If you wonder why he can walk around freely: He is a foreign diplomat from Indonesia. He works for the embassy there so he didn't have to be with a government tour guide. He basically lives there. Moreover, Indonesia has a pretty good relationship with NK since we are not part of NATO and has built a special diplomatic relationship since 1955 called the Jakarta-Beijing-Pyongyang Pact.

The West forced North Korea into isolation. And no, in North Korea you don't get killed for minor stuff like "breathing wrong", don't be ridiculous.
