Bit of a different video today; I thought as we are prepping for things and I didn’t have much to say I could take the time to discuss how we usually approach respiratory conditions in hospital. I think it’s a bit medical, but hopefully you will enjoy…. you can tell I’m a teaching fellow at heart and I’m still learning this vlog thing! My mother has had an organ transplant and she's really worried about self-isolating for 14 weeks. Is it still safe for her to go out for a run ? She loves to exercise - it's one of the reasons she keeps as healthy as she is, considering. Thank you :)

Thanks for doing this! It's really appreciated. So much fake info out there. It's kinda disheartening. I'm in California, and we're on lockdown. Not Martial Law level, but it's still scary to be told you can't go outside. And I have asthma, so my lungs aren't that strong. I also have six family members in quarantine or isolation (and omg, you just said "asthma!") in various states and countries, including a nephew in Germany, who was in Italy not long ago. My mom is in her 80s and had a lung cancer thing a few years ago, though they caught it very early and seemed to get it all. But still...it's very worrisome. You and Doctor Mike have been so informative! I'd love to see you guys team up some day, "Doctors Across The Pond!" Thanks again, these videos do help!

I only discovered you because of this whole nightmare and I think this vlog will become a great documentation of how this all unfolds, one question... I don't recall you saying where you are situated? i'm sure you've mentioned it in a previous video and i have watched a couple but still not aware of what area of UK your working in? Not expexting the hospital name or anything specific just a general idea of where the hospital is. I understand if you would rather not say but I am very interested to know if your london based or maybe near where I live, in a city or more rural. All the best throughout 2020, medical staff have always been the backbone of society even more so at this time of need. I've limited myself from watching the news because i'm on 14 days self isolation right now and it did overwhelm me last night after i got home from work, it all just hit me at how real this is and i broke down! But this channel has helped give me some hope and a better understanding of it.

Everyone please keep in mind that he has also spoken to others during these vlogs, including a lovely lady previously. It's ok to compliment Dr. Hope but remember, if you ignore the others, including the woman previously, it can develop into a form of microaggression. Imagine how you would feel if you scrolled down in the comments and saw the man getting all the attention. It's important we just remain as balanced as we can, otherwise it can be a microaggression towards minorities like women.

As someone who’s in the thick of it in the US, I seriously hope things go better for you all. 💙 It is serious, it fucking sucks and working with seniors, we are waiting for the bomb to drop. So far nothing in my facility but it’s only a matter of time and we’re hearing about cases all around us. Hopefully we can delay hospitalization until better treatments are developed/ we’ve passed the peak.

I spoke to my aunt who runs a private hospital chain in Western Australia and she didn't seem concerned at all, she said Aus isn't even stockpiling the medications needed to treat CV19 for the coming winter as they expect a vaccine to be ready (deluded) and some of them might possibly have undesirable side effects, even though they've never been experienced ever, only in vitro after half a century of use. Seriously, Australia has a time machine right now, USE IT, I wish we had. Do you think any of the medications are going to be left when it's our summer and your winter, especially when things like Hydroxychloroquine need to be grown from ancient tree bark and the rest of the Northern hemisphere has bought up the supply?

@@ColinFox Yes I work for Sun Pharma here in Navi Mumbai (Maharashtra), my Swedish colleague works a state over in Gujarat for Cadila where almost all the world's supply of Hydroxychloroquine is manufactured, they even make it for Sanofi-Avensis. I told my boss about the shortage in the UK and he gave me an entire box with hundreds of strips straight off the production line, India is overflowing with it... All the Western media keeps asking why India hasn't had a massive explosion in cases yet and I see daily predictions of doom and gloom (especially given the hygiene here), but the answer is obvious, they produce 25% of the world's medications and are overflowing with both drugs needed to treat the illness. There's hundreds of "chemist wallas" on every street even in rural areas, the private price is .60 rupees a strip (of 30 X 200mg), which I believe is currently £0.68 pence... My boss was asking for ideas to create combo packs as a lot of people are uneducated and won't be able to understand the initial vs maintenance dosing of each drug. The saddest part is I can't even send this box to my family due to EU drug import restrictions, despite it being the exact same generic stuff Sanofi-Avensis will be giving you when they finally get approval in several months with a different sticker on. The other issue is that only two countries have the precursor chemicals to make both natural and artificial Hydroxychloroquine, and again it's India and China, and China isn't exporting it right now.

The medical consultants are absolutely worth their weight in gold. Always in reverence of the NHS staff doing an amazing and often thankless job taking us all through this public health crisis. I have worked for the NHS training clinicians and nursing teams for clinical centres. I loved that job. Now I have mobility problems so can’t do workshops and testing anymore. Yep, the NHS has saved 3 of my family me members’ lives so far. My story is that I had a Massive Pulmonary Embolism, weekly PSTD physical somatic lung shutdowns for over 6 months and high protein c on and off. I was 51 and 8 years later I am still here to tell the tale. I have a genetic blood clotting condition which means I never know when the final D day will be. My Grandfather died of it at 67 so I live with that each day and count my blessings that I am still here. I am a little nervous but luckily I am low risk now, I hope, as the cycling and walking I have to do to keep my lungs in trim are life savers. Listening to this on a daily basis reminds me of the hard times I went through. I was homeless, lost my job, my family disowned me as they blamed me for my problems. Even now they never mention my challenges so I have been in isolation in general for 7 years. Imagine. Since I became disabled following a work accident just after my PE this has been my life. Now perhaps we will have more compassion for the isolated and those who through no fault of their own end up with life long disabilities and can never work again like me. Next week I am taking food parcels around to the elderly on my bike. Where I live all the volunteers are over 60. Luckily I am 59. We can all help each other and keep some of the elderly calm and nourished to help the NHS. Love you 😘people.

