Ani was in a coma brought on by a traumatic event that broke is unwavering world view that he "can do this". The shock was so great and the denial so complete; he fell into a lucid coma or "Dream Crisis". I hypothesize he was sleep walking and his Doctor was paying people to play along in order to help Ani come to terms with the fact that no matter how capable he is he has to accept the fact that some things can't be done in order to accept his trauma and heal. Anti was just the manifestation of his own denial. He even "rejected" his origin story because it involved his trauma which he repressed and rejected. Ani believed he was an Anime protagonist because anime protagonists always win and simply put "can do this!"
