argue of who might have did wrong is in another place another time. And it is also important to not start blaming the person. Too often it is the system that is at fault. No blame game as it is called.

I always hate it when you have an overworked controller handling too much, and extra conversation on frequency while planes are getting TCAS RAs. This controller did an excellent job of moving aircraft, with excellent situational awareness, but we still need more controllers and better working conditions so they can focus on their primary goal of keeping the blips from meeting.

Great controller and she even recognized the B747 may need a brake cooling time. She handled that with grace, likely knowing full well the B747 pilots did not have a takeoff clearance.

What is a "brake cooling time". The time needed is to set up checklists for takeoff plus a takeoff briefing. The brakes are not hot from the rejected takeoff

@@johnhanson9245 "Brake cooling time" is a time needed to cool the brakes when they absorb the kinetic energy of a heavy loaded airplane they stop. That time may be quite long, but they didn't gain much speed yet so it was not the case.

Its a good thing controllers have been on top of it lately. A few years ago I had a controller clear me to land about 15 miles out and once we got to short final at about 1-2 miles they cleared an aircraft for takeoff from an intersecting runway. I think they forgot about us landing on the other runway. I queried the controller and they immediately cancelled the takeoff clearance for the other aircraft. Stuff happens. Be safe out there.

Excellent ATC! That gal had her head on a swivel! And she handled it way better than most! You could hear the empathy in her voice saying she has to report it and to copy a phone number. Class A all the way!💕🇨🇦

Hey, it costs nothing to be polite, and I can't see that the controller missed anything that needed to be done in the aftermath. The important thing was first to keep everyone safe, and then to keep everyone moving.

She is being nice because she knows that this crew needs to fly an airplane for the next several hours, and the worst you can do to have them in a bad mindset for this upcoming flight, is to give them a hard time. They probably debriefed the event themselves once they were in cruise, and got back on their game. And then got also debriefed on the phone after.

Wow! I can only speculate that the pilot absent mindedly did some routine things and the brain saw they're on the runway so he took off. But, what a potentially dangerous mistake! Great job by ATC!!!!

The other plane was never gonna cross his path, it already had gate instructions, he was in fact up next..however he did commit an error, all he had to do..but I’ve been on plenty of flights when the same happened. Does anyone know if the pilot gets “points” on his license for this or what happens?!

Great controller. Handled the situation professionally, kept radio traffic to a minimum distraction. People make mistakes and there is no need to grind them into the ground with snarky comments.

Yes, of course. But still, IMHO: "Bla, bla, bla... PERIOD - Line.Up.And.Wait PERIOD - bla, bla,bla..." for both sides: Why can't they just slow down and pronounce the most IMPORTANT stuff?? You really barely hear that from both sides! Why??

@@Greattk78pilots are trained and ingrained in their mind to know and understand what ATC says. Tower was being concise and clear and the reason she (and virtually every controller) has to say it fast is because there are other traffic who needs instructions. All the stuff she says is important. There’s no “unimportant” stuff. This was just a case of the pilot misunderstanding/forgetting something. I mean he even read it back correctly.

@@WhenTheTucker I'm not convinced, honestly. "Line up and wait" is the key statement, as accidents on high speeds when landing/taking off are usually lethal. Of course they have to be fast speaking... but IMHO here they overdo it, it even sounds nonchalant. And this might have been the reason for the error here. And I am not asking for slow speaking either. But here it seems to me the pilot was repeating like a parrot being late, without grasping what he is saying.

@@Greattk78I don’t know what you’re talking about. Tower was clear in what they said. They didn’t overdo anything. I was able to understand her perfectly even without subtitles. Whatever the reason the pilot decided to not follow ATC instructions, even when he read it back correctly, is completely on him and he is at fault. Maybe his brain was on autopilot and he just decided to roll out. Maybe he didn’t understand the instructions. Maybe he was checked out and tired after flying long haul. Whatever the reason is, we can only speculate. Tower isn’t at fault here. The fault lies solely on the pilot.

@@WhenTheTucker Of course it is not the tower's fault. But besides correct and wrong there is actually also "precautious". I've heard lot's of operators who simply pronounce crucial things appropriately, instead of monotonous yada-yada. Might have helped here, and it is not about being guilty or not. Best would be to eliminate the human factor here, AI will do a better job in future eventually.

Reminds me of the Tenerife disaster. They actually read back the clearance on this one and still deviated from the ATC instruction. Amazing.

Teneriffe was caused SOLELY by an Arrogant " Popstar " Senior Pilot . 538 people died for his Ego .

@@johncunningham4820 It wasn't caused solely by the senior pilot of the KLM, saying that is just plain ignorant. Read some articles, watch some videos, inform yourself. Yes, that pilot made a big mistake but you can NOT forget ALL the other factors that played a huge role in that crash.

She was extrmely sympathetic and polite considering the B747 crew, despite acknowledging the clear instruction to line up and wait due to the landing aircraft on the intersecting runway, commenced the take-off roll without clearance. She even told Kalitta to "cancel take-off clearance" when she never gave them a clearance to begin with.

Hey! This is my town's airport! Very nice job by the controller. The pilot even repeated back the instruction to line up and wait. Can't believe they started their roll.

Heard about this. There is a video I believe of this incident where you can see smoke roll off the brakes/tires I suppose he hit the brakes hard. I'm a truck driver and work at and around CVG everyday, but mostly Polar and Delta.

