I agree being a swing out door on the floor landings, it's equipped with a mechanical/electrical interlock which prevents the door from being opened when the cab is not on the landing. It's malfunctioning here. Elevator should have been "red tagged" out of service.

I always look before I step inside an elevator! If she was paying attention she wouldn't have fallen. She was either on the phone or was distracted by her g'kid. Always pay attention when stepping into an elevator! If it's dark it's probably not there or the light is not working, so it's a no for the elevator. Now if you can see the bricks, the elevator is not there.
