In the late 1970s IBM did a hush hush audit for the Los Angeles Dodgers. The team felt that ticket sales revenue was being siphoned off but had no proof. A team of IBM systems engineers worked weeks before uncovering and understanding the scam. The Dodgers fired many people in the ticket office. But kept the theft from MLB because they would have had to do a complete redo of the books and write checks to all the other teams for shorted revenue. Baseball teams split revenue of home games 60-40 with the visiting team. Theft was in the millions.

I did consulting to small business owners for 10 years. 80% of them had no idea how much was in their checking account and had zero internal financial controls. The number one flaw was not having spending limits for employees. I worked with a company that almost went under due to outright fraud by the book keeper. The D.A. told him he'd pursue charges, but given that he had awarded her bonuses annually and sat 30 feet away from her in the same office for years, a jury probably won't rule in his favor. The kicker was that after he elected to not press charges after she was fired, her attorney contacted him and said he needed to work him to move her well funded 401k out of his management. How the guy never had a heart attack was beyond me.
