I had to tighten one of the two on my bike because it loosened like yours. Now mine work perfectly.

Does it stay exactly on the side of the handlebar, almost as a continuation of the throttle?

Yeah...you can place then anywhere you want and they will stay....you are supposed to be able to move them to 3 different "locked" positions so you can move them out of the way (like the CRG mirrors) but when you do it loosens the set screw and the whole mechanism fails...
