There is a bug in v4.0 that you can't complete the last level in underworld because Mario die in off screen after he went to the off screen(I think they forgot to add the level end blocker like in rainbow engine) but they fixed it in v4.02! So I think you're using v4.0

This video was made before MFR 4.02 coming out, also MFR 4.02 just came out because I've reported a bunch of bugs like these to the developer.
