Sir - I had lymph node swelling for which I visited doctor. FNAC was done and gene expert detected mycobacterium with low load and not resistant. I started ATT. After 10 days I went for lymph node excision. Sample was sent for testing and culture. This time genexpert showed very low load but resistant. So doctors started mdr tb treatment. After 6 weeks culture report came. Smear, ZN Stain were both negative and there was no growth in culture after 6 weeks. What needs to be done now. Should I continue with medicine? Also there was contradiction in both genexpert tests with both giving different result for resistant to drugs. Need your valuable suggestion

First of all .... Excision of lymph node is not suggestible as per me when active Tb treatment is ongoing and that too atleast not in first 2 months Contradictory gene expert reports are never considered as evidence for starting MDRTb treatment. That too with MTB detected Very low rifampicin resistance cannot be reliably commented upon in very low status. Now cultures are negative !!!! We will need Dr Ankush sir opinion in this regards. I am still experiencing cough and breathlessness after recovering from pulmonary Tuberculosis. The chest specialist here said that i would have to use inhaler for the rest of my life. I did not want to be on an inhaler for the rest of m life. So i decided to do yoga and improve. It helped a lot. But i am easily susceptible to any weather change. Doctor you think i should repeat my ct scan to be on a safer side?

Your information is partial. Cannot comment. There are various types of Culture. There are various types of Cbnaat Without seeing reports and its clinical utility with patient hisrory, then only a remark can be made

Post covid lung changes are usually reversible in 3-6 months mostly. Very rarely can also reverse even upto 1yr. Any changes left after that are usually not post covid and maybe due to other diseases

Good morning doctor I was recently diagnosed with pulmonary TB. I had scar in my chest x ray . Doctor asked me if i had TB in the past and if i took medicines. I said No. He asked to do CT scan,post which he said i have TB and prescribed medication. I didnt have any symptoms of TB such as weight loss, loss of appetite or cough. I suffered from bacterial pneumonia same time last year so when i got slight chest pain, i wanted to confirm if everything is ok so consulted a physician and requested for chest x ray in which scar was identified. Since i dont have cough and was not getting sputum, doctor adviced me to stop the medication and do bronchoscopy after 4 days. My geneexpert is positive, AFB negative, BAL culture has no pathogen growth. I am 69 kgs and i was given Akurit 4 - 5 tablets each day and benadon 1 tablet. When i took the tablet for the first day i felt severe eye pain and headache. It was very difficult to manage.Kindly advice if

I was in Chennai for diwali when I met my first doctor. He confirmed TB after ct and gave medication. When I asked him if I am infectious because I have a 4 year old child, he said I am not infectious and I cannot spread infection to anyone as I don't have symptoms and my immunity is fighting with the bacteria. However, I need to take medication and asked to send x ray after a month as I was travelling to Bangalore. He just said send geneexpert test result to him. Since I didn't get sputum I had to consult another doctor here in Bangalore for further plan and hence had to go through bronchoscopy. When I took the medication which the first doctor gave for 3 days, I didn't have much side effects except the headache but here when I took Akurit 4 - 5 tablet which is 1 gm higher dose than the previous dose I was unable to tolerate so requesting your advice.

Hello sir ,my mother in law montex test is positive ,genxpert and sputum afb are negative .but some kochs are there in lungs .doctor suggested first line dots medicine . Any other tests are required to rule out mdr .

Kindly do not give Tb medicines like this without diagnosis. Gene expert is negative you said, then on what grounds your doctor started medicines. Please consult with dr ankush sir on 9958253661 with apoointment so that appropriate diagnosis can be reached and then treatment can be given. Treatment by just doubt (empirical treatment) on basis of Mantoux test is not given these days

@@Netra2801 its not possible to comment like this mam. "Some findings are there in xray and ct" .... that is a vague statement. Also as told earlier we dont start treatment in our patients without confirming diagnosis first. No lungs specialist would advice TB medicines without confirming. Bronchoscopy is needed in such.cases

Question itself is incomplete .... First thing important is what are the symptoms and then chest xray and ct scan findings Then comes the role of gene expert and other tests. Also gene expert and sputum smear are just few test We also advised for afb cultures if relevant in each case

@@Taurus_0109 Truenat Cbnaat (gene expert) Afb smear Afb cultures All are diferrent tests to diagnose tUberculosis based on diferrent mechanism. Truenat & Cbnaat are very sensitive and quick to detect very low number of Tuberculosis bacilli and also can detect drug resistance. Afb smear is the age old method but is dependent upon number of bacilli present and thus is least sensitive but we get report within 1-2 days. Afb cultures are gold standard meaning the best. However takes a long time to come positive minimum being 7 days to maximum upto 8 weeks.

Agar TB ke baad aap medical fitness puch rahe hai to aapke treating doctor ye certificate de sakte hai sab tarah ke tests karke ki ab patient ko tb nahi hai and is fit for work. Bahut se patients ko hamne certify Kiya hai for govt jobs in india
