There's a way from the top when they want to give an Ace to the small blind. A dealer locally does this all of the time. She washes the cards, finds an ace puts it on top. Then she shuffles bottom heavy keeping the Ace on top. Then when she Splits the deck she pulls the middle the bottom and again sets the same top 1/3 of the deck on top. Then (how and when I caught her) she doesn't cut the deck but slides the cut card under while she's talking distracting you. Her boyfriend was in the small blind every time. She could only guarantee him one Ace, but yeah, she did it.
