Mad Sweetie well, actually he was dead the entire time. This was June imagining what would have happened if she died instead of Carl. This game was her way of getting over the loss of her husband. We as the player did not show either Carl or June that there was no way of saving one without dying, but we were simply helping her to get over it, to move onto acceptance. That's why in the last level you hear her screaming and you find her note book with the events that we stopped from happening and the different set of outcomes that we created. That's why also the world outside of the house during the night is so fantasy like, it was June imagining Carl alive this whole time. What we saw at the end with them switching places was actually what happened. That's why Carl didn't tell her about the crash that was coming, because he didn't know about it. That was June replaying the memory of the true crash and accepting it. This is what I've got from it at least...

@@TOASTEDMARSthat’s a pretty accurate explanation, my only doubt about it is why the freaking wheelchair if Carl was already dead? Doesn’t make sense to me, if the sequence is abstract then it’s strange for him to face concrete consequences, especially considering June isn’t on a wheelchair in the epilogue meaning it’s not a projection either Pretty strange choice. I think it’s only beneficial to gameplay as it prevents Carl from reaching certain places and for artistic presentation, we feel worse for him seeing that and him struggling to reach a can of food, also we understand quickly we failed once we see the chair in the background to create frustration. Still it doesn’t really make sense for June to imagine Carl like that. It’s definitely not a Carl sacrifice though: we have the notebook and the screams to confirm it. The final scene where Carl asks June to drive is what really happened and her realizing she can’t go back to take his place instead. The notebook is obviously her remembering all the stuff that went wrong on that trip and asking what if things went differently. It’s fitting because she was in the driving seat so she blames herself for not driving better I guess. Ps. What the heck was the present? (My best guess is something for the baby to announce Carl she was pregnant, he didn’t seem to know otherwise he would’ve not make her drive).