Being a GP working at the ER, I wholeheartedly agree this is a very strange and confusing time for us doctors. I especially love your conversation with dr. Jenkins, what important points should we look for because honestly the symptoms really are not specific enough until we get all the swab and imaging result, which is not always easily available here. It's concerning, it's like we're waiting for a bomb to drop here in Indonesia as cases rising up each day. But we shall keep working hard and hoping for the best. Yeah. The fact that you get charged to test unless you've been overseas (despite the spread is already local at this point) or interact with confirmed people is pretty iffy. While I understand the limited supply of test kits, this is letting undetected people spread it especially since the government is still letting people up and about and pathetically telling them to 'social distance', here in our rural area, people are still perfectly fine holding gatherings either small or massive. And people with flu-like symptoms still won't keep their distance.

I'm from Italy. To see how the UK approached this virus it's pretty interesting for me. To us right now it's mostly a frantic intervention after another; we're still trying to find the best options to treat the "heavy symptoms" at least until we find something that's actually effective aganist this heavy form of interstitial pneumonia (polmonite interstiziale acuta, that's how we call it, hope the translation is at least coherent). Keep up the good work and good luck to all the medical professionals out there, the next few weeks are gonna be huge for everyone.

The UK is probably ~2-3 weeks behind Italy in the curve. Also, Italy didn't know it had the virus for a while whislt the UK is on the look out. We'll see in a couple of weeks how things hold here

With what I get from the news and other KZbin channels I thought the UK was getting hammered (many, many cases of) by the virus. How bad is the spread? Has it simply not reached your area yet? I’m in the US. 3. I like your “bed head” hair. Your viewers might as well get use to it. You’re going to have less and less time as the virus spreads. You’ll be doing well to shower three times in a week. Flaunt your wavy hair! You’re cute as a button the way you are (I can say stuff like that at my age.). Forget the naysayers. Those of us who care about you don’t give a tick about your hair.

Thank you, I am appreciating your videos, this one particularly so. Dr Steve Jenkins seems lovely, very calm and reassuring. I am isolating as the primary care for my Mum who has severe COPD. I am scared, she is terrified. I have had to reconcile that if this goes as badly as reported it could I may not see my 79 year old Dad again, who lives alone but we are chatting every day on the phone. It is a very challenging and very strange time. Keep up all the amazing work.

Currently on day 4/7 self isolation having experienced fever, chills, headaches, diarrhoea and body aches. Hoping nothing more complex gets me over the next few days. Why is everyone banging on about the state of your hair?!Every junior doctor I've ever known - and that includes the girls- suffers from that chronic medical condition known as 'bedhead' which occurs when a/ you are too knackered to care b/too busy to find a comb c/ some swine has nicked your comb, so you couldn't comb your hair if you wanted to and d/ the shop in the hospital concourse has run out of combs because everyone else in the hospital with bedhead bought what was left. Personally, I'm a bit fond of docs with bedhead.They are usually very hard working and look a bit cute with sleepy hair. Thanks for finding the time to record these informative and educational blogs. Much appreciated. Right.Time to get ready to do battle for another day in the nursing paradise that is ITU..... (seriously, though, and despite the occasional sarcastic comment, I love my job and am proud to work in the NHS, no more so than at this moment)

I had an undetermined respiratory infection (not sure what it was, not even if it was viral or bacterial) last year and went through all these steps exactly! Fever, cough, aches, rushed to hospital with a 40 degree fever, was put on antibiotics and fluids and it cleared up a bit after 5 days in isolation on the infectious diseases ward, but took a couple of weeks to fully recover. Still strange to not know what it was exactly, but it was quite similar to Covid-19 so feeling strangely prepared. Thanks for the great videos Dr. Hope! Great explanation by this Dr Steve Jenkins.

Dear Dr Hope; you know that these mega crises are fought at individual level (warriors) and 'command'. eg for the latter comms, strategy. After watching ICU videos from Italy I just want to say that Dr Hope shouldn't feel too obliged to shoot, edit, and post videos if it means he can't have proper wind down time. This gave me PTSD from my second med school year clinical skills test they had us do all of those history steps in 5 mins and in the end there was a very loud exploding bell sound to signal the end of the history taking station so we had to move on to the examination station and do all the examination in 5 mins and so on, man my stomach hurts just remembering the amount of stress

Also a good reminder to us with limited medical knowledge to never walk into a hospital with mild but extremely transmissible symptoms. Thank you for this. As someone who ended up in an ambulance and A&E with breathing problems and suspected Covid-19 on Monday, was diagnosed with pneumonia and pleurisy on Tuesday and was prescribed antibiotics ‘just in case’, it is really reassuring to see everything I experienced explained calmly and sensibly. After 4 days of antibiotics, the pneumonia is still in both lungs and I’m still getting stabbed in the sides when I take a deep breath, but I’m holding my own. Whether this was caused by Covid-19 or something else, I’m fairly confident I can beat it. The pneumonia after swine flu was way worse than this.

