My theory is that the narrator is the voice in his head. That would explain everything. The "Get out of my head", the "Good Dog", everything. The "I'm free" could actually have been the narrator (voice in the guys head) taking full control.

My only advice would to work on your creative phrasing, to say "turned her face into minced meat" after using a blunt object isn't fitting saying " turning her face into a bloody manhole". It keeps that same charm and it fits
