When I was 47 years old I figured I needed to change my lifestyle, so I quit smoking two packs of cigarettes a day and a case of beer a day. I stated studying about health and now at the age of 70 I'm a lot healthier than when I was 47. I made my mind up I wanted to live and also I wanted to not feel old and be able to do things beside sit around watching TV.

Doc I have started taking Niacin 500 mg from 10th Dec, my HDL was 28, now on 14 Feb it has improved to 48, at the same dose. Tgl 117, and LDL 95.

I use Niacin, I don't care for the flush effect so I cut the pills into 4 parts and take it throughout my days. It has made me feel warmer. Thank you🌸💐🌸. I have Doc visit in June so I will be getting blood work then. I will be back to give my report. I hate statins, all give my pain in my muscles, I'm supposing I am sensitive.

Just one tweak to your comments about the KZbin video of Dr Bill Buten an author of AIM-HIGH and Dr John Chapman, who were interviewed about the Study. Dr Buten waffled mightily on the niacin results. It was Dr Chapman (not involved in the Study) who made the full-throated defense of niacin and the likely over-interpretation of negative (red herring) conclusions of the Study. I noticed a very significant level of flushing depending on the number of hours fasted when taking 500mg niacin. With food nearly no flush. Taking at 3 days in to a water fast, you would think you're going to self-combust. I would not recommend without kowing how days 1 and 2 effect you. My HDL is slowly increasing but no signs of my LDL decreasing (no statins for 1 year)

John Lorscheider I generally use a cold pack or frozen vegetables on my face and arms when I take niacin. If I can keep my face from burning the rest is not bad. It usually only takes about 5 min with the bag of frozen veggies to be tolerable. I do have a question in regards to exercise and niacin. When I take it before I exercise I seem to get low blood sugar or bonking. Is that normal?

My husband has been using NR for several years instead of niacin, but even with a statin his TG was too high, and his HDL too low. IS NR supposed to affect TG and HDL the way niacin can, and is it ok to take both?

Ford Brewer MD MPH , my earnest contribution to you: you’ve had helpful responses and some good videos. The newly adapted adding several paragraphs to your signature begging to be more popular on KZbin is in bad taste and it is the first time in over ten years I have seen it. Instead, make the quality so high and it will happen naturally. You’ve shown improvement investing time in learning to use the incorporation of the digital screen slides which is more professional and will save you more time, money, and potential air pollution than printing and showing the camera. You might also question if the extra bradykinin from the ramipril (which happens to increase inflammation) is worth it when you could swap with potentially better once a day liver protecting, PPAR activating telmisartan or twice candesartan you are focusing on being a presenter especially as you plan on monetizing the educational videos in the future. Can also save our time with the ”brief introductions” and spelling your name when it is right there for us to read and telling how you went to Hopkins to run the place time after time in most videos. One video is enough on the KZbin and prevmed homepage.

Interesting analysis. Thank you. I’m on rosuvastatin and ezetimibe, but my liver can’t tolerate ezetimibe at full dose, so I’m currently taking a 1/4 dose. I’ve been considering adding niacin even though my cardiologist basically said it’s of no benefit.

Thank you Dr. Brewer. I recently started taking Crestor 5mg daily. I also take Losartan 5mg daily. I have been taking niacin timed released 2000mg daily. My last 2 fasting blood sugars were 103 and 104. So I believe that I am at least a prediabetic and as such have insulin resistance. I am perhaps 10 lbs overweight at most. I walk 2 miles daily. Is there anything else I might do. Will be 70 years old next birthday.

What dose of niacin should be safe to take for someone with ldl of 250? Does one need to slowly increase their dose to tolerate the flush Is slow release /non flushing niacin effective Excited that this is an option as I can tolerate a niacin flush (I tried up to 250g/day) over the myalgia that statins give me and was struggling to understand the paradox which I guess is a faster short term anti inflammatory effect from statins

The only niacin that works for cholesterol control is pure nicotinic acid. All of the "niacin" brands sold in stores is "niacinimide" NOT nicotinic acid. To get the pure form of nicotinic acid, you have to buy it online at Amazon or other sellers. It comes in bulk powder form and you can mix it with a drink.

The "niacin" sold in stores is not pure nicotinic acid. According to Dr. Abram Hoffer, the "niacinimide" form sold in most stores will not work for cholesterol control. Only the nicotinic acid will work. You will not find this pure form in any store. You have to purchase it online at Amazon or other sellers.

