What is Dermal Melanocytosis? AKA Hori's Naevus | Dr Davin Lim

What is Hori nevus?
It is an acquired form of face pigmentation that is super common in Asian women. Onset is around 20 years of age. It progressively darkens over the decades. Hori is characterized by dusky grey to blue macules (dots) on the cheeks & around the eyes.
A fancy name is ABNOM or acquired bilateral nevus of Ota like macules. Both entities are due to pigment cells called melanocytes occupying the dermal layer of skin.
Hori is very different from melasma, sunspots, lentigo, freckles & other common forms of pigmentation.
What is Hori nevus?

How common is it?
Hori nevus is one of the most common causes of face pigmentation, especially in Asian women. The clinic sees at least three to six cases a week. Not all patients request treatment. It is not hereditary.
Will Hori pigmentation worsen in time if not treated?
Yes, however progression is slow. Some patients elect not to treat. Others elect treatment when make-up cannot cover pigment.
What are other causes of pigmentation associated with Hori?
The majority of Hori patients will also have what dermatologists’ term as non-specific pigment, meaning elements of dusky dermal pigment, spots from sun & age as well as specific pigment disorders like melasma & Ota. It gets much more complex as some may have associated pigment demarcation lines & constitutional periorbital dark circles.
My team & I approach all cases with a diagnosis, as this confers prognosis & treatments. Hence in most cases we use nana & pico lasers with 532, 730, 1064 wavelengths with pigment correctors.

I have that on both upper cheeks. Doing Pico laser treatment right now (first session) and having glutathione treatment. I am mixed blood, (Chinese & Spanish) and my grandparents' fair skin had white and brown patches which I am trying to prevent in my case hence the gluta treatment. Using kojic/glutathione soap and some mild facial cleanser. I don't go out without sunscreen. So far so good. No post-laser hyperpigmentation and it is fading a bit. Thank you for letting me understand these pigmentations. Your channel has been very informative. More power.

@@komalshahzadi1128 thankfully it is fading gradually. The big melasma patch that looks like a map is already breaking down. Had 3 pico lasers done already every 3-4 weeks. Same routine except I have added a tretinoin cream at night. I hope next year they will not be noticeable anymore.

I've had both q switch and picosend. They were like 8 sessons done, then took a break. it's much worse now, I have deep dermal spots on my cheekbones, that have not budged. Will dermal melasma caused by laser ever fade? Or am I screwed for for life? I'm not Asian at all but I'm yellow olive toned

It at least somewhat should be fading by now if you have kept the sun off of it and avoided messing with any possible breakouts on the same areas. I am olive toned complexion as well and would be happy to tell you what I did to get rid of my Malasma and hyperpigmentation. Anyone reading this please do your research before putting anything on your face these days. Over the counter creams and just about all basic lotions contain some type of retinol, retinal palmitate, various acids, etc..and will cause sun sensitivity causing hyperpigmentation or malasma!! Such a shame these companies are allowed to add these products and altar your skin sensitivity without a person being very educated on these subjects!! I could understand certain facial treatment moisturizers but not body lotions or basic facial moisturizers, cleanses, etc…what man do you know understands these ingredients or wears sunscreen as a daily regimen? I know I suffered from pigment issues a long time not paying attention to one slip up in a hurry and unaware of what I put on my face. These companies need to be liable for this crap!!

I am about 12% or maybe 6% "Asian". I have these spots on the sides of my face. They never go away. I use retinol 4 times a week. Twice tretinoin twice a OTC retinol. I always believed I had melasma. My aunts do. Now I am not sure. Problem is derms are herd followers. If I ask my dermatologist thus, she will tell me to stop playing doc and looking up my condition online like a psycho. But she is wrong. Why won't my "Melasma" spots budge. It's the same. For years. I have been taking pictures. I hate this

Doc, I've tried all those lasers. I've spent a lot of money. pico laser or q-switch 7 or 8 times and even more. but my hori is still there. but not as thick as before. and when I come from the beach, it tends to turn black again. now they offer me salmon DNA. I said no. I have made peace with my hori. At least I diligently use skincare and simple treatments