Retired F18 25 year CAF instructor/pilot also 5 year Boeing 737 Captain here. After all these years as I near 65, this video is an excellent reminder that complacency kills. Monitor the fuel. Look outside. Watch mother nature. No chit-chat during the sterile period. There are people back there, and I want to get home to my loved ones too. My nightmare has always been to be involved in one of these incidents but Ive never had to call the "number" lol

Professional service like this is the majority every day in the USA where thousands of airport operators occur. The few instances of rude or non professional communications are the exception. Our ATC pros are the best in the world

Negative. The incoming flight was cleared to terminal and had a direct path to the terminal without crossing the outgoing runway. Yes, there COULD have been a problem if the incoming flight needed more room, but it didn't. NONE of this is to diminish the pilot's (and copilot's!) error. I'm only speaking to your Tenerife comparison.

uh oh, when ATC gives you their number it's like going to the principles office. But well done by ATC and Connie 690, very professional. Feel like what may have happened is Connie 690 was monitoring another channel where someone got take off clearance and mistook it for theirs.

The ASRP is a cooperative safety reporting program that invites pilots, controllers, flight attendants (F/A), maintenance personnel, dispatchers, and other users of the National Airspace System (NAS), or any other person, to report to the National Aeronautics and Space Administration (NASA) actual or potential discrepancies and deficiencies in aviation safety. NASA serves as a third party to receive and process Aviation Safety Reports. Examples of operations covered by the program include departure, en route, approach, and landing operations and procedures; air traffic control (ATC) procedures and equipment; crew and ATC communications; aircraft cabin operations; aircraft movement on the airport; near midair collisions (NMAC); aircraft maintenance and record keeping; airport conditions or services; and unmanned aircraft operations. The effectiveness of this program in improving safety depends on the free, unrestricted flow of information from the users of the NAS. @@mannythelimodriver6003NASA ASRS (Aircraft Safety Reporting System). Any pilot deviations, aircraft incidents can be reported. In most cases self reporting relieves aircrews of penalties for mistakes or misunderstandings. The FAA, NTSB, and NASA came up with this to promote safety. If pilots can freely admit to mistakes data can be gathered to help identify issues and promote a safer environment. As long as there is no damages or injuries the matter should be dropped. ATC has a similar program called the Air Traffic Safety Action Program (ATSAP).

There seems to be a lot of unauthorized takeoff attempts since the phraseology was changed from "Taxi into position and hold." to "Line up and wait." Also, the controller, issuing instructions to "Cancel takeoff clearance" implies that the clearance was issued to begin with. A better phrase would have been "Abort takeoff!".... Former ATC controller at CVG.

@@Falcon-um7vo The phraseology "line up and wait" has no meaning when there are 15 planes, already lined up and waiting on the parallel taxiway for takeoff clearance. It's totally redundant, while "taxi into position and "hold" implies instructions to taxi onto the runway and wait for a clearance to takeoff.

Yep, that captain is the reason for that phrase. That, and "departure" instead of takeoff when used outside of specifically giving _takeoff_ clearance. The word takeoff was not uttered here, and still. Good catch from the controller. Fortunately the place wasn't fogged in like that other time.

The problem I note is that the tower told Connie to line-up and wait, then uses the Connie call sign to say "traffic holding", when she should have said Endeavour. But, no question, Connie should have queried it and stayed still.

Listen again to the audio, the call was to Endeavor 5117, the captions were wrong.

The beginning of the call sign is a little cut off but even it's not Endeavor it's definitely not Connie either, and the number is definitely 5117 and not 690. The caption is off. But even if there was confusion over the callsign, instructions for takeoff are very clear. You have to be told "cleared for takeoff." If you don't hear those words, you are not cleared. If you think an instruction was given to you and it was meant to give you permission to take off but you didn't hear those words, you should call the tower back and request that the instruction be repeated.

She told Connie that traffic was holding on the intersecting runway followed by garbled transmission that immediately cut in. If the transmission was garbled for Connie, I can see where it gave the mistaken impression that cross traffic was holding and takeoff clearance was given. However Connie failed to call back that takeoff clearance was recognized, and simply rolled on a bad radio call.

I NEVER said possible pilot deviation. The last thing I wanted was a pilot distracted the entire flight worrying about losing his license. I just gave them our number and asked them to call when they landed. I also didn’t want to embarrass them on the frequency.

I agree that the controller saw the Connie rolling on takeoff without a takeoff clearance. She handled the situation beautifully. I am confused as to why she cleared Connie to line up and wait, told him about the short final landing traffic, then told Connie there was an aircraft on the intersecting runway. She must have meant to tell the landing aircraft that.

Not quite as bad had they missed it - but it was READ BACK correctly. Definite pilot error, good thing traffic was low and the controller caught their mistake.

Great gal in the ATC center. She even gave the 747 an immediate solution to the problem. "Take the first left...you need more time to line up?" Or something like that. She restores a little bit of faith in humankind despite all the monsters out there.

Jets, particularly heavies like 747s, need brake cooling time after a rejected takeoff before they're safe to try another. That's why she offered them some waiting time.

She had no reason to assume that they started their takeoff roll. Unless they have some ground TCAS instrumentation, this was all done by observation. Good diligence and awareness.

Them actually taking off, indicates they thought they had a takeoff clearance. The controller used appropriate, known phraseology they would immediately react to. Hearing that instruction on a takeoff roll, is known instantly, and they would know to immediately reject the takeoff. Using other, ambiguous or conversational phraseology during that phase, could delay their reaction and prolong their take off roll.

@@bradskis81 Incorrect. Nowhere in the pilot/controller glossary of the FAR/AIM is the term "cancel takeoff clearance" found. The proper phraseology would have been "abort takeoff".
