How to Administer a Blood Transfusion- Packed Red Blood Cells

This video will take you through the steps of starting a blood transfusion (PRBC's). The most common blood transfusion reactions are: fever, chills, urticaria and itching. The signs of a more serious transfusion reaction are: respiratory distress, low blood pressure, high fever and dark urine. Among the signs of an allergic reaction are hives, rash, itching. The signs of an anaphylactic reaction are usually: flushed skin, hives, itching, swelling, difficulty breathing, wheezing, blue lips, vomiting, diarrhea and low blood pressure. A febrile non-hemolytic reaction is the most common, and occurs when the receiver’s white blood cells responds to the donor’s blood - in that case the temperature will usually rise to 38C/100.4F so fever and chills will be experienced. Sceptic transfusion reaction occurs when the blood is contaminated, and usually include these symptoms: fever, chills and low blood pressure. An accute hemolytic transfusion reaction is rare. It occurs after about 24h when the donor’s red blood cells are being destroyed. Symptoms include: chills, low blood pressure, flank/back pain and red/brown urine, renal problems. A TACO reaction is when there is a circulatory overload - a pulmonary oedema occurs. Symptoms include: tachypnea, coughing, shortness, tachycardia and high blood pressure. A delayed hemolytic/serologic transfusion reaction occurs after 1 day to 4 weeks after the transfusion. Symptoms include: fever, jaundice, abdominal pain, dark urine, high blood pressure and dyspnea or labored breathing. Check your nursing books for more accuracy. If you witness any of these symptoms, stop the blood transfusion STAT and notify the MD. It is crucial to monitor closely when administering blood products and ask the receiver to notify the team quickly if they experience any of these symptoms. If the doctor order says: check hemoglobin and hematocrit post one hour of blood transfusion. Do we put the blood transfusion on hold after one hour to carry out this order? And what if the patient hematocrit and hemoglobin value is low still?

This video is excellent! Thank you. However, question: Regarding packed RBC expiry date, you mentioned that the lab changes the expiry date?? That would mean that we would potentially be giving expired blood??

You must never give expired blood. I believe you are referring to how much time the blood is allowed to be out of the blood bank. Once out of the blood bank fridge, the blood is only good for a certain amount of time, because it can develop bacteria. I hope this answered your question.

The Baxter Clearlink system you were using as a "gravity set" was actually designed to go in the Baxter infusion pump.

@@NurseErica The dark blue little slide clamp that looks like a key. The Baxter pumps don't have fixed segments like the BD-Alaris system. The dark blue pump key is above the light blue roller clamp on all Baxter tubing. You should look up the Baxter Sigma Spectrum IQ system. That's the pump I use in my simulated operating theatre for oral surgery.