Had my first flush today. Was a bit breathless, (have mild asthma), Started 25 mg, flush came at 225 mg. Flush lasted about 1 hour 10 mins, then 45 mins itching, warmth, redness on legs. After a total of 1 hour 58 mins, my knee legs are still warm, some redness on knees. Felt no peace or well being during or after. Will the above decrease or increase if up the dose? 2 half hours later notice strong appetite, been trying to gain weight! As very underweight with no appetite. Taking it for mental health. Shall I just take it once a day at 225 mg till flush is less strong then increase dose after a week or so? Obviously, I cannot take it 3 times a day at this point as flush was so long....any suggestions, please??

I take around 100 mg four times throughout the day. Sometimes I get a little flush but it doesn't last very long. Ive seen taking smaller doses more frequently is better for absorption

Aug 2018 my triglycerides was 175, hdl 48, ldl 106. Total chol 189 . Oct 2019, triglycerides dropped to 94, hdl 51, ldl 118. Total chol 187. I took 500 mg inositol haxanicotinate , no flush , 3x a day for 2 mths before my 2nd cholesterol test. Today im still doing 500 mg, 2 or 3x /day more regularly.

I seem to be ok using it thus far - I use copious amounts. Obviously if my numbers get to dangerous levels, I will add a supplement. It is also noteworthy that tryptophan can also be converted into niacin by the body- so a daily meal that includes chicken or poultry and nutritional yeast should keep this old gal in niacin. (I am not taking capsules, I buy it bulk and bread my chicken and fish with it, and I do get a high enough B3 for the flush)

I was once put on niacin for high triglycerides, and didn't have a problem, but I moved and didn't get the prescription renewed. Recently, after several decades, I bought a bottle of niacin that said it was guaranteed to lower triglycerides. It warned against flushing, and I experienced some flushing in my legs, but it only lasted 15 min. The next time I took a capsule, and I got severe flushing all over my body from head to toe, and I was miserable. But I thought I could tolerate it for 15 min. But it lasted over an hour and made me really weak, I cou;dn't even stand. Obviously, I won't take it any more. Anybody know of an effective niacin that wouldn't cause such misery?

I have found as a pharmaceutical researcher, that compounds that inhibit cholesterol can actually weaken the bodies ability to deal with cholesterol after cessation of the medical compound.

Hello Dr. Brewer, I received my test. Triglyceride 1120, hdl 28, ldl 145. I powerlift 3 times a week without suppliements and without cardio. Diet isn't great but not terrible. 41yrs, 5'10 and 220ib. I eat 4-5 x day. Have gout. I don't have any symptoms. I can't understand why my level is so high. I'M going to try niacin, red yeast rice and omega 3 with a lifestyle changes. Any other suggestions would be great.

@@PrevMedHealth It was from my yearly uric level blood test for gout. I added cholesterol test with it. What is RFM? 26% bodyfat. Waistline 36 I was eating 4500 to 5000 calories per day from eggs, chk breast, pork, veggies, pasta, rice, and beers on the weekends. Doctor wants to put me on meds. I want to try changing my workouts and eat 2 meals within 5 hours. Omega, red yeast, and niacin supplements. Is my triglyceride too high to fix my levels without taking statins? I have been walking for 50 minutes with 45 minutes of heavy workout this week.

@@PrevMedHealth Looking forward to it. I was wondering what it burns up in the process in methylation ? When I took glucosamine without calcium, I got arthritis instead only to find out I needed calcium as a fuel to burn for it to work. I tried NAD Tru niagen for 5 days stopped a month, then tried it for another 5 days but found my energy reduced. So I suspect like 3/6/9 omega, there is some ratio for it to work ok and must use something up in process. Since then, I use it 3 to 5 days, then stop for 3 months.

I have a question. Can niacin cause blood vessel breakage or weakening of the connective tissue or blood vessels. And can things like citrulline/arginine or niacin, beetroot have “blowback” effects. I mean after we take these can we have an opposite effect after it wears off? Will it cause a blood vessel constricting after the fact? I’ve taken some niacin (500mg) and started busting blood vessels again. Could be other stuff I’ve done but looking to narrow things down.

