Because parts of a scrapped zone named "genocide city" was combined into it, thus why it's 3 acts long. Also, the name genocide city was because the japan devs didn't know the meaning of the word. If this zone wasn't scrapped, it would likely have gone under a different name in the west.

I HATE this zone! On a touchscreen device, it is almost impossible to not get hit and lose all of your rings. Btw, did you know that SEGA is actually recycling old zones and freshening them up? For example, the Mad Gear Zone is a redone version of the Metropolis Zone. Just thought I would share that. And what is even more is that the boss of both the Metropolis Zone and the Mad Gear Zone are basically the exact same!