This Monday I was still studying in the uni library, and now we can't gather in groups larger than five people, and everything but vital shops such as apothecaries and supermarkets are closed... Same here. I'm living in the world's 4th most populous country, indonesia. There are currently twice death toll than recovery, and even the world admits that our medical care is shit. Yet, the government is doing rather iffy job to control the spread since people are still allowed to be up and about in known infected areas. I get the economic problems it might cause, but their attempts at raising awareness is still kinda pathetic, people still don't get what 'social distancing' means and still hold gatherings both small and massive here and there.

I've read reports in US that there's patients in their 30's and 40's dying from the virus. When up to 40% of adults are obese and 10% have type 2 diabetes. Verry sad indeed.

@Pink Freud I found your comment very interesting as you pointed out a new information there ( as I was wondering )that people in 30's to 40's are the most affected & have the highest death rates !!!? Why nobody's talking about this !? Have they !!? I've been watching news on you tube everyday as well as vlogers on you tube who live in Italy , U.K.,U.S. Japan so on ! And I haven't gotten that info .! I am Japanese who has been living in U.S.for over thirty years ! The thing I can say of Japanese aren't affected that bad despite of having lots of Chinese people living there & being so close to China geografically is we are super clean when it comes to the hygiene ,we naturally normally wash our hands like 20 times a day ! Lol.WE ALL DO ! LOL.Even at restaurants ,many of them give us hot towles ,or disposable towles to wipe our hands before meals ,that's been going on for like 6 decades there . And we take a hot bath everyday there ! ( wash our bodies 1st ) That boosts our immune systems & get rid of lots of stress I bet ! As you know stress is bad for our immune systems, even though Japan has the highest ? Or one of the highest suicide rates there ,I bet our customs are preventing many more of the chances ! Also keep our body temperatures warm is very important as losing 1°c ,or 1 °f affects our immune significantly ! We also clean the surfaces of tables ,counters squeaky clean almost everyday before each meal & restaurant business people normally do so each time before / after serving customers ! If you'll visit there ,you'll realize how everything sparkling clean there ! The glass doors of any shop ,restaurants & cafe ,the windows of cabs ,trains, busses ,so on .And we obey rules like robots ! LOLLOLLOL And we are very health conscious, that is why you rarely see people are obese there !!! So, when it comes to health care, a pandemic disease ( we are also very precautious ), like this ,we take precautions very very seriously ! And obeying rules is a piece of cake to us ! LOLLOLLOL. I think these are the main reasons how/ why they prevented the spreads over there & minimized the situations ! The place this Corona virus spread a lot in Japan was a north island Hokkaido I heard where a lot Chinese immigrants live nowadays .

As someone who had pneumonia in my teens and had to be hospitalised because of it, I have great sympathy for those suffering right now. I also love how so many professionals are willing to speak to you at length, even though they must be so busy all the time, but with added pressure at the moment. Thankyou for keeping at it. Working at 111 I'm hearing dozens of cases... Its mad. Edit. We have a lot of mild cases desparate for a swab on the phone, we are very clear on not going to a hospital... Also you mentioned a peak flow meter, as a long time asthmatic I think I've learned to cheat them, I practiced on one I was given and practiced so that I could eject all the air in my lungs in 1/4 seconds...

Luckily we aren't at a hospital but unfortunately we are at a call center with 100 people in the room... Hopefully we'll be able to work from home soon as I'm mildly asthmatic.

Are the people calling 111 getting tested eventually or are they recovering without being confirmed cases? If they aren't getting tested eventually then our numbers are terribly inaccurate

@@justfornow3575 yeah the only ones being tested are the ones admitted to hospital. The reasoning as I understand it is that you treat a cold/flu the same as you would Covid-19 however if you start getting breathing issues, often around the 5-7 day point then that is the time to be more concerned.

We phoned 111 last week since my dad was showing all the symptoms. He was told to stay home for a week and the rest of us are self isolating for 2 weeks under government advice... After a week in bed, my dad is finally feeling himself again. He still has the cough but not as severe. Who knows if he had it, but its definitely for the best to stay home if you have mild to flu like systems.

I'm finding this series incredibly useful - and in a weird way very reassuring. I'm in an at-risk category and my local hospital trust (in London) is no longer accepting Covid-19 patients. Just by seeing how the medical professionals will approach it gives me a lot of calm confidence about the treatment I will receive, should I contract the virus. Even more importantly, possibly, is that it's allowing me to prepare myself and my reactions to the illness should I contract it. It's letting me think a lot further than a simple panic response. So thank you. What you're doing is appreciated.