John Lorscheider thank you. I was sick recently and started doing an uptick of fresh Garlic, ginger and a little clove. But also niacin and red wine. , My red blood cells typically runs low to low normal the last this was happening was Garlic, cinnamon, ginger and magnesium sulfate. At this point it has to be the I’m thinking fresh Garlic and alcohol. 😢. I thought I had a pes aneserine bursitis , then a bruise appeared a few days later just under the inside of the knee. Rested it some. Rode my bike last night , and noticed a bit of aggravation. Then today my calf, and food are swollen. Local Dr said take ibuprofen and elevate and ice. Ok that takes care of the temporary problem, but because of the connective tissue and blood vessels are the underlying cause I guess, I’ll keep looking.

John Lorscheider very interesting list. I take no medications. I knew a few of these cause it, and found a couple more that are in my diet. I saw ginkgo, and I do avoid that, but would ashwaganda be on that list, I do supplement with that? Thanks for your help in advance. it's interesting what you said about crashing carbs ... I did an experiment on myself where I did a low-carb diet for several months and tried really hard to keep my blood sugar under 100 and it obliterated my cholesterol and my triglycerides :-) ... I ate lots of bacon actually.... And even when I'm not on a low-carb diet I try to eat less than a hundred grams of carbohydrates per day...just because I have a family history of diabetes and I don't agree with the nutritional guidelines for carbohydrates...I think they have to be wrong... if they're not wrong why in the world is everybody getting diabetes.... Or I guess said another way, the carbohydrate levels are not appropriate as they don't match most people's activity levels.... Most people don't need two or three hundred grams of carbohydrates per day... I suppose...

First, Thanks for all the info in your videos. Tremendously helpful. I started taking Niacin about a month ago (Slo-Niacin Brand - Time Release) and saw a significant rise in blood sugar throughout the day - from around 95 to 130. (2G of Niacin a Day, 1g in AM and 1G in PM). If have the freestyle LIbre CGM, so feel like relative rise is real. How long for this effect to abate ? Is Niacin just not for everyone ? When is discontinuation advise ?

It’s usually 2-4 months. If it goes to 6 months, there is probably something else going on. Have you actually alternated days/weeks? It’s surprising to hear that amount of change on a daily basis.

I titrated up quickly (1 week). Perhaps that would explain some of the response. Will go back to 1g a day (500mg am/pm) and see if this helps and then try to titrate up from there.

When statins is supposed to prolong life but then blocks CoQ10 and causes heart attack, the very thing it's supposed to prevent plus other side effects like muscle atrophy, cancer, sleep problems, ED, dementia it's common sense to stop it. Yes. Very few things are harmless. I wouldn’t take The 2 most important cv supplements (Vit D & Niacin) if they weren’t needed.

Dr Brewer, thank you for another excellent video. So, my recent reading of a meta-analysis of several clinical studies leads me to the conclusion that Niacin can contribute to diabetes. As a diet-controlled T2diabetic this concerns me. And despite having a good A-1C, I wonder if taking Niacin could cause problems for me if I took it for lipid improvement. It looks like a trade-off. PS...I’m working on losing 12 pounds of too much me but my trigs are very low and my other lipids are where they should be, although my LDL was a bit elevated last time. Because of co-morbidities ( hypertension as well as T2, ) my very good PCP has referred me to a cardiologist next month for a full cardiac screening “ just to be sure. we aren’t missing something .” I’m going to ask him for a CIMT and possibly a GTT.

Thanks for sharing that. The OGTT is very good. Another consideration is a full Kraft Insulin Survey. That depends on your situation. The increase in glucose is minimal, temporary & outweighed by the benefits. Glad to hear you have access to good care. So few do!

Hi thank you for all your very informative thorough videos. A Naturalpath told me to take 6000mg of niacin for anxiety. Six thousand seems way too high. Is niacin effective for anxiety and what dose?

I agree that sounds like a big dose. I’ve heard of that before, re: mental health. But I haven’t seen the evidence.

Yes. But only temporarily. The benefit outweighs this.

@@PrevMedHealth I agree about the benefits and I am taking it daily. I am trying to solve the mystery of why my FBS in the mornings stays slightly over 100 when I am eating less than 50 grams of carbs per day and my a1c is 5.1.

By what percentage I wonder? I take chromium polynicotinate (as usual) less than 4.5 mgs worth of nicotinate though, the evening before fasting tests. Never thought about it. Fastings run a bit elevated, 98-105.

I accidentally got 500mg niacin capsules last weekend, they are over 3000% RDA. Sunday I had the flush from hell after I took my first capsule, but i found that if I take my niacin immediately after a meal the flushing is much less. I would suggest 100mg, there may be some flushing, but it will pass quickly. The flushing can scare a person if they have never experienced it, it feels like a serious allergic reaction, but it will pass.
