Mirror Box Therapy & NEUROPLASTICITY Following Stroke

Mirror box therapy can be utilized in conjunction with other therapies to help stroke patients retrain their brain, regain function and improve their overall quality of life. When a stroke patient puts his weakened hand in the mirror box, and moves his strong hand, the mirror provides a reflection of the stronger hand, thus giving the illusion that movement is occurring in the hand affected by the stroke. This activates mirror neurons in the premotor cortex of the brain. In other words, the mirror tricks the mind and the weak hand into working better. Studies have shown that patients who have used Mirror Box Therapy show an improvement in motor function and in the rewiring of the brain.
Neuroplasticity refers to the potential of the brain to reorganize by creating new neural pathways to adapt. This reorganization allows for positive functional improvements after a stroke. The brain is more flexible than we've ever thought before and it is constantly optimizing itself. It has the potential to reorganize itself by transferring cognitive abilities from one area to another. After a stroke, for instance, your brain can reorganize itself to move functions to undamaged areas. Mirror Box Therapy takes advantage of this potential.

Thanks for posting this, it is very helpful for those of us working in neuro or chronic pain . I really like giving this to patients when they are discharged to go home, and have over a year to take advantage of neuroplasticity. I would love an update to see how this fellow is doing now!

This gentleman was a good sport letting these OT’s film him! Really cool mirror box therapy. I read about this when I was getting ready to go to OT School myself. However, at the time, it seemed to be used more for phantom limb syndrome after amputation, so very cool to see it used in post-stroke recovery. Two thumbs up, literally! :)

@@nathaliafaria1177 One can use direct observation with passivity in the affected limb for mirror neuron activation alone, but I like using the mirror box with stroke patients to have them more directly imagine the affected limb moving and trying to mimic the movements their unaffected limb is making to work on bridging the gap between mirror neuron activation and the 'wiring' of direct use. Like a difference between passively observing the limb is moving, and showing the brain that an order has been given to complete a necessary task. Similar, but different in an important way. Despite the activations that are elicited by mirror neuron firing, I want a top down order being given to the brain for the use of the affected limb. I will give homework which goes through a range of sensory and propropceptive activation, passive movement of the limb, attempts to move the limb directly, and mirror box work, etc.

I thought the same! But this will work when the patient will focus completely into the mirror and imagine that the weakened hand is performing the activity, we need to give lots of verbal feedback, then this is much possible

My Mother had a stroke and lost function in her right arm (she is left handed) I'm trying this out with a jury rigged cardboard version. Stroke outcomes are a unique as fingerprints and snowflakes, so we shall see what happens.

GREAT DEMONSTRATION. I was shown mirror therapy for my brother-in-law and it was a little different. I would like your input if possible. The activity was to open and close the hand from a fist. Instead of just watching the good hand in the mirror w/ the affected hand not moving I was instructed to move the affected hand at the same rate as the non-affected hand (which was being watched in mirror). In other words I was opening and closing the affected hand as he opened and closed and watched in mirror the non- affected hand. Should he only watch-or is manipulation of affected hand at same rate to his benifit? Thank You

your patient here already has a very good functioning L UE. Does this work well for those with flaccid or trace movements? I have used this with patients for posture sitting in front of a mirror as feedback to improve sitting posture/balance.

the affected arm is in the box. you want the "good" arm to be on the outside, which is what you see moving. thats the good arm. the bad arm is in the box.
