മലാശയത്തിനോ വൻകുടലിനോ കാൻസർ ഉണ്ടെന്ന് സംശയമുണ്ടെങ്കിൽ ഏതൊക്കെ ടെസ്റ്റുകളാണ് ചെയ്തു നോക്കേണ്ടത് എന്നാണ് കഴിഞ്ഞ വീഡിയോയിലൂടെ സംസാരിച്ചത് അതിന്റെ തുടർ വീഡിയോയാണിത്.
എല്ലാ സ്റ്റേജിലും ഉള്ള കോളോറെക്ടൽ കാന്സറിന് കീമോതെറപ്പി ആവശ്യമില്ല വളരെ ചുരുക്കം കോളോറെക്ടൽ കാൻസർ രോഗികൾക്ക് മാത്രമേ കൊടുക്കാറുള്ളു. കീമോതെറാപ്പി ആവശ്യമുള്ള രോഗിക്ക് DPYD എന്ന മ്യൂറ്റേഷൻ ചെയ്യാനുണ്ട്. കീമോതെറാപ്പിയുടെ പ്രവർത്തി കഴിഞ്ഞാൽ കീമോതെറാപ്പി നമ്മുടെ ശരീരത്തിൽ നിന്ന് പുറത്തു പോവേണ്ട ഒന്നാണ്. ഇവ ശരീരത്തിന് ദോഷമല്ലാത്ത പഥാർത്ഥമായി പുറത്തേക് പോകുവാൻ വേണ്ടിയാണ്‌ DPYD മ്യൂറ്റേഷൻ ചെയ്യുന്നത്.
ഇങ്ങനെ കീമോതെറാപ്പിയുടെ പ്രവർത്തനം കഴിഞ്ഞിട്ടും ശരീരത്തിൽ തന്നെ നിലനിൽക്കുകയാണെങ്കിൽ വയറിളക്കം, കൗണ്ട് കൂടുക, പനി എന്നിങ്ങനെ ഒരുപാട് സൈഡ് എഫക്ട് ഉണ്ടാക്കും. ഒരുപക്ഷെ കീമോതെറാപ്പിയുടെ സൈഡ് എഫക്ട് കാരണം രോഗി മരണപ്പെടാനും സാധ്യതയുണ്ട്. അതുകൊണ്ട് തന്നെ കീമോതെറാപ്പി എടുക്കുന്ന എല്ലാ രോഗികളും ബയോ മാർക്കർ ടെസ്റ്റ് ചെയ്യേണം. സ്റ്റേജ് 4 കാൻസർ ഉള്ള എല്ലാ രോഗികളും മോളിക്യൂലർ ടെസ്റ്റ് ചെയ്ത് നോക്കേണ്ടതാണ്. അതുപോലെ എല്ലാ രോഗികളും MSI ചെയ്ത് നോക്കണം രോഗം നേരത്തെ കണ്ടെത്താനും കീമോതെറാപ്പിയുടെ പ്രയോജനവും കണ്ടെത്താൻ MSI ക്ക് സാധിക്കും.
ഇതൊന്നും കൂടാതെ രോഗിക്ക് ചികിത്സകൾ ചെയ്യാനുള്ള ആരോഗ്യം ഉണ്ടോ എന്നറിയാൻ പലതരം ബ്ലഡ് ടെസ്റ്റുകളും ചെയ്യേണ്ടതുണ്ട്. മലദ്വാരത്തിലാണ് അർബുദമെങ്കിൽ റേഡിയേഷൻ ചികിത്സയാണ് കൊടുക്കുക, വൻകുടലിലാണ് അർബുദമെങ്കിൽ പൊതുവെ റേഡിയേഷൻ ചികിത്സ ചെയ്യാറില്ല.
കാൻസർ മുഴയുള്ള സ്ഥാനം രോഗത്തിന്റെ സ്റ്റേജ് എല്ലാം തിരിച്ചറിയേണ്ടത് അതാവശ്യമാണ്. അതിനായി കൊളോനോസ്‌കോപ്പി, MRI , ബയോപ്സി, ബയോ മാർക്കർ ടെസ്റ്റ് , മറ്റ് ബ്ലഡ് ടെസ്റ്റുകൾ ഒക്കെ ചെയ്യാനുണ്ട് . ഇത്രയൊക്കെ ചെയ്താലേ കോളോറെക്ടൽ ക്യാൻസറിന് ഏത് ചികിത്സയാണു നല്ലതെന്ന് തീരുമാനിക്കാൻ സാധിക്കുകയുള്ളു.

It's important to note that chemotherapy isn't needed for every stage of colorectal cancer.
Indeed, just a minority of colorectal cancer patients are recommended for this treatment. Specifically, those with a DPYD mutation might be deemed unsuitable for chemotherapy. The rationale behind this is simple: post-chemotherapy, the body should effectively and safely eliminate the treatment compounds. The DPYD mutation aids this process, ensuring these agents are safely expelled.
However, if remnants of chemotherapy persist in the body, patients could face severe side effects such as diarrhoea, elevated blood counts, and more. In extreme cases, these effects could even be life-threatening. Hence, biomarker testing is crucial for all patients slated for chemotherapy. Those at stage 4 should additionally undergo molecular testing.
Comprehensive screening for MSI is equally paramount, as it can highlight the potential advantages of chemotherapy, especially in early disease detection. Beyond this, routine blood tests ensure a patient's holistic health and readiness for subsequent treatments. Treatment modalities differ based on cancer location: while anal cancers typically receive radiation, colon cancers usually do not.
Determining the precise location and the cancer's stage is paramount. Diagnostic procedures like colonoscopy, MRI, biopsy, biomarker tests, and a suite of blood tests provide this insight. With all this information, we can best decide how to treat colorectal cancer patients.
