You have no idea. Some of the regular songs in the first Theatrhythm are insane on Ultimate Difficulty... but the DLC. The DLC songs can make you swing your stylus like Light from Death Note. Your wrist muscles will get a work out... heh. I hope this game is even more difficult.

it's going to break your wallet more than your hand if you try playing it first theatrythm game: nintendo announces dlc>immediately theathrythm>full retail price and day 1 dlc>1year later 10$ ios release

The Idiot-OS version is like 5% of the game and 95% DLC. The original 3DS version is a very full game that would take you 1,000 hours to truly 100%: please try and 100% it, I dare someone. It also had DLC but it wasn't an evil scheme like all of Squeenix's Idiot-OS games.

Eh, what is so different compared to the original Theatrhythm Final Fantasy? I see a couple of new songs, and basically a "real" story mode but, as much as I love FF and its musics and Osu-like games, there's nothing that could justify to pay another game "just" for that (unless this game has an according price)

Game is claiming to have 221 songs in it from the series. The original had 70, plus another 50 or so from dlc. So assuming this isn't full of crap, at bare minimum there should be about 100 extra songs. My wonder is how they'll do that since 70 was supposed to be pushing the limit before. That might end up being a whole ton of dlc songs.
