The song is about heroin use, heroin being both a "friend" and a "known enemy". "Take your time, hurry up" refers to the anxiety he feels before using it. "Doused in mud, soaked in bleach" are references to a dirty and clean needle, the gun is also a reference to a needle.

I can and do enjoy this song without analyzing it, and know KC wrote this one and “Smells ..” before he was famous, but maybe because it was the big story everyone heard about his life and the tragic end, I hear these lyrics and Smells as being contemptuous and confused of a certain kind of audience, and as Enoma^ said the people who surrounded him after success. I understand the contempt. Many people, and I include myself in this, who cannot create great music to furnish to themselves can be so demanding and greedy of others who can, because they are dependent on these talented people for that amazing joy to the point of insanity for the performer. Many musicians may not have realized how much of a dancing monkey they would have to become after getting signed. We become addicted to great art and new ideas, but they are not our own, and there is a certain jealousy/love-them-to-death sort of thing. Respect to the respectful whom share in good music, not consume it. The grungers are not the only ones who have talked about this, Pink Floyd and Pete Townsend both have spoken about the industry a lot, Pete called Rock and Roll a big fire that keeps burning on and on and looks great until you realize it is fueled by bodies. Such is life, and art. And it has all changed now anyway. 🤷🏻♀️ Can’t say I’m not glad we have the music.

i guess you could call it bass guitar but the iconic riff is actually played on a normal guitar in a lower tuning. most will say drop d, but i think it is debatable. the guy that actually did it (cobain) isn't here any longer to tell us, but it is surely played on a guitar that is in a non-standard tuning.

Not to be that guy, but it was recorded with the entire guitar tuned a whole step down from standard tuning (D-G-C-F-A-D) A very common non-standard tuning.
