This interpretation of an intellect devourer just gave me an idea, what if in the ritual they need to graft the brain onto an already existing creature's legs/body in order to succeed? And the 'canon' intellect devourer was a human's brain grafted onto the body of his beloved dog, (Maybe lion if they were a ranger or druid or something) but more often the brain could be grafted onto any number of creatures in the underdark. Baby versions of cloakers, giant spiders, rust monsters, etc. I think that could give an amazing amount of variety to the creature.

Speaking of intellect devourers, pugs and body horror. I was running Waterdeep: Dragon Heist for a group of friends, this was near the very start of the campaign, and the group had tracked down an abandoned sewer maintenance station that the infamous Xanathar thieves guild had converted into staging grounds for a big heist job. Visiting to oversee the sight on behalf of its aberrant master was Nihiloor, a rogue ilithid (who I converted to a mind flayer arcanist to make things interesting). When the players found Nihiloor in its sanctum, overseeing preparations from his chair at the back of the room, he was wearing the illusory disguise of a hooded human man with his innocuous pug dog resting his lap. Suffice to say it got to the players and characters both to watch as this little pug’s head split open like an peeling orange rind to reveal the intellect devourer now practically right next to the wounded bard at the back of the group’s formation. This design would have made that moment ten times better/worse, point of perspective I guess.

Hah, In my Homegame i also removed the Intellect devourers ability to teleport into skulls. My players are currently in a town of miners. Most of them have bandages on ears or around their head. Some have eyepatches and some have bandaged noses. My players do not know that this means that Intellect Devourers burrowed their way into these peoples skull.

The 5th level spell greater restoration fixes any lowered stats, making it the easiest way to unstun a party member. However any parties under 9th level might have a tougher time healing their brain dead companion

If someone does go with the Teleporting version, I would flavour it as having the new host's brains liquefy and run out occasionally from his ears and nose. But I prefer your version, this crab like thing splitting your head open and devouring the old brain as it gets comfy and closes the hatch if you will.

Ancient Egyptians g took the brains from mummies through the nose. I think it would be cool for the devoured to enter or exit bodies way. Eyes are another access point.

I get what your saying and I got no problem with body horror either but I think the big thing with the intileck devolor. Is the parano fuel. There no way to no if some one is one and if it appends one your never going to trust again. If it makes very visible grotesque entry point or even just something duffle you can learn to check for. Or leave a dead boy behind ever time they do it that kinda takes away. Also give that made by mind flers there probably made to look silly has a on purpose to make you under estimate them or many just to dee cute they are pets to
