“Never again”: key lessons on medication safety from coronial inquiries

Benita Suckling, Assistant Director of Pharmacy, Metro North, at Queensland Health, hosts a podcast, created in collaboration with the Coroners Court of Queensland, to explore the lessons from real healthcare investigations with a range of experienced clinicians. She talks about important insights after hosting a workshop discussion on preventable deaths at #MM2024AdPha, with a particular focus on the role of bias. Her aim is to give easy access for clinicians “to these lessons that we've learned in the hardest way possible”.
