I found this video to be exactly what I was looking for; a how- to plus illustrations of what is happening in the canals with each manuever. I wish the canals were labelled in the sitting to supine positions. This video is a great resource for the practicing clinician who treats BPPV patients.

After researching the 3D model of Vestibular portion(semicircular canals), I come up this procedure to treat SUPERIOR or ANTERIOR canal BPPV Vertigo. 1. lay flat(face straight up) on the bed(edge of the bed) for 1 min 2. lay head hanging(head top toward ground, face toward horizon) for 1 min 3. fast rotate whole body 180 degree, head straight, face toward ground for 1 min 4. sit up or stand up fast, face horizon.

I am not a medical person. I just know I get dizzy as all get out when laying down enough and looking up in certain ways. To me, this video is so full of jargon that I'd need to look up many of these terms to understand them to be able to better understand the video. Is there a layman's terms version of this?

khusbu khushi, this appears bppv. my wife had similar condition for 6weeks.we had various medication with little improvement. my daughter who is doctor performed epley manauver. she is now completely cured.

But therez no sincere doc available anywhere who sincerely get u into the real cause of vertigo , just by giving medicine doesn't resolve any issue, which just merely dependent on physiotherapy treatment like different maneuvers for different types of bppv , luckily some people is able to get back to their life after this horrible vertigo.

i'm 33 and have been suffering with BPPV for a month now. first few days were terrible and vomitted a couple of times doing the exercises. Sort of went away after a week of rest, but i'd still get vertigo strangely when i laughed or had a poo. It was annoying me, so i tried to do more exercises to finally get rid of it, but then triggered it all over again and i'm back where i started. doctors in the UK are useless. Right now i get vertigo when i walk, move my head up or down and when i roll over in bed. it's frustrating more than anything, because i feel healthy but just so effing dizzy.

@@justme-j9t So much better thanks. I feel bad for saying doctors in the UK are useless. Eventually your brain recalibrates your balance for you. I get vertigo spells in the dentist chair still and when I roll over in bed, but I just shake my head a bit and it goes away luckily. Hope you are alright.

suppose you do this to a patient who is having vitiligo caused by Menieres? Also how can the physician tell from looking if it’s BPPV or Meneries vertigo? Is there any treatment to stop Menieres vertigo other then drugs?

Are the diagrams correct? At 1:33 and 8:08, the patients' heads are oriented the same, but the image of the canals seems to have a 135 degree rotational difference that I haven't been able to explain.

Yes -- the mistake seems to be at 1:13, where the head is rotated 90 degrees, but the canals are not rotated the same amount. The error then propagates as the head is rotated 180 and the canals are rotated 180. I think there is a similar error later on in one of the other sections, but not as dramatic.

Actually both diagrams are correct. The first one is the situation in canalolisthesis (free particles in the canal); the latter depicts the situation in cupulolisthesis, in which the particles initially are attached to the anterior side of the semicircular canal, and the movement makes the attached particles free.

OK question. Once these crystals get moved into the utricle is there a risk that they could find their way back into the same or another semi circular canal? Or do they dissipate somehow? Also, what is the chance of recurrence?

Gti Driver I’ve been plagued with it multiple times over the past 2 years. But every time I’m able to get an appointment, it goes away. I’ve never been able to be seen when I’m having it. I wouldn’t wish this on my worst enemy ☹️

Thanks for sharing this information! Quick question, how does one determine which side to treat if they have horizontal canal? BPPV? When I lie down doing the supine roll test my nystagmus eye movement is from side to side beating in an upward direction towards the ceiling, on both the left and right one does seem to be more rapid than the other. If my Nystagmus seems to be worse on the right side for Example, would I then lie down on my left side OPPOSITE, doing the Gufoni maneuver? This is where I seem to get confused on which side to apply the treatment maneuver. Thanks in advance for your help.

So I was reading that this maneuver is not helpful if the crystals are on the horizontal canal, it's only use for posterior canal. If it's on the horizontal canal then the log maneuver is helpful

Good use of 3D graphics, but when the narrator says eye movement “to the left” it seems like it should have been from a patient’s perspective (I.e. to the right). Or am I missing something?

These are good videos but you would have to be treating patients regularly and often to remember all these manoeuvres. I treat these patients but I can only remember the signs of posterior canal lithiasis and the Epley manoeuvre for this. We need a chart with all these clearly demonstrated on it for reference for each patient. I would do one but I would probably get mixed up and do it wrongly!

I went to numerous different general parctitioners, and finally one identified this as the problem of my dizziness. He was a general practitioner, but a very good one. If you suspect that this might the problem, you can have a friend help you. Lay back, and have a friend turn your head to one side, watching your eyes. In my case, when the doctor moved my head to the left, I felt the world spin; for him, he saw my eyes jerk to the side (watch around the 2 minute mark of this video). After doing the Epley maneuver for about two days (with my wife controlling my head), I was cured.
