It's more of helping Tony to be aware of the present or his surroundings. Notice how Gaga didn't leave Tony throughout the song? Also, the comment uses is so innocent and uses the word "guide." The comment didn't suggest that Tony needs help singing for he is incredible. It is more of how lovely and genuine their relationship are, supporting one another. Just a reminder that during this time, it is not public knowledge that Tony was diagnosed with Alzheimer's. The family stated that Gaga had been very helpful and sensitive from the start.

Tony is a man who lives on music. It's amazing how it manages to "light up" after listening to notes. Obviously he is not in shape, and after the Alzheimer's revelation some points of bewilderment are noticeable immediately. Despite this he performed in a perfect, fantastic voice.
